[Overview] This herb is also known as Xianlingpi, Yanghuo, Yangjiaofeng, and Yanghuo Ye. It is the aerial part of plants from the Berberidaceae family, including Epimedium brevicornum Maxim., Epimedium sagittatum (Sieb. et Zucc.) Maxim., Epimedium pubescens Maxim., Epimedium wushanense T.S. Ying, or the Korean Epimedium koreanum Nakai. It is mainly produced in Shaanxi, Liaoning, Shanxi, Hubei, Sichuan, and other regions. Harvested in summer and autumn by cutting stems and leaves, removing coarse stems and impurities, then sun-dried or shade-dried.

[Properties] Taste: pungent and sweet; nature: warm. Enters the liver and kidney meridians.

[Actions] Tonifies the kidney and strengthens yang, strengthens tendons and bones, dispels wind and eliminates dampness.

[Applications]
1. Used for kidney yang deficiency causing impotence, spermatorrhea, infertility due to uterine cold, frequent urination and incontinence, chronic cough and asthma. For impotence and spermatorrhea, it can be taken alone soaked in wine or combined with Cistanche, Morinda, Curculigo, etc.; for infertility due to deficiency cold, combined with Cnidium and Cynomorium; for frequent urination, incontinence, or dribbling urine, often combined with Rubus, Rosa laevigata, and Staphylora; for kidney deficiency asthma and cough, combined with Psoralea and walnut kernel.
2. Used for wind-cold-damp painful obstruction syndrome with soreness and weakness of the waist and knees. For chronic wind-damp painful obstruction and cold pain in the waist and knees, it can be soaked in wine or decocted with Clematis and Cinnamon. For soreness and weakness of the waist and knees due to liver and kidney deficiency, often combined with Eucommia and Drynaria.
Additionally, it can treat deficiency cold stomach pain, often combined with Alpinia officinarum.

[Dosage and Administration] Oral administration: decoction 3–9 g; or used in pills, powders; or soaked in wine, or made into paste.

[Cautions] Contraindicated in cases of yin deficiency with hyperactive fire.

[Modern Research]
1. Chemical constituents: The aerial parts mainly contain icariin and epimedin I flavonoid glycosides, as well as alkaloids, sterols, triterpenes, and inorganic elements such as potassium and calcium.
2. Pharmacological effects: Processed Epimedii products promote gonadal function, significantly increasing plasma testosterone levels in mice, and increasing weight of testes and levator ani muscle; some reports show increased weight of anterior pituitary, ovaries, and uterus in rats without changes in plasma luteinizing hormone levels. Epimedii enhances immune function, especially improving immune deficiency in kidney-deficient patients; it dilates peripheral blood vessels, improves microcirculation, increases blood flow, reduces peripheral resistance, and increases coronary artery flow; it inhibits poliovirus and other enteroviruses; it also has anti-hypoxia, sedative, anticonvulsant, antitussive, and expectorant effects.
3. Clinical reports: 1) Male sexual dysfunction: 120 g Epimedii, 90 g Curculigo and Acanthopanax each, powdered and soaked in 1000 ml glutinous rice wine or low-alcohol liquor for 7 days. Dose: 20–25 ml each time, twice daily. Among 32 cases treated, 21 markedly effective, 7 improved, 4 ineffective. [Chinese Medicine Research, 1993, 34(2):40] 2) Coronary heart disease: Epimedii tablets 4–6 tablets per dose (each tablet 0.3 g, equivalent to 2.7 g crude drug), twice daily. Among 140 cases, total effective rate for angina symptoms was 77.8%, and for ECG improvement 74.3%. [Shanghai Journal of Traditional Chinese Medicine, 1989,(8):26] 3) Leukopenia: Single Epimedium sagittatum granules (15 g per pack), 3 packs daily first week, 2 packs daily second week, treatment for 30–45 days. Among 22 leukopenia cases with typical qi deficiency symptoms, 14 adhered to treatment, with 3 cured, 4 markedly effective, 5 effective, 2 ineffective. Among 228 cases treated, 138 took extract tablets with total effective rate 89.85%; 61 took total flavonoid tablets with 93.44% effective rate; 29 took pure icariin with 89.66% effective rate.
