[Overview] This herb is also known as Longnao, Longnao Xiang, Meihuanao, and Meipian. It is a crystalline compound precipitated from the resin of Dryobalanops aromatica Faertn.f., a plant of the Dipterocarpaceae family. This is natural borneol, also called “Meihua Borneol,” mainly produced in Sumatra, Indonesia. Most of the borneol commodities in China are imported via Hong Kong. Besides natural borneol, there is also chemically synthesized borneol, recorded in the Pharmacopoeia of the People’s Republic of China as synthetic borneol, also called “mechanical borneol.”
[Properties] Taste: pungent, bitter; nature: cool. Channels entered: Heart, Lung, Spleen.
[Actions] Opens orifices and awakens the spirit; clears heat and relieves pain; brightens the eyes and removes visual obstruction.
[Applications]
1. Used for closed syndrome with coma, locked jaw, and phlegm-induced syncope in stroke; often combined with musk. For heat-induced closure, commonly used with rhinoceros horn, bezoar, musk, etc., in formulas such as Zhibao Dan and Angong Niuhuang Wan; for cold closure, combined with Styrax, musk, benzoin, etc., in formulas like Suhe Xiang Wan; if coma and locked jaw persist, can be ground with raw Arisaema to a fine powder and applied to teeth, as in Kaiguan San. Recently, often combined with Styrax to treat coronary heart disease angina pectoris, as in Subing Di Wan.
2. Used for sore throat, gum swelling and pain, oral ulcers; often combined with borax, mirabilite, cinnabar as fine powder for throat blowing or topical application, as in Bingpeng San; for heat toxin sores and swelling on head, eyes, throat, mouth, combined with bezoar, Coptis, chrysanthemum, etc., as in Niuhuang Shangqing Wan.
3. Used for ulcers, carbuncles, hemorrhoids, and chancroid. For carbuncles and boils, often combined with musk, bezoar, realgar, pearl, etc., for internal and external use, as in Lei’s Liushen Wan; for hemorrhoids, combined with potassium nitrate, as in Shengxue Gao; for chancroid with foul odor and ulceration, combined with calamine powder and applied to lesions.
4. Used for red swollen painful eyes, visual obstruction, itching and dryness. For red swollen painful eyes with excessive discharge or visual obstruction, can use Coptis decoction with borneol added for eye drops, as in Longnao Huanglian Gao; or ground with calamine, pearl, musk to very fine powder for eye drops, as in Babao Yanyao San. For visual obstruction, ground with mirabilite for eye drops, as in Longnao San. For acute fiery eyes and external clouding, combined with musk, Gentiana, calamine, as in Boyun San.
5. Used for burns and scalds; borneol ground to fine powder and mixed with peanut oil for topical application; or combined with calcined gypsum, red clay, and red lead powder mixed with sesame oil for topical use.
6. Used for earache, ear swelling or purulent discharge; borneol ground with Coptis, potassium alum, and dragon bone to fine powder for ear blowing, as in Ting’er San.
[Dosage and Administration] Internal use: incorporated into pills or powders, 0.15–0.3 g; not used in decoctions. External use: appropriate amount ground to powder for topical application; or blown, rubbed; or used as eye drops; or mixed for topical application.
[Cautions] Use with caution in pregnant women and those with qi and blood deficiency. Not suitable for long-term use.
[Modern Research]
1. Chemical constituents: Synthetic borneol mainly contains borneol, isoborneol, and camphor.
2. Pharmacological effects: Borneol or its isomer (isoborneol) and synthetic borneol all show significant analgesic and sedative effects; borneol or isoborneol emulsions significantly inhibit egg albumin-induced paw edema in rats, indicating anti-inflammatory effects, with isoborneol showing stronger activity; borneol, isoborneol, and synthetic borneol exhibit notable antibacterial effects in vitro against Staphylococcus aureus and inhibit Vibrio cholerae; borneol has significant abortifacient effects during early and late pregnancy in animals; borneol and isoborneol significantly prolong survival time of mice under normobaric hypoxia; topical application of borneol mildly stimulates sensory nerves and has some analgesic and antiseptic effects.
3. Clinical reports:
1) Oral ulcers: Qingbing powder (equal parts Indigo naturalis and borneol powder) applied in appropriate amounts to ulcer surface, closed mouth for 10 minutes, 3–5 times daily. Treated 350 cases, all cured within 2–5 days. [Shaanxi Traditional Chinese Medicine, 1986, 7(9):412]
2) Toothache: Mix 100 g each of Semen Crotonis (peeled) and Piper longum with 10 g borneol powder, store in sealed bottle. When used, place small bottle near nostrils and inhale alternately through each nostril for 5–10 minutes to relieve pain. Treated 50 cases with satisfactory results.
3) Pediatric acute tonsillitis: Mix 5 g borneol, 10 g whole scorpion, and 2 ml vegetable oil; crush scorpion and borneol, then mix with oil to form a pill about 5 fen (0.5 g) in size; apply with adhesive tape to Wai Lian Quan (Extra point) externally, change every 24 hours. Treated 46 cases: 21 cases returned to normal tonsil size in 1–2 days, 17 cases in 3–4 days, 8 cases in over 5 days. [Zhejiang Journal of Traditional Chinese Medicine, 1991, 26(7):297]
4) Suppurative otitis media: Soak 1 part borneol in 10 parts rapeseed oil for 1 week, fill ear drop bottle; after cleaning ear canal secretions, instill 3 drops 3 times daily. Treated 82 cases: 77 cured, 5 ineffective; total effective rate 93.9%. [National Medical Forum, 1990, (5):47]
5) Vertigo: Select ear points Shenmen, Brain, Subcortex, Heart, Sympathetic; apply rice-grain sized borneol on 0.5 cm × 0.5 cm adhesive tape to 2–3 points on both ears, change every 3 days; 4 times is one course. Treated 77 vertigo patients: 53 cured, 22 improved, 2 ineffective. [Henan Traditional Chinese Medicine, 1986, (4):14]
6) Insomnia: Select ear points Shenmen, Brain, Subcortex, Sympathetic, Neurasthenia, Insomnia; apply rice-grain sized borneol on 0.5 cm × 0.5 cm adhesive tape to ear points, massage 1 minute, then 3–5 minutes before sleep nightly. Treated 92 cases: 40 markedly effective, 48 effective, 4 ineffective or unclear. [Zhejiang Journal of Traditional Chinese Medicine, 1987, 22(4):158]
7) Cervical erosion: After routine vaginal disinfection and irrigation, apply sterile cotton balls soaked with varying amounts of Bingpeng San to lesions once daily for 6–7 days per course. Treated 171 cases: 144 cured, 17 markedly effective, 6 improved, 4 ineffective; total effective rate 97.7%. Minimum 3 applications, maximum 10 for cured cases.
