[Overview] This herb is also known as Baigen, Ebaodan, Shandigua, and Dilaoshu. It is the tuberous root of Ampelopsis japonica (Thunb.) Makino, a plant of the Vitaceae family. It is produced in southern Northeast China, North China, East China, and Central-South provinces. The roots are harvested in spring and autumn, with stems, leaves, and fibrous roots removed, then cleaned, longitudinally cut into halves, quarters, or oblique slices, and sun-dried.

[Properties] Bitter in taste, slightly cold in nature. Enters the Heart, Liver, and Spleen meridians.

[Actions] Clears heat and detoxifies, disperses nodules and relieves pain, promotes tissue regeneration and contracts sores.

[Applications]
1. Used for sores, ulcers, swelling, and toxin accumulation, as well as scrofula and tuberculous lymphadenitis. Bailian treats sores and carbuncles regardless of whether they have ulcerated or not, or if ulcers have long failed to close. For early-stage heat toxin carbuncles with swelling and hardness, it can be used alone internally or combined with Jin Yin Hua (Lonicera japonica), Lian Qiao (Forsythia suspensa), Pu Gong Ying (Taraxacum mongolicum), etc. For sores with pus formation that is difficult to discharge, it can be combined with Chuan Re Shan Jia (Manis pentadactyla scales) and Zao Ci (Gleditsia sinensis thorns) internally. For sores that have ulcerated but fail to contract for a long time, it should be combined with Huang Qi (Astragalus membranaceus) and Dang Gui (Angelica sinensis) internally, and externally with powdered Bai Ji (Bletilla striata) and Wu Zei Gu (Sepia esculenta bone). For scrofula, this herb can be combined with Xuan Shen (Scrophularia ningpoensis) and Da Huang (Rheum palmatum) for external application, such as in the formula Bailian San.
2. Used for burns from water or fire, non-healing damp eczema, traumatic bleeding, and hand and foot fissures. For burns from water or fire, it can be used alone as a powder mixed with sesame oil and applied to the affected area, or combined with Ce Bai Ye (Platycladus orientalis leaves) and Di Yu (Sanguisorba officinalis) for topical use. For damp eczema with oozing, it can be combined with Lou Lu (Trichosanthes kirilowii), Huai Bai Pi (Sophora japonica bark), Wu Jia Pi (Acanthopanax gracilistylus bark), etc., decocted for washing, as in Lou Lu Tang. For traumatic bleeding, it can be combined with Bai Ji powder for external application. For hand and foot fissures, it can be combined with Da Huang (Rheum palmatum), Bing Pian (Borneol), and Bai Ji for topical use. When combined with Bai Zhi (Angelica dahurica), Ju Hua (Chrysanthemum morifolium), and Xing Ren (Prunus armeniaca seed), it can also treat alcoholic rhinitis, acne, and freckles.
Additionally, Bailian can be used to treat febrile convulsions in children due to excessive heat, warm malaria caused by seasonal pathogens, as well as damp-heat vaginal discharge, bloody dysentery, and intestinal wind syndromes, in combination with herbs that clear heat, resolve phlegm, open orifices, and clear damp-heat.

[Dosage and Administration] Internal use: decoction, 4.5–9 g. External use: appropriate amount, powdered and sprinkled or mixed and applied; or decoction for washing.

[Cautions] Contraindicated in cases of spleen and stomach deficiency cold and absence of real fire; use with caution during pregnancy. Antagonistic to Aconitum (Fu Zi).

[Modern Research]
1. Chemical constituents: The tuberous root contains mucilage and starch. Seven compounds have been isolated, including n-pentacosane, triacontanoic acid, octacosanoic acid, β-sitosterol, stigmasterol, quercetin, and β-D-glucoside of stigmasterol.
2. Pharmacological effects: The aqueous extract of Bailian shows varying degrees of inhibitory effects on dermatophytes such as Trichophyton concentricum, Microsporum audouinii, inguinal and red epidermophyton fungi. The decoction inhibits Staphylococcus aureus. It has inhibitory effects on cervical cancer. Bailian decoction significantly enhances the analgesic effects of Hei Fu Pian and Zhi Chuan Wu, antagonizes their contractile effects on isolated frog hearts, and exhibits strong anti-hepatotoxin and anti-lipid peroxidation activities.
3. Clinical reports:
1) Bacterial dysentery: Dried or roasted Bailian powder was encapsulated (0.3 g per capsule), 6 capsules per dose, three times daily. A total of 140 cases were treated, including 116 acute bacterial dysentery and 24 chronic bacterial dysentery cases. The total effective rate was 96.55% for acute and 91.66% for chronic cases. 2) Sprains: Two Bailian roots and appropriate salt were mashed into a paste and applied externally. Eighty cases were treated with 66 effective. [Pharmacology, Toxicology and Clinical Application of Chinese Medicine, 1992:45]
3) Hand and foot fissures: Bailian and Bai Ji 30 g each, Da Huang 50 g, roasted and powdered, mixed with 3 g Bing Pian and honey for external use. Fifty-five cases were treated with a total effective rate of 94.5%. [Modern Clinical Application Manual of Chinese Medicine, 1993:420]
