[Overview] This herb is also known as Xiyangren Shen, Yangshen, Xishen, and Huaiqi Shen. It is the root of Panax quinquefolium L., a plant of the Araliaceae family. Native to the Americas, it is now cultivated in Northeast China, Beijing, Xi’an, Jiangxi, and other regions. It is harvested in autumn, cleaned, and dried in the sun or by baking.

[Properties] Taste is sweet and slightly bitter; nature is cool; it enters the Lung, Heart, and Stomach meridians.

[Actions] Tonifies Qi and nourishes Yin; clears heat and generates fluids.

[Applications]
1. Used for syndromes of dual Qi and Yin deficiency. For example, after febrile diseases or profuse sweating, severe diarrhea, or massive blood loss causing depletion of Qi and Yin, manifesting as fatigue, shortness of breath, rapid breathing, irritability, thirst, spontaneous sweating, and night sweating. Commonly combined with Ophiopogon japonicus (Maidong) and Schisandra chinensis (Wuweizi). American ginseng’s effect on replenishing original Qi is weaker than Panax ginseng, but due to its cool nature and ability to clear fire and nourish Yin and generate fluids, it is suitable for Qi and Yin dual deficiency syndromes.
2. Used for Lung Qi deficiency or Lung Yin deficiency presenting with cough, asthma, shortness of breath, fatigue, scanty or blood-tinged sputum, irritability, and thirst. It can be taken alone in capsule form or combined with Lilium brownii (Baihe), Polygonatum odoratum (Yuzhu), Ophiopogon japonicus (Maidong), Anemarrhena asphodeloides (Zhimu), etc.
3. Used in late-stage febrile diseases or constitutional internal heat with Yin deficiency or dual Qi and Yin deficiency, symptoms include thirst, irritability, shortness of breath, fatigue, spontaneous sweating. It can be taken alone as a decoction or combined with Dendrobium (Shihu), Ophiopogon japonicus (Maidong), Anemarrhena asphodeloides (Zhimu), as in Wang’s Qing Shu Yi Qi Tang formula. If internal heat and thirst persist with Qi and Yin deficiency and fire, it can be combined with Astragalus membranaceus (Huangqi), Trichosanthes kirilowii (Tianhuafen), Anemarrhena asphodeloides (Zhimu), etc. American ginseng also tonifies Qi and Yin of the Heart, Spleen, and Kidneys and can be used for Qi and Yin deficiency of the five viscera.

[Dosage and Administration] Internal use: decoct separately, 3–6 g; or powdered, 1–3 g, taken with warm water on an empty stomach; or steamed over water until cooked; or chewed directly.

[Cautions] Avoid use during colds with fever; avoid raw and cold foods and radish during medication. Contraindicated with Veratrum (Lilu).

[Modern Research]
1. Chemical constituents: American ginseng contains various ginsenosides, with ginsenoside Rb1 content higher than that of Panax ginseng. It also contains volatile oils and resins.
2. Pharmacological effects: American ginseng has analgesic, sedative, antipyretic, and antispasmodic effects, as well as significant anti-fatigue, diuretic, and anti-hypoxia abilities. Additionally, it has hypoglycemic, antiarrhythmic, anti-shock, and anticonvulsant effects.
3. Clinical reports: For non-healing ulcers, slices of American or Korean ginseng are decocted and drunk; simultaneously, the residue slices are applied topically to the affected area. In a study of 35 cases of infected wounds, chronic ulcers, and pressure sores, 26 cases showed reduced ulcer size and healed after 1–2 applications; 7 cases with larger ulcers healed after 3–4 applications; 2 cases with poor constitution showed significant ulcer reduction.
