[Overview] This herb is also known as Zishen, Mumeng, Huoxuelian, and Hongsanqi. It is the rhizome of Polygonum bistorta of the Polygonaceae family. It is mainly produced in North China, Northwest China, and regions including Shandong, Jiangsu, and Hubei. The rhizomes are harvested in spring and autumn, with stems, leaves, and fibrous roots removed, then cleaned and sun-dried, or sliced and sun-dried.

[Properties] Bitter and astringent in taste, slightly cold in nature. Enters the Lung, Liver, and Large Intestine meridians.

[Actions] Clears heat and detoxifies, cools the blood and stops bleeding.

[Applications]
1. Used for external wind-heat, sore throat, lung-heat cough. For wind-heat common cold and sore throat, it can be combined with Banlangen (Radix Isatidis), Daqingye (Folium Isatidis), and Lianqiao (Fructus Forsythiae); for lung-heat cough with yellow sputum, it can be combined with Huangqin (Radix Scutellariae) and Zhimu (Rhizoma Anemarrhenae); for heat disease convulsions, it can be combined with Gou Teng (Ramulus Uncariae) and Jiang Can (Bombyx Batryticatus).
2. Used for damp-heat diarrhea and dysentery with pus and blood, can be combined with Chishao (Radix Paeoniae Rubra), Huangqin, and Pugongying (Herba Taraxaci).
3. Used for blood-heat bleeding syndromes. For vomiting blood and nosebleed, can be combined with Shengdi (Radix Rehmanniae), Danpi (Cortex Moutan), and Chishao; for traumatic bleeding, powdered and applied externally; for hemorrhoidal bleeding, decoction used for fumigation and washing or combined with Huaimi (Flos Sophorae) and Diyu (Radix Sanguisorbae) for decoction and oral administration.
4. Used for carbuncles, sores, and snake bites; fresh herb is pounded and applied topically or decocted with Zihuadiding (Herba Violae) and Jinyinhua (Flos Lonicerae) for oral administration.
Additionally, this herb can treat burns from fire and water by applying juice ground with vinegar; for stomatitis and gingivitis, decoction is used as a mouthwash.

[Dosage and Administration] Internal use: decoction, 4.5–9 g; or incorporated into pills or powders. External use: appropriate amount, pounded and applied topically, or decoction used for gargling and fumigation.

[Cautions] Not suitable for use in cases without real heat.

[Modern Research]
1. Chemical constituents: The rhizome contains gallic acid, methyl gallate, hydrolyzable tannins, condensed tannins, dextrorotatory catechol, levorotatory epicatechol, hydroxymethyl anthraquinone, vitamins, and β-sitosterol isomers.
2. Pharmacological effects: In vitro, Rhizoma Bistortae exhibits antibacterial effects against Staphylococcus aureus, Pseudomonas aeruginosa, Bacillus subtilis, and Escherichia coli; a preparation named “Zhexuejing No. 1” made from Rhizoma Bistortae filtrate and gelatin shows certain hemostatic effects; the contained levorotatory epicatechin significantly reduces cholinesterase activity, lowers cholesterol in rat serum and liver, and has preventive effects on experimentally induced diabetes in rats.
3. Clinical reports: Infantile autumn-winter diarrhea: alongside rehydration and acid-base correction, Rhizoma Bistortae powder was administered at 4–10 g/kg/day, twice daily. A total of 28 cases were treated, all cured and discharged within 2–3 days, showing significantly better efficacy than rehydration and acid-base correction alone.
