[Overview] This herb is also known as Gui Duyou, Shixia Changqing, Yizhixiang, Tianzhuxiang. It is the root and rhizome of Cynanchum paniculatum (Bge.) Kitag., a plant of the Asclepiadaceae family. It is distributed throughout most regions of China, mainly produced in Jiangsu, Zhejiang, Anhui, Shandong, Hubei, and Hunan provinces. It is harvested in autumn by digging up, impurities are removed, and then dried in the shade.

[Properties] Pungent in taste, warm in nature. Enters the liver and stomach meridians.

[Actions] Dispels wind and transforms dampness, relieves pain and itching.

[Applications]
1. Used for pain syndromes. For wind-cold-damp bi syndrome with joint pain and muscle spasms, often combined with Mu Qigua and Weilingxian; for liver and kidney deficiency with cold-damp bi causing soreness and pain in the waist and knees, often combined with Xuduan, Dufengshizhong, and Duhuo; for epigastric and abdominal cold pain, combined with Gaoliangjiang and Xiangfu; for qi stagnation and blood stasis causing dysmenorrhea, combined with Taoren, Honghua, Wulingzhi, Xiangfu, Chuanxiong, and Danggui; for traumatic injury pain, can be used alone by decoction for washing or fresh herb pounded and applied topically; for toothache, besides internal use, can also be decocted for gargling.
2. Used for urinary difficulty, diarrhea, and dysentery. For urinary difficulty, combined with Fuling and Cheqianzi; for diarrhea and dysentery, often combined with Huanglian, Muxiang, and Huangqin.
3. Used for pruritus due to wind rash, eczema, herpes zoster, and psoriasis. Decoction of single herb for oral administration and external washing; or combined with Kushen, Difuzi, and Baixianpi.
4. Used for poisonous snake bites and venomous insect stings. Can be used alone as a water decoction orally and pounded for topical application; or combined with Bianbianlian, Yejuhua, and Qiye Yizhihua.

[Dosage and Administration] Internal use: decoction, 3–12 g, not suitable for prolonged decoction; or powdered form, 1.5–3 g. External use: appropriate amount, decocted for washing, or pounded for topical application, or decoction for gargling.

[Cautions] Use with caution in weak patients.

[Modern Research]
1. Chemical constituents: The whole plant contains paeonol, isopaonol, erythritol, triacontane, hexadecene, and decanoate of stearic acid. Also contains steroid compounds such as β-sitosterol, erectin B, cynanchoside A, B, C, etc. The root contains new cynanchoside A.
2. Pharmacological effects: Cynanchum paniculatum has analgesic, sedative, anti-inflammatory, anticonvulsant, and anti-allergic effects; paeonol has significant antipyretic effects on mice with fever induced by typhoid vaccine injection; paeonol significantly inhibits the beating frequency of cultured neonatal rat cardiomyocytes, showing antiarrhythmic effects; it significantly lowers serum total cholesterol and β-lipoprotein in experimental hyperlipidemic animals and markedly inhibits the formation of atherosclerotic plaques in rabbits fed with a high-fat diet; paeonol significantly inhibits platelet aggregation in experimental animals and humans, counteracting thrombosis formation; it inhibits various pathogenic cocci and bacilli.
3. Clinical reports: 1) Neurasthenia: whole plant of Cynanchum paniculatum prepared as powder, pills (honey pills), and capsules for oral administration; powder 10–15 g twice daily; pills (5 g raw herb per pill) 2 pills twice daily; capsules 0.5 g each, 10 capsules per dose, 2–4 times daily, about 20 days per course. Among 300 cases treated, 274 cases of headache had an effective rate of 94.1%, 290 cases of insomnia 95.5%, 251 cases of anxiety 95.21%, 254 cases of amnesia 93%, and 232 cases of palpitations 95.2%. [Zhong Yi Za Zhi, 1983,(10):38] 2) Chronic gastritis: Cynanchum paniculatum injection (2 ml containing equivalent of 4 g raw herb) injected at acupoints, 4 ml per session, 2 ml per acupoint, total 40 cases treated, including 21 cases of simple chronic gastritis and 19 cases of complicated type, total effective rate 92.5%. [Zhong Yi Za Zhi, 1980,(5):50] 3) Psoriasis: Cynanchum paniculatum injection (1 ml containing 40 mg raw herb crystallized) 4 ml intramuscular injection twice daily; mild cases treated for 20 days per course, severe cases for 40 days per course, generally no more than 2 courses. Among 150 cases treated, cure rate was 48.7%, total effective rate 85.7%. [Jiangsu Zhong Yi Za Zhi, 1985,(5):7] 4) Anal fissure: Cynanchum paniculatum injection 10 ml mixed with 1% lidocaine 10 ml for point infiltration injection around anus, followed by finger dilation and application of Shengji ointment to wound. Among 102 cases treated, 88 cases had symptom disappearance and ulcer healing (86%), 14 cases had symptom relief and ulcer basically healed or healed after second treatment (14%).
