[Overview] This herb is also known as Honey Fragrance, Zhanxiang, Sinking Water Fragrance, and Qinanxiang. It is the resin-containing wood of Aquilaria sinensis (Lour.) Gilg, a plant of the Thymelaeaceae family. It is mainly produced in Hainan, Guangxi, Guangdong, and other regions. It can be harvested year-round by cutting the resin-containing wood, removing the non-resin parts, air-drying, then crushing or filing into powder.

[Properties] Pungent and bitter in taste, slightly warm in nature. Enters the Kidney, Spleen, and Stomach meridians.

[Actions] Promotes the movement of qi and relieves pain; warms the middle burner and descends rebellious qi; consolidates qi and relieves asthma.

[Applications]
1. Used for chest and abdominal distension and pain due to cold stagnation of qi; often combined with Aucklandia (Muxiang), Areca (Binglang), Lindera (Wuyao), etc., as in the formula Chenxiang Simu Tang (沉香四磨汤).
2. Used for cold hernia abdominal pain; often combined with Piper longum (Biba), Lindera (Wuyao), Fennel (Xiaohuixiang), etc.
3. Used for vomiting due to stomach cold; often combined with Citrus peel (Chenpi), Piper cubeba (Bichengqia), Sichuan pepper (Shujiao), etc. For spleen and stomach deficiency cold with vomiting and hiccup, often combined with Clove (Dingxiang), Perilla leaf (Zisuye), White cardamom (Baidoukou), etc. For food obstruction vomiting, can be combined with Atractylodes (Baizhu), Poria (Fuling), Areca peel (Dafupi), and Citrus aurantium (Zhike).
4. Used for lung and kidney deficiency with kidney failing to grasp qi causing rebellious qi and asthma; often combined with Rehmannia glutinosa (Shudihuang), Psoralea corylifolia (Buguzhi), Gecko (Gejie), etc. For kidney yang deficiency with turbid yin rising causing cough and asthma, can be combined with Aconite (Fuzi), Cinnamon (Rougui), Fennel (Huixiang), as in the formula Heixidan (黑锡丹). For deficiency below and excess above phlegm cough, often combined with Perilla seed (Suzi), Pinellia (Banxia), Tangerine peel (Chenpi), as in Suzi Jiangqi Tang (苏子降气汤).

[Dosage and Administration] Internal use: decoction 1.5–4.5 g, added near the end of decoction; powder 0.5–1.0 g.

[Cautions] Use with caution in cases of yin deficiency with fire blazing or qi deficiency with sinking.

[Modern Research]
1. Chemical constituents: The volatile oil of agarwood contains benzylacetone, para-methoxybenzylacetone, hydrogenated cinnamic acid, agarospirol, agarol, agarotetrol, α- and β-agarofurans, dihydroagarofurans, demethylagarofurans, 4-hydroxy-dihydroagarofuran, 3,4-dihydroxy-dihydroagarofuran, etc.
2. Pharmacological effects: Agarwood aqueous extract inhibits isolated rabbit small intestine; its volatile oil promotes secretion of digestive juices and bile; benzene extract prolongs hexobarbital-induced sleep time in mice, indicating central nervous system inhibition; in vitro tests show agarwood inhibits cancer cell proliferation by over 90%, and inhibits human cervical cancer cell line JTC 26 by over 95%; decoction exhibits strong antibacterial effects against Mycobacterium tuberculosis, Salmonella typhi, and Shigella flexneri.
3. Clinical reports: (1) Bronchial asthma: 4 g agarwood and 10 g Biota orientalis leaves ground into fine powder, taken with appropriate amount of rock sugar and hot water twice daily for 3 days per course, 1–3 courses total. Among 26 cases treated, 3 cured, 7 markedly effective, 16 effective, 1 ineffective. [Shaanxi Traditional Chinese Medicine Correspondence, 1985,(4):31] (2) Postoperative hiccup: 3 g agarwood powder rolled into cigarette shape, lit and inhaled; 65 cases treated. Control group of 61 cases received atropine 0.2 mg, metoclopramide 5 mg with saline to 4 ml by acupoint injection.
