[Overview] This herb is also known as Peng Ezhu, Pengshu, Guangshu, and Qingjiang. It is the rhizome of plants from the Zingiberaceae family: Curcuma phaeocaulis Val., Curcuma kwangsiensis S.G. Lee & C.F. Liang from Guangxi, and Curcuma wenyujin Y.H. Chen & C. Ling. Peng Ezhu is mainly produced in Sichuan; Guangdong, Guangxi, Yunnan, Zhejiang, Fujian, and Hunan also produce it. Guangxi Ezhu is mainly produced in Guangxi and is also called “Gui Ezhu”; Wenyujin is mainly produced in Zhejiang and is also called “Wen Ezhu.” It is harvested in winter when stems and leaves wither, dug up, cleaned of soil, steamed or boiled until thoroughly cooked, then sun-dried or oven-dried, with fibrous roots and impurities removed.
[Properties] Bitter, warm in nature. Enters the Liver and Spleen meridians. The taste is pungent.
[Actions] Promotes qi circulation, breaks up blood stasis, reduces accumulation, and relieves pain.
[Applications]
1. Used for masses and lumps caused by qi stagnation and blood stasis, chest and abdominal pain due to blood stasis, amenorrhea, and dysmenorrhea. Commonly combined with Sanleng (Rhizoma Sparganii) and others. For treating masses and lumps, often used with Sanleng, Danggui (Radix Angelicae Sinensis), Xiangfu (Rhizoma Cyperi), etc.; for chest pain and chest impediment, combined with Danshen (Radix Salviae Miltiorrhizae), Chuanxiong (Rhizoma Chuanxiong), Gualou (Fructus Trichosanthis), etc.; for dysmenorrhea and amenorrhea, often combined with Danggui, Chuanxiong, Honghua (Flos Carthami), etc.
2. Used for epigastric and abdominal distension and pain. If caused by food accumulation and indigestion, often combined with Qingpi (Pericarpium Citri Reticulatae Viride), Binglang (Semen Arecae), etc., as in Ezhu Wan formula; if due to cold congealing qi stagnation, often combined with Gaoliangjiang (Rhizoma Alpiniae Officinarum); if qi stagnation and blood stasis, can be combined with Yanhusuo (Rhizoma Corydalis); for Shaqi abdominal pain, combined with Awei (Resina Ferulae); for cold hernia abdominal pain, combined with Xiaohuixiang (Fructus Foeniculi). Additionally, this herb is often used for traumatic injuries and blood stasis swelling and pain. Clinically, Ezhu volatile oil is made into ointments or emulsions for external use to treat cervical erosion or early cervical cancer, or to prevent radiation-induced skin burns.
[Dosage and Administration] Internal use: decoction, 6–9 g; or incorporated into pills or powders. For promoting qi and relieving pain, usually used raw; for breaking blood stasis, vinegar-fried is preferred.
[Cautions] Contraindicated in cases of excessive menstrual bleeding and pregnancy.
[Modern Research]
1. Chemical constituents: Ezhu contains volatile oil, whose main components are curdione, borneol, and germacrone; also contains α- and β-pinene, camphene, limonene, 1,8-cineole, etc. It also contains dihydrocurdione, curcumadiol, and curcuminoid compounds.
2. Pharmacological effects: Ezhu oil preparations significantly inhibit and destroy the growth of various tumor cell lines in vitro; the rhizome alcohol extract and its active components show significant anti-early pregnancy effects in experimental animals; volatile oil inhibits the growth of various pathogenic bacteria in vitro; Ezhu extracts can increase white blood cell count; significantly increase femoral artery blood flow; alcohol extracts and volatile oil reduce elevated serum ALT caused by carbon tetrachloride and thioacetamide in mice; water extracts inhibit platelet aggregation and have anti-thrombotic effects; Wenyujin volatile oil has significant anti-inflammatory effects.
3. Clinical reports: 1. Coronary heart disease: Ezhu Hong injection 40–60 ml added to 5% or 10% glucose solution 500 ml for intravenous drip once daily. In 50 treated cases, symptoms were significantly improved; among 33 angina cases, 23 markedly effective, 7 improved, 3 ineffective; among 43 cases with ECG abnormalities, 11 markedly effective, 17 improved, 15 ineffective. [Henan Journal of Traditional Chinese Medicine, 1988,(3):2] 2. Peptic ulcer: oral Ezhu oil capsules, 0.3 g three times daily; if night pain occurs, add 0.3 g before sleep. In 10 cases, 7 markedly effective, 3 effective. [Journal of Practical Integrated Traditional and Western Medicine, 1990,(3):164] 3. Fungal vaginitis: Ezhu oil vaginal suppository, one inserted deep into the vagina every night before sleep. In 73 cases, 60 cured, 3 markedly effective, 5 effective, 5 ineffective. [Liaoning Journal of Traditional Chinese Medicine, 1985,(12):37] 4. Lower limb ulcers: 10 g Ezhu powdered, decocted with 600 ml water, filtered to 460 ml; 25 g boric acid and 64 g slaked lime dissolved separately in boiling water, 20 ml clear solutions taken from each and mixed with Ezhu decoction, bottled. Sterile gauze soaked in the solution applied to wounds, bandaged after slight drying, changed once daily. In 120 cases, 46 markedly effective, 58 effective, 16 ineffective.
