[Overview] This herb is also known as Chuncao, Weicao, Bailongxu, and Baimawei. It is the root and rhizome of Cynanchum atratum Bge. or the climbing Cynanchum versicolor Bge., both belonging to the Asclepiadaceae family. It is distributed in most regions of China. The roots are harvested in early spring or late autumn, with stems and leaves removed, washed clean of soil, and dried in shade or sun.

[Properties] Bitter and salty in taste, cold in nature. Enters the stomach and liver meridians.

[Actions] Clears heat and cools the blood, promotes urination and unblocks strangury, detoxifies and treats sores.

[Applications]
1. Used for febrile diseases, especially suitable for yin-deficiency heat and postpartum deficiency heat. For example, in the late stage of warm diseases when residual pathogenic factors remain, with night fever and morning coolness, bone steaming and tidal fever, it is often combined with Diguipi (Lycii Cortex), Zhimu (Anemarrhena Rhizome), Qinghao (Artemisia Annua), etc. For postpartum blood deficiency heat with low-grade fever that does not subside and fainting, it can be combined with Renshen (Ginseng), Danggui (Angelica Sinensis), and Gancao (Licorice), as in Baiwei Decoction (Baiwei Tang). If warm pathogen enters the nutritive level causing high fever, irritability, thirst, delirium, and crimson tongue, it can be combined with Sheng Dihuang (Rehmanniae Radix) and Xuanshen (Scrophulariae Radix). For individuals with pre-existing yin deficiency who contract wind-warmth pathogen and present with fever, it is often combined with Yuzhu (Polygonati Rhizoma), Dan Dou Chi (Fermented Soybean), and Bohe (Mentha), as in the modified Weirui Decoction (Jiajian Weirui Tang).
2. Used for hot strangury and bloody strangury, often combined with Mutong (Akebiae Caulis), Huashi (Talcum), Sheng Dihuang, etc.
3. Used for sores, carbuncles, swelling and toxicity, sore throat, and snakebite. For heat toxin sores with redness, swelling, and pain, it is often combined with Tianhuafen (Trichosanthis Radix), Chishao (Paeoniae Radix Rubra), and Gancao. For sore throat, it is commonly combined with Jinyinhua (Lonicerae Flos), Jiegeng (Platycodonis Radix), and Shandougen (Sophorae Tonkinensis Radix). For poisonous snakebites, the fresh herb is pounded and applied topically.

[Dosage and Administration] Internal use: decoction, 4.5–9 g; or incorporated into pills or powders. External use: appropriate amount, pounded and applied topically.

[Cautions] Use with caution in cases of spleen deficiency with loose stools.

[Modern Research]
1. Chemical constituents: The root of Cynanchum atratum contains erect cynanchoside, baqian glycoside, baqian aglycone, and erect cynanchoside new glycoside; the root of climbing Cynanchum contains climbing cynanchoside, climbing cynanchoside new glycoside, and baqian glycoside.
2. Pharmacological effects: Aqueous extracts of Baiwei show significant antipyretic effects on yeast-induced fever in rats; exhibit marked anti-inflammatory effects on croton oil-induced acute exudative inflammation in mouse ears; cynanchoside has a cardiotonic effect; it also inhibits pneumococcus.
3. Clinical reports: For lymphangitis (red thread boil), Baiwei 30 g and Cangzhu (Atractylodis Rhizoma) 10 g are decocted with 2 bowls of water to 1 bowl, taken frequently; the dregs are pounded and applied to the affected area, one dose daily for 2 consecutive days.
