[Overview] This herb is also known as Yimu, Chongwei, Wild Gastrodia, and Earth Mother Herb. It is the whole herb of Leonurus japonicus Houtt., a plant of the Lamiaceae family. It is mainly produced in Henan, Anhui, Sichuan, Jiangsu, Zhejiang, Guangdong, Guangxi, and Hebei provinces. Fresh herb is harvested in spring during the seedling to early flowering stage; dried herb is harvested in summer when stems and leaves are lush and flowers are unopened or just beginning to open, then sun-dried or cut into sections and dried.
[Properties] The taste is pungent and bitter, with a slightly cold nature. It enters the Liver, Kidney, and Pericardium meridians.
[Actions] Invigorates blood and regulates menstruation, promotes urination and reduces swelling, clears heat and detoxifies.
[Applications]
1. Used for menstrual disorders caused by blood stasis, dysmenorrhea, amenorrhea, postpartum abdominal pain, or retention of lochia. Herb Leonuri is an essential gynecological herb for menstruation and childbirth, often used alone as a decoction, syrup, or powder, or combined with Angelica sinensis, Paeonia lactiflora, Salvia miltiorrhiza, etc., in formulas such as Yimu Shengjin Dan. For dysmenorrhea, it can be combined with Angelica sinensis, Red Peony Root, and Aucklandia, as in Yimu Wan; for qi deficiency with blood stasis causing scanty or irregular menstruation and abdominal pain during menstruation, it is added to qi-tonifying formulas such as Bazhen Yimu Tang; for delayed or stopped menstruation with abdominal masses, it can be combined with Persicae Semen, Chuanxiong, and Achyranthes.
2. Used for edema and difficulty urinating, it can be used alone in large doses or combined with Imperata cylindrica root and Plantago seed for decoction. It is currently commonly used to treat nephritis-related edema.
3. Used for traumatic injuries and carbuncles with swelling and toxicity. For traumatic injuries with redness, swelling, and pain, it can be combined with Boswellia, Myrrh, and Aquilaria; for furuncles and mastitis, fresh herb juice can be taken or combined with Wild Chrysanthemum, Lonicera japonica, Taraxacum, Viola yedoensis for decoction.
Additionally, it is now often used to treat headaches and dizziness caused by hypertension, combined with Prunella vulgaris, Scutellaria baicalensis, and Uncaria.
[Dosage and Administration] Internal use: decoction 9–30 g, fresh herb 12–40 g; or as syrup; or in pills and powders. External use: appropriate amount, decoction for washing or fresh herb mashed and applied.
[Cautions] Contraindicated in pregnancy, excessive menstruation, and in cases of dilated pupils.
[Modern Research]
1. Chemical constituents: Herb Leonuri contains leonurine, stachydrine, pre-spain charo diterpenes, ursolic acid diterpenes, and pre-leonurine diterpenes. The related species Leonurus heterophyllus contains leonurine, 4-guanidinobutanol, 4-guanidinobutyric acid, arginine, Leonurus heterophyllus terpenes, and lactones.
2. Pharmacological effects: Decoctions, ethanol extracts, and leonurine show uterine stimulant effects in various animal models; Herb Leonuri increases coronary artery flow and slows heart rate in isolated animal heart ischemia models; it and its extracts have anti-platelet aggregation and anti-thrombosis effects, and significantly reduce erythrocyte aggregation; pre-leonurine enhances cellular immune function; decoctions show some in vitro antifungal activity against skin fungi.
3. Clinical reports:
(1) Coronary heart disease: Leonuri injection 8 ampules (each containing 4 g crude drug) added to 500 ml 5% glucose solution, intravenous drip once daily. In 100 cases, 45 markedly effective, 39 improved, 16 ineffective; total effective rate 84%. [Journal of Traditional Chinese Medicine, 1985,(3):29]
(2) Blood stasis with hyperviscosity: Leonuri injection 12–15 ml added to 250 ml 5% glucose solution, slow intravenous drip once daily. In 105 cases, dizziness effective rate 91.3%, headache 73%, insomnia 81%, limb numbness 82%; in 100 cases blood viscosity and hemorheology improved with 94.5% effectiveness. (3) Acute cerebral infarction: 80 patients randomized to treatment and control groups; treatment group received 20 ml Leonuri injection plus 500 ml saline IV once daily for 14 days; control group received 20 ml compound Danshen injection plus 500 ml saline IV once daily for 14 days; both groups observed for over 1 month. Results: Leonuri group showed greater reductions in plasma viscosity and fibrinogen, and better neurological deficit score improvement than control. (4) Obstetric hemorrhagic diseases: 30 g each of Herb Leonuri and Portulaca oleracea decocted once daily. In 100 cases, 83 cured, 13 improved, 4 ineffective; total effective rate 96%. [Journal of Traditional Chinese Medicine, 1990,(7):47]
(5) Diarrhea-predominant irritable bowel syndrome: 50 g Herb Leonuri decocted once daily. In 200 cases, 180 cured, 12 markedly effective, 4 effective, 4 ineffective. [Journal of Traditional Chinese Medicine, 1999,40(11):696]
(6) Female pruritus: Herb Leonuri syrup 20 ml orally three times daily. In 21 cases, all cured. [Chinese Rural Doctor, 1999,(1):36]
