[Overview] This herb is also known as Bai Jiang and Jun Jiang. It is the dried rhizome of Zingiber officinale Rosc., a plant of the Zingiberaceae family. The main production areas are Sichuan and Guizhou provinces, with Zhejiang, Shandong, Hubei, Guangdong, and Shaanxi also producing it. The rhizomes are dug up in winter when the stems and leaves wither, then the stems, leaves, and fibrous roots are removed. The rhizomes are sun-dried or dried at low temperature. When sliced fresh or dried at low temperature, it is called “Ganjiang Pian” (Dried Ginger Slices).
[Properties] It has a pungent taste and a warm nature. It enters the Spleen, Stomach, Kidney, Heart, and Lung meridians.
[Actions] Warms the middle burner and disperses cold, restores yang and unblocks the pulse, dries dampness and resolves phlegm.
[Applications]
1. Used for cold syndromes of the Spleen and Stomach, with symptoms such as epigastric and abdominal cold pain, vomiting, and diarrhea. For epigastric and abdominal cold pain, it is often combined with Gao Liang Jiang (Alpinia officinarum). If the cold pain is due to Spleen and Stomach deficiency cold, it is combined with Dang Shen, Bai Zhu, and roasted Gan Cao, as in the formula Li Zhong Wan. If there is middle burner stomach cold with dry retching and vomiting, it is combined with Ban Xia, as in Ban Xia Gan Jiang San.
2. Used for yang collapse with cold limbs and faint pulse, often combined with Fu Zi, as in Tong Mai Si Ni Tang and Gan Jiang Fu Zi Tang.
3. Used for cold phlegm obstructing the lungs causing cough and asthma with chills, cold back, and copious clear thin phlegm, often combined with Xi Xin and Ma Huang, as in Ling Gan Wu Wei Jiang Xin Tang.
Additionally, Gan Jiang can disperse cold and dry dampness, used for cold-damp bi pain, often combined with Cang Zhu, Du Huo, and Wu Tou; ground with Xiong Huang for external use to treat toothache; combined with Chuan Wu Tou, Cang Zhu, and Dang Gui ground into a paste for external use to treat traumatic injuries.
[Dosage and Administration] Internal use: decoction, 3–9 g; or incorporated into pills or powders. External use: appropriate amount, decoction for washing or ground powder applied topically.
[Cautions] Contraindicated in cases of yin deficiency with internal heat or reckless movement of blood heat.
[Modern Research]
1. Chemical constituents: Gan Jiang contains volatile oils such as zingiberene, citral, geraniol, beta-bisabolene, linalool, nerol, pinene, camphene, and limonene.
2. Pharmacological actions: Methanol extract of Gan Jiang has sedative effects, significantly prolonging pentobarbital-induced sleep time in mice and mildly inhibiting the central nervous system; ether extract has analgesic effects, significantly inhibiting acetic acid-induced writhing in mice and prolonging latency of thermal pain response; water extract has anticoagulant effects, inhibiting platelet aggregation and prolonging experimental thrombosis formation; Gan Jiang or its active components enhance adrenal cortex function; ether extract has anti-inflammatory effects, significantly antagonizing carrageenan-induced paw edema in rats; also has antiemetic and anti-hypoxia effects.
3. Clinical reports:
1) Acute enteritis: 10 g dried Gan Jiang, 15 g raw Zhi Zi, one dose daily, decocted in 350 ml water to 150 ml, taken warm 50 ml three times daily half an hour before meals. One case treated with marked effect. [Practical Journal of Traditional Chinese Medicine, 2002,(6):43]
2) Pediatric diarrhea: 25 g Gan Jiang, 20 g Wu Zhu Yu, ground into fine powder, placed in gauze bag and applied to navel, warmed with hot water bag for 20–30 minutes, 3–4 times daily for 3 days per course. Treated 98 cases with 94% clinical cure rate. [Huaihai Medicine, 1997,(1):31]
3) Hand and foot fissures: 20% Gan Jiang tincture 30 ml, Gan Jiang powder 3 g, sodium chloride 0.5 g, glycerin 30 ml, 3 drops essence, water added to 100 ml, shaken well and applied topically. Treated 70 cases with total effective rate of 88.6%. [Chinese Journal of Integrated Traditional and Western Medicine, 2001,(6):469]
4) Prevention of seasickness: Gan Jiang powder in capsules, 1 g each, taken 30 minutes before boarding. Observed 54 cases; compared with placebo and Dimenhydrinate, Gan Jiang powder was more effective against seasickness. [Chinese Journal of Maritime Medicine, 1999,(1):23]
5) Bone and joint tuberculosis: equal amounts of Gan Jiang and Sha Mao (a small sand-dwelling reptile resembling a stink bug but larger, moving backward), dried on new tile surface and ground into powder, mixed with rice to form strips of various sizes as medicinal pellets, air-dried for later use. After local anesthesia, pellets inserted into fistula. Treated 10 cases with all cured. [Chinese Folk Therapy, 1999,(1):20]
6) Coronary heart disease and myocardial infarction: oral Gan Jiang capsules (main ingredient Gan Jiang, each capsule containing 0.4 g Sheng Qi Yao), 2 capsules three times daily. Treated 60 cases with 35 markedly effective, 23 effective, 2 ineffective; total effective rate 96.7%. [Chinese Journal of Emergency Traditional Chinese Medicine, 1998,(1):11]
