[Overview] This herb is also known as Chunlian Qiuliu, Yijianxi, Kudan Cao, and Sifanglian. It is the aerial part of Andrographis paniculata (Burm.f.) Nees, a plant of the Acanthaceae family. It is mainly produced in Guangdong and Fujian provinces, with additional production in Jiangxi, Hunan, Guangxi, Sichuan, and Shanghai. Harvested in early autumn when stems and leaves are luxuriant, then dried.

[Properties] Bitter in taste, cold in nature. Enters the Lung, Stomach, and Liver meridians.

[Actions] Clears heat and detoxifies, drains fire, dries dampness.

[Applications]
1. Used for externally contracted wind-heat or early-stage warm diseases with fever and headache. It can be used alone or combined with mint and chrysanthemum; if heat is excessive, often combined with honeysuckle and forsythia. Currently commonly used to treat meningitis, Japanese encephalitis, epidemic parotitis, measles, or leptospiral infections; if the pathogen is in the qi level, it can be combined with gypsum, anemarrhena, honeysuckle, and forsythia; if both qi and blood are scorched, it can be combined with rehmannia root, moutan bark, and large leaf indigo.
2. Used for lung heat cough and asthma, lung abscess with purulent sputum, sore throat, and oral ulcers. For lung heat cough and asthma, often combined with scutellaria and mulberry bark; for lung abscess with foul purulent sputum, often combined with houttuynia, tangerine peel stem, and winter melon seed; for sore throat, combined with scrophularia, arctium seed, and isatis root; for heat-induced oral ulcers, combined with mint and borneol, ground into powder and blown onto affected areas.
3. Used for damp-heat diarrhea, urinary turbidity, leukorrhea, and jaundice. For diarrhea, combined with pulsatilla, cortex fraxini, scutellaria, and costus; for urinary turbidity, combined with plantain seed and akebia; for leukorrhea, combined with toona bark and sophora root; for jaundice, combined with gardenia, capillaris, and moutan bark.
4. Used for erysipelas, ulcers, eczema, herpes zoster, and poisonous snake bites, either alone or combined with honeysuckle and viola. Both internal and external use are applicable. Currently used clinically for treating tumors, especially for gynecological choriocarcinoma and malignant hydatidiform mole.

[Dosage and Administration] Internal use: decoction 6–9 g; powder 0.6–1.2 g per dose, encapsulated and swallowed. External use: appropriate amount, crushed or made into ointment for topical application, or decoction used as eye or ear drops.

[Cautions] Use with caution in yang deficiency and spleen-stomach weakness. Avoid overdose or prolonged use.

[Modern Research]
1. Chemical constituents include andrographolide, neoandrographolide, andrographiside, caffeic acid and dicaffeoylquinic acid mixtures, and andrographis flavonoids.
2. Pharmacological effects: andrographolide inhibits and delays fever caused by Streptococcus pneumoniae and hemolytic streptococcus B, increases capillary permeability, and has good anti-inflammatory effects; deoxyandrographolide and andrographolide enhance adrenal cortex function; decoction enhances immune function; flavonoid glycoside injections reduce isoproterenol-induced ST segment depression, decrease pathological Q waves and severe arrhythmias, and prevent Na+, Ca2+ increase and K+, Mg2+ loss during myocardial ischemia-reperfusion, possibly reducing reperfusion arrhythmias; inhibits platelet aggregation and thrombosis formation, and increases fibrinolytic activity; also has antifertility effects.
3. Clinical reports: 1) Chronic colitis: 60 g of herb and 30 g of rehmannia root decocted to 100–150 ml, retained enema once before sleep for 14 days as one course. Among 85 cases, 61 cured, 22 improved, 2 ineffective. [Shanghai Journal of Traditional Chinese Medicine, 1992,(4):19] 2) Upper respiratory tract infection: 30 patients orally took andrographolide tablets, cure rate 80%, total effective rate 93.33%. [Chinese Journal of Emergency Traditional Chinese Medicine, 2004,13(2):87] 3) Pediatric Helicobacter pylori-associated gastritis: treatment group (12 cases) and control group (10 cases) both received amoxicillin 40 mg/kg, metronidazole 18 mg/kg orally three times daily for 2 weeks; treatment group additionally took hot andrographis tablets (2 tablets for ages 6–8, 3 tablets for 9–10, 4 tablets for 10–14, three times daily for 2 weeks). Results: treatment group 6 markedly effective, 5 effective, 1 ineffective, total effective rate 91.7%; control group 2 markedly effective, 3 effective, 5 ineffective, total effective rate 50%. [Chinese Journal of Integrated Traditional and Western Medicine, 2001,21(7):549] 4) Coronary heart disease: orally took one andrographis flavonoid tablet morning and evening after meals for 1 month. Among 35 cases, 14 markedly effective (40%), 16 effective (46%), total effective rate 86%. [Journal of Radioimmunology, 1999,12(1):30] 5) Eczema: andrographolide injection 40 mg intramuscularly once daily for 10 days as one course. Among 8 cases, 31 cured, 27 markedly effective, 9 effective, 15 ineffective. [Journal of Clinical Dermatology, 1984,(6):43]
