[Overview] This herb is also known as Zhouma, Jigu Shengma, and Guilian Shengma. It is the rhizome of plants from the Ranunculaceae family: Cimicifuga foetida L., Cimicifuga dahurica (Turcz.) Maxim., or Cimicifuga heracleifolia Komar. It is mainly produced in Liaoning, Jilin, Heilongjiang provinces, with additional production in Hebei, Shanxi, Shaanxi, Sichuan, and Qinghai. Harvest occurs after 4 years of cultivation in autumn when the aerial parts wither. The rhizomes are dug up, cleaned of soil, sun-dried to about 80% dryness, then the fine roots are removed by fire, followed by complete drying. The outer skin and remaining fine roots are then removed by pounding.
[Properties] Taste: sweet, acrid, slightly bitter; nature: cool. Channels entered: Lung, Spleen, Stomach.
[Actions] Releases the exterior and vents rashes; clears heat and detoxifies; raises yang and lifts what has sunken.
[Applications]
1. Used for external wind-heat invasion with headache and fever, often combined with Mulberry leaf, Chrysanthemum, Mint, Forsythia, etc. When combined with Ephedra, Perilla, Angelica dahurica, Ligusticum chuanxiong, it can also treat wind-cold common cold with chills, fever, no sweating, headache, and cough, e.g., in formulas like Shishen Tang. For Yangming channel headache with wind-heat and dampness, characterized by frontal headache, nausea, irritability, and epigastric fullness, it is combined with Atractylodes, Kudzu root, fresh lotus leaf, as in Qingzhen Tang.
2. Used in early-stage measles with incomplete rash eruption, often combined with Kudzu root, Peony, Licorice, as in Shengma Gegen Tang; if measles rash is delayed with fever, no sweating, cough, and sore throat, combined with Kudzu root, Mint, Burdock fruit, as in Xuandu Biaofa Tang.
3. Used for various heat toxin syndromes: for excessive stomach fire with swollen gums and mouth ulcers, combined with Coptis, Rehmannia glutinosa, Moutan bark, as in Qingwei San; for wind-heat obstructing the throat with sore throat, combined with Platycodon and Scrophularia, as in Niubangzi Tang; for heat toxin sores and ulcers, often combined with Honeysuckle, Forsythia, Dandelion; for warm disease with rash, combined with Gypsum, Isatis leaf, Rhinoceros horn, as in Shengma Biejia Tang.
4. Used for middle qi deficiency or qi sinking causing shortness of breath, fatigue, chronic diarrhea with rectal prolapse, uterine prolapse, or continuous uterine bleeding due to qi failing to contain blood, often combined with honey-fried Astragalus, Ginseng, Poria, Atractylodes, as in Buzhong Yiqi Tang.
[Dosage and Administration] Internal use: decoction, 3–9 g; or incorporated into pills or powders. External use: appropriate amount, powdered and applied topically; or used as a gargle; or for washing. For releasing the exterior and venting rash, and clearing heat and detoxifying, use raw; for raising yang and lifting sunken, use honey-fried.
[Cautions] This herb is toxic; dosage should not be excessive to avoid poisoning. Contraindicated in cases of excess above and deficiency below, yin deficiency with fire, and when measles rash has fully erupted.
[Modern Research]
1. Chemical constituents: Rhizoma Cimicifugae contains cimicifugine, salicylic acid, tannins, resin, caffeic acid, ferulic acid. Cimicifuga dahurica rhizome contains cimicifugin, alkaloids, sugars, resin, glycosides, isoferulic acid, ferulic acid, caffeic acid, β-sitosterol, cimicifugol, cimicifugol xyloside, and dahuricol. Cimicifuga heracleifolia contains alkaloids.
2. Pharmacological effects: extracts or components such as isoferulic acid have antipyretic and anti-inflammatory effects; aqueous decoction has analgesic, sedative, and anticonvulsant effects; methanol extracts and xylosides show significant protective effects against carbon tetrachloride-induced liver injury in rats; it can inhibit the heart, slow heart rate, lower blood pressure, and inhibit isolated intestinal and pregnant uterine contractions, but excite the bladder and non-pregnant uterus; it inhibits Mycobacterium tuberculosis, Staphylococcus aureus, Moraxella catarrhalis, and some dermatophytes.
3. Clinical reports:
1) Herpes zoster: 30–50 g of Rhizoma Cimicifugae decocted to a concentrated juice, soaked in gauze and applied topically to lesions, generally cures within 3–5 days. [Sichuan Traditional Chinese Medicine, 1988,(6):42]
2) Gastroptosis: 100% stomach-lifting injection (Rhizoma Cimicifugae and Astragalus) 3 ml per injection at acupoints Zusanli, Weishu, and Pishu alternately once daily, with a 1-day rest after 6 treatments. Among 146 cases, total effective rate was 96.7%. [Hubei Journal of Traditional Chinese Medicine, 1985,(2):44]
3) Uterine prolapse: 4 g Rhizoma Cimicifugae powdered; one egg with a small hole drilled at the top, powder inserted and mixed inside, hole sealed with damp paper, steamed, shell removed, taken orally once daily. Among 120 cases, 106 cured after 1–3 courses, 12 significantly improved, 2 ineffective. [Shandong Journal of Traditional Chinese Medicine, 1986,(3):43]
