[Overview] This herb is also known as Hu Wang Shi Zhe, Du Hua, Chang Sheng Cao, and Chuan Duhuo. It is the root of Angelica pubescens Maxim. f. biserrata Shan et Yuan, a plant of the Apiaceae family. It is mainly produced in Sichuan, Hubei, Shaanxi, Anhui, Zhejiang, and Jiangxi provinces. The roots are harvested either in early spring when the shoots just sprout or in late autumn when the stems and leaves wither. After digging up, the roots are cleaned of fine roots and soil, then baked until half dry, piled for 2–3 days until soft, and then baked again until completely dry.

[Properties] The taste is pungent and bitter; the nature is slightly warm. It enters the Kidney and Bladder meridians.

[Applications]
1. Used for wind-cold-damp bi syndrome with soreness and pain in the lower back and knees. Duhuo is a principal herb for treating wind-cold-damp bi, especially suitable for pain localized in the lower body such as the lumbar and knees. It is often combined with Qin Jiao, Wei Ling Xian, and Xi Xin. For chronic cases with liver and kidney deficiency and qi and blood insufficiency manifesting as cold pain, soreness, and weakness in the waist and legs, it is commonly combined with Sang Ji Sheng, Di Huang, Dang Gui, and Ren Shen.
2. Used for headache and heaviness of the head caused by external wind-cold with dampness. It is often combined with Qiang Huo, Gao Ben, and Fang Feng, as in the formula Qiang Huo Sheng Shi Tang.
3. Used for headache and toothache. For Shaoyin headache, it is often combined with Xi Xin and Chuan Xiong. For toothache, it can be used alone by decocting with wine and gargling while hot, or combined with Xi Xin, Chuan Xiong, Qiang Huo, and Sheng Di. If toothache is caused by wind-fire, it is combined with Shi Gao, Sheng Ma, and Xi Xin.

[Dosage and Administration] Internal use: decoction, 3–9 g; can be soaked in alcohol or made into pills or powders. External use: appropriate amount, decoction for washing.

[Cautions] Use with caution in cases of yin deficiency and blood dryness.

[Modern Research]
1. Chemical constituents: Duhuo contains several coumarin compounds, dihydropteryxin and its acetate, methyl ether of parsley phenol, isoimperatorin, xanthotoxin, hydroxy-α-sanshool, dihydropteryxin angelate, angelol, and other compounds. Volatile components include atractylone, thymol, and caryophyllene.
2. Pharmacological effects: crude Duhuo preparations have antihypertensive effects; water extracts can counteract arrhythmia; alcohol extracts inhibit ADP-induced platelet aggregation in rats in vitro; decoctions or fluid extracts have sedative and hypnotic effects; methyl ether of parsley phenol has anti-inflammatory effects; xanthotoxin and hydroxy-α-sanshool have significant antispasmodic effects on rabbit ileum; decoctions inhibit Escherichia coli, Shigella, Proteus, Salmonella typhi, Pseudomonas aeruginosa, Vibrio cholerae, and Mycobacterium tuberculosis; hydroxy-α-sanshool and xanthotoxin have cytotoxic effects on Ehrlich ascites carcinoma cells.
3. Clinical reports: 1) Insomnia: Duhuo 30 g, cinnabar and amber each 6 g, ground into fine powder and mixed evenly, encapsulated; 6 capsules taken 2 hours before sleep for 10 consecutive days. Among 210 cases treated, 175 were clinically cured, 30 effective, 5 ineffective; total effective rate 97.6%.
