[Overview] This herb is also known as Gualou shell, melon shell, or Gualou. It is the mature fruit pericarp of plants from the Cucurbitaceae family: Trichosanthes kirilowii Maxim. or Trichosanthes rosthornii Harms. The former is mainly produced in Shandong, Anhui, Henan, Zhejiang provinces, and the latter mainly in Sichuan province. The mature fruits are harvested in autumn, split open to remove the pulp and seeds, then dried in the shade.
[Properties] Sweet in taste, cold in nature. Enters the Lung and Stomach meridians.
[Actions] Clears Lung heat and transforms phlegm, regulates Qi and expands the chest.
[Applications]
1. Used for Lung-heat cough and asthma, fever and irritability; can be combined with Niubangzi (Arctii Fructus), Chuan Beimu (Fritillariae Cirrhosae Bulbus), Madouling (Aristolochiae Fructus), etc. If phlegm and saliva are congested, Lung Qi fails to descend, causing wheezing and restlessness, and constipation, it is often combined with Xingren (Armeniacae Semen) and Dahuang (Rhei Radix et Rhizoma), as in the formula Xuanbai Chengqi Tang (宣白承气汤).
2. Used for phlegm-heat accumulation causing chest and hypochondriac fullness and pain; can be used alone or with Xiebai (Allii Macrostemonis Bulbus) and Banxia (Pinelliae Rhizoma).
3. Used for breast masses; can be combined with Xiakucao (Prunellae Spica), Haizao (Sargassum), Danggui (Angelicae Sinensis Radix), Chishao (Paeoniae Radix Rubra), etc.

1. Chemical constituents include volatile oils, with acidic components such as nonanoic acid, decanoic acid, lauric acid, palmitic acid, linoleic acid, linolenic acid, and stearic acid; non-volatile components include mixtures of fatty alcohols, saturated fatty acids, sterols, glucosides, amino acids, and trace elements.
2. Pharmacology: Gualoupi injection significantly dilates coronary arteries of isolated hearts, increases coronary blood flow, and has a protective effect against myocardial ischemia; in vitro it markedly inhibits platelet aggregation and thromboxane A2 synthesis and release; Gualoupi decoction significantly prolongs PR, QT, and RR intervals on ECG in normal rats and has a clear preventive effect on arrhythmia; total amino acids isolated from Gualoupi have good expectorant effects.
3. Clinical reports: For asthmatic bronchitis and cor pulmonale asthma, a sterile aqueous solution of Gualoupi (equivalent to 58 g crude drug per 1 ml) was administered intravenously at 12–16 ml once daily. Among 35 cases treated, the control rate was 45%, and the total effective rate was 82.5%. [Beijing Zhongyi 1984,(1):22] It clears phlegm and stops cough.
