[Overview] This herb is also known as Xiang Baizhi and Baichai. It is the root of plants from the Apiaceae family: Angelica dahurica (Fisch. ex Hoffm.) Benth. et Hook. or Angelica dahurica (Fisch. ex Hoffm.) Benth. et Hook. f. var. formosana (Boiss.) Shan et Yuan. The main production areas are Anguo in Hebei, Changge and Yuxian in Henan. The Baizhi from Hebei Anguo is commonly called “Qi Baizhi,” while that from Henan is called “Yu Baizhi.” The variety Angelica dahurica var. formosana is mainly produced in Hangzhou, Yuyao, Linhai in Zhejiang, and Suining, Daxian, Neijiang, and Chongqing in Sichuan. The Sichuan-produced variety is commonly called “Chuan Baizhi.” The roots are harvested in summer and autumn when the leaves turn yellow, with fibrous roots and soil removed, then sun-dried or dried at low temperature.

[Properties] Pungent in taste, warm in nature. Enters the Lung, Spleen, and Stomach meridians.

[Actions] Disperses wind and eliminates dampness, opens the orifices and relieves pain, reduces swelling and expels pus.

[Applications] 1. Used for wind-cold common cold, headache, and toothache. For wind-cold with dampness common cold characterized by aversion to cold, fever, no sweating on the muscle surface, headache with neck stiffness, and severe limb soreness, it is especially suitable. Often combined with Qiang Huo, Fang Feng, Chuan Xiong, etc., as in the formula Jiu Wei Qiang Huo Tang. For Yangming meridian headache and pain in the supraorbital ridge caused by wind-cold, Baizhi can be used alone in pill form, such as Du Liang Wan, or combined with Chuan Xiong and Fang Feng, as in Chuan Xiong Cha Tiao San. For wind-heat syndrome, it is combined with Bo He, Ju Hua, Man Jing Zi, etc. For toothache caused by wind-cold, it is combined with Xi Xin; for wind-fire toothache, combined with Shi Gao.

2. Used for nasal congestion and nasal inflammation. For nasal inflammation with nasal obstruction, continuous turbid nasal discharge, and headache with frontal distension, it is often combined with Cang Er Zi and Xin Yi Hua, as in Cang Er San. If nasal discharge is yellow, thick, and foul-smelling due to wind-heat, it is combined with Dan Pi, Pu Gong Ying, Yu Xing Cao, etc.

3. Used for chronic diarrhea due to dampness and excessive leukorrhea. For cold-damp injuring the middle burner causing diarrhea with sinking clear yang and chronic diarrhea, it can be combined with Sheng Ma, Chai Hu, Ge Gen, Qiang Huo. For cold-damp descending causing excessive leukorrhea, it is often combined with Bai Zhu, Hai Piao Xiao, Fu Ling, Lu Jiao Shuang; for damp-heat descending with yellow and red leukorrhea, it is combined with Huang Bai, Chuan Gen Pi, etc.

4. Used for abscesses and sores. For early-stage swelling and toxicity with redness, swelling, heat, and pain before ulceration, it is often combined with Jin Yin Hua, Gan Cao, Dang Gui, as in Xian Fang Huo Ming Yin. For abscesses with pus difficult to discharge, it can be combined with Ren Shen, Huang Qi, Dang Gui. For yin sores on the back, cold discharge, and cold-type ulcers, it is suitable to combine with Wu Tou and Rou Gui.

Additionally, this herb can dispel wind, overcome dampness, and relieve itching, used for skin itching; combined with Qin Jiao and Xi Xian Cao, it can also treat wind-damp painful obstruction.

[Dosage and Administration] Internal use: decoction, 3–9 g; or incorporated into pills or powders. External use: appropriate amount, powdered and sprinkled or made into a paste for topical application.

[Cautions] This herb is pungent, aromatic, warm, and drying. It is contraindicated for those with blood deficiency with heat or yin deficiency with hyperactive yang.

[Modern Research] 1. Chemical constituents: Both Baizhi and Hang Baizhi contain coumarin compounds such as oxypeucedanin lactone, isoimperatorin, imperatorin oxide, hydrated imperatorin oxide, coralene, and angelicotoxin. Hang Baizhi also contains hydroxy-alpha-sanshool, bergapten, and falcarinol, as well as beta-sitosterol, palmitic acid, and elements including sodium, magnesium, calcium, iron, and phosphorus. Baizhi also contains various coumarin glucosides such as peucedanin glucoside.

2. Pharmacological effects: Baizhi decoction exhibits antipyretic, analgesic, and anti-inflammatory effects; angelicotoxin dilates coronary arteries; isoimperatorin reduces contractility of isolated frog hearts and lowers blood pressure in cats; hydroxy-alpha-sanshool, oxypeucedanin lactone, and hydrated imperatorin oxide indirectly promote fat breakdown and inhibit fat synthesis; furanocoumarins in Baizhi have photosensitizing effects, with imperatorin showing the strongest phototoxicity. Baizhi inhibits Escherichia coli, Shigella sonnei, Shigella flexneri, Proteus, Salmonella typhi and paratyphi, Pseudomonas aeruginosa, Vibrio cholerae, some Gram-positive bacteria, Mycobacterium tuberculosis, and fungi. Methanol extract of Baizhi protects mice from radiation-induced skin damage.

3. Clinical reports: 1) Vitiligo: Baizhi coumarin prepared as 0.5% and 1% tinctures and ointments applied daily at noon on lesions, followed immediately or after 10–20 minutes by sunlight exposure starting at 5 minutes and gradually increasing to 20–30 minutes. In 321 cases observed, total effective rate was 61.5%. [Journal of Dermatology Prevention and Treatment, 1980,(1):8] 2) Psoriasis: Baizhi No.1 (alcohol extract made into granules, 10 g per pack equivalent to 125 g raw herb, one pack per dose), No.3, No.4, No.5 (capsules containing active extracts equivalent to 25 g raw herb per capsule, 4–6 capsules per dose), taken once daily or every other day, followed by black light exposure 2 hours after dosing. For some stubborn lesions, 2.5% Baizhi tincture is applied topically a few minutes before light exposure. In 159 cases observed, effective rate reached 94.4%, with a cure rate of 47.2%. [Chinese Journal of Dermatology, 1981,(3):129] 3) Rheumatoid arthritis and soft tissue injuries: Baizhi and Du Huo mixed in a 3:1 ratio, ground into fine powder, mixed with kerosene into a paste and applied to affected areas. After about 20 minutes, when burning sensation occurs, remove the paste. After 2–4 hours, small blisters appear; then disinfected gauze is applied and bandaged to prevent blister rupture. One application is one treatment course. In 34 cases of rheumatoid arthritis, total effective rate was 88.2%; in 46 cases of soft tissue injury, total effective rate was 84.8%.
