[Overview] This herb is also known as Ma Mutong, Ku Mutong, and Northeast Mutong. It is the vine stem of Aristolochia manshuriensis Kom., a plant of the Aristolochiaceae family. It is mainly produced in Heilongjiang, Jilin, and Liaoning provinces, with additional production in Shanxi, Shaanxi, Gansu, and Sichuan. Harvested in autumn and winter, the outer bark is scraped off, then cut into sections and sun-dried.

[Properties] Bitter in taste, cold in nature, and toxic. Enters the Heart, Small Intestine, and Bladder meridians.

[Actions] Promotes urination and unblocks strangury; clears Heart fire and reduces fire toxicity; unblocks the channels and promotes lactation.

[Applications]
1. Used for damp-heat strangury and edema. For damp-heat sinking downward causing scanty, painful urination, often combined with Plantago seed (Che Qian Zi) and Talcum (Hua Shi). For edema, often combined with Polyporus (Zhu Ling) and Mulberry root bark (Sang Bai Pi).
2. Used for Heart fire rising upward causing irritability, dark urine, and mouth and tongue ulcers; often combined with Rehmannia glutinosa (Sheng Di Huang), Bamboo leaf (Zhu Ye), and Licorice (Gan Cao).
3. Used for insufficient lactation postpartum; often combined with Vaccaria seed (Wang Bu Liu Xing) and Pangolin scales (Chuan Shan Jia).
Additionally, it can treat damp-heat painful obstruction syndrome, often combined with Coix seed (Yi Yi Ren), Mulberry twig (Sang Zhi), and Lonicera vine (Ren Dong Teng).

[Modern Research]
1. Chemical constituents: The stem contains aristolochic acids A, B, C, and D; aristolochic acid glycosides; aristolochic acid D methyl ether; magnoflorine; β-sitosterol; and (+)-dehydroisosophorone aldehyde.
2. Pharmacological actions: Decoctions of Caulis Aristolochiae Manshuriensis enhance cardiac contractility in experimental animals; they can cause an initial rise followed by a sustained decrease in blood pressure. In normal humans, oral decoction increases urine output, though no diuretic effect was observed in animals. The decoction stimulates isolated mouse small intestine but inhibits uterine contractions.

Note: Aristolochic acids contained in this herb are toxic. Excessive or prolonged use can cause acute renal failure and even death. Symptoms of poisoning include upper abdominal discomfort, followed by vomiting, headache, chest tightness, abdominal distension with dull pain, diarrhea, facial edema, frequent and urgent urination with reduced volume, progressive generalized edema, and altered consciousness. There have been numerous recent reports of renal failure caused by long-term use of proprietary Chinese medicines containing Caulis Aristolochiae Manshuriensis. Therefore, use with extreme caution.
