[Overview] This herb is also known as Shengjian and Mujiang. It is the fresh rhizome of Zingiber officinale Rosc., a plant of the Zingiberaceae family. It is produced throughout China. Harvested in autumn and winter, the rhizomes are dug up, cleaned of fibrous roots and dirt, sliced, and used raw.
[Properties] Pungent in taste, slightly warm in nature. Enters the Lung, Spleen, and Stomach meridians.
[Actions] Releases the exterior and disperses cold, warms the middle burner and stops vomiting, transforms phlegm and stops coughing, detoxifies.
[Applications]
1. Used for wind-cold common cold with chills, slight fever, headache, nasal congestion, and other mild symptoms. Commonly used in folk medicine with brown sugar decoction taken warm (ginger sugar water). For more severe colds, it is combined with Perilla, Notopterygium, Saposhnikovia, and scallion white.
2. Used for cold invading the middle burner or spleen-stomach deficiency cold causing epigastric and abdominal cold pain, vomiting, and diarrhea. For middle burner cold abdominal pain relieved by warmth, it can be combined with Alpinia officinarum and pepper; for vomiting, it can be used alone or with Pinellia, as in the formula Xiao Ban Xia Tang. If vomiting is due to cold, it can be combined with Evodia; if due to heat, combined with Coptis and Bamboo shavings, as in Ju Pi Zhu Ru Tang. For vomiting and diarrhea due to spleen-stomach qi deficiency, it is combined with Ginseng and Atractylodes.
3. Used for phlegm-fluid cough, especially suitable for wind-cold invading the lungs or lung cold phlegm-fluid conditions. For wind-cold invading the lungs with cough and expectoration, chills, and headache, it is often combined with Ephedra and Apricot kernel, as in San Ao Tang; for cough with excessive phlegm, combined with Perilla, Apricot kernel, Aster, and Tangerine peel, as in Xing Su San and Zhi Sou San; for phlegm-heat obstructing the lungs with fullness, restlessness, and wheezing, it can be combined with Bamboo sap and Qing Meng Shi, as in Zhu Li Da Tan Wan. For wind-stroke with phlegm obstruction causing locked jaw and aphasia, the juice of this herb combined with bamboo sap can open orifices and resolve phlegm.
4. Used for food poisoning from fish, crab, bird, or beast meat. It can be used alone as a decoction or juice taken orally, or combined with Perilla. Additionally, it can detoxify poisoning from Pinellia, Arisaema, Aconite, and Wu Tou.
[Dosage and Administration] Internal use: decoction, 3–9 g; or juice. External use: appropriate amount, crushed and applied topically; or rubbed on affected area; or heated and used for moxibustion.
[Cautions] This herb is pungent and warm with dispersing properties; therefore, it is contraindicated in exterior deficiency with spontaneous sweating, yin deficiency with internal heat, and conditions of excessive heat.
[Modern Research]
1. Chemical constituents: Fresh ginger contains volatile oils, whose main components include gingerol, zingiberene, β-phellandrene, camphene, citric acid, ginger ketone, β-bisabolene, and α-curcumene. It also contains the pungent compound gingerol.
2. Pharmacological effects: Ginger oil, ginger ketone, or ginger enone exhibit antipyretic, anti-inflammatory, sedative, analgesic, and anticonvulsant effects; ginger decoction promotes secretion of gastric acid and gastric juice and protects gastric mucosa; extracts and gingerol inhibit copper sulfate-induced vomiting and significantly reduce motion sickness symptoms such as dizziness and nausea; it has preventive and therapeutic effects on carbon tetrachloride-induced liver injury; acetone extracts show significant choleretic effects; it has anti-allergic, anti-serotonin, and antitussive effects; it significantly inhibits Staphylococcus aureus, Salmonella typhi, Shigella dysenteriae, Vibrio cholerae, Salmonella, and Pseudomonas aeruginosa, and also kills pathogenic skin fungi and Trichomonas vaginalis.

1. Severe vomiting: applying ginger slices to the Neiguan point fixed with a pain-relieving plaster treated over ten cases with good results. [New Chinese Medicine, 1983, (12): 24]
2. Ascaris intestinal obstruction: fresh ginger 30 g juiced and mixed with 60 g honey as one dose, taken orally three times daily for patients over 15 years old, with reduced doses for children, combined with vegetable oil and Western supportive and symptomatic treatment. Treated 541 cases with a relief rate of 75.8%. [Journal of Integrated Traditional and Western Medicine, 1986, 6(2):114]
3. Acute lumbar sprain: ginger juice and rhubarb powder mixed into an ointment applied externally to the sprain area, covered with oil paper and gauze fixed; reapplied if not healed after 12–24 hours. Treated 110 cases, all cured. [Journal of Traditional Chinese Medicine, 1984, 25(2):46]
4. Burns: fresh ginger cleaned and dried, crushed for juice, applied with sterile cotton soaked in ginger juice or with ginger juice gauze compresses to the affected area. Treated 19 cases with immediate pain relief and all cured. [Zhejiang Journal of Traditional Chinese Medicine, 1990, 25(10):451]
5. Shoulder-hand syndrome: ginger cut into thick slices (~0.3 cm), 20–30 slices stir-fried with white wine until hot, then rubbed on painful or restricted movement areas of shoulder, wrist, and fingers until local redness appears. Then steamed locally with boiled cinnamon twig 50 g and ginger slices for about 30 minutes, followed by hot compress with gauze-wrapped residual herbs until cooled. Treatment once or twice daily. Treated 32 cases with good efficacy. [Journal of External Treatment of Traditional Chinese Medicine, 2001, (2):31]
