[Overview] This herb is also known as Bai Guo Ye. It is the leaf of Ginkgo biloba L., a plant of the Ginkgoaceae family. It is produced in most regions of China. Harvested in autumn, impurities are removed, then dried in the sun.
[Properties] Bitter, sweet, astringent in taste; neutral in nature. Enters the Heart and Lung meridians.
[Actions] Astringes the lungs to relieve asthma, activates blood circulation and nourishes the heart, astringes the intestines to stop diarrhea.
[Applications]
1. Used for lung deficiency cough and asthma; can be combined with apricot kernel (Xing Ren) and fritillaria (Bei Mu).
2. Used for chest tightness, heart pain, dizziness, and headache. For angina pectoris of coronary heart disease, can be combined with Salvia miltiorrhiza (Dan Shen), Allium macrostemon (Xie Bai), Trichosanthes kirilowii (Gua Lou), etc.; for hypertension with dizziness and headache, can be combined with Uncaria (Gou Teng), Gastrodia (Tian Ma), Polygonum multiflorum (He Shou Wu), etc. It is also commonly used for hyperlipidemia.
3. Used for diarrhea and dysentery; can be ground into powder and made into cakes with flour, then cooked and eaten. Decoction can be used externally to wash the soles of the feet, palms, and chest area to treat infantile enteritis. Additionally, external use can treat sores and carbuncles, facial freckles, lacquer dermatitis, and corns.
[Dosage and Administration] Internal use: decoction, 9–12 g; or used in pills or powders; extracts are commonly made into tablets, capsules, injections, etc. External use: appropriate amount, crushed and applied or rubbed on, or decoction used for washing.
[Cautions] Contraindicated in cases with excess pathogenic factors.
[Modern Research]
1. Chemical constituents: Ginkgo leaf contains flavonoid compounds, including flavonols and their glycosides such as kaempferol, luteolin, quercetin, isorhamnetin, and myricetin; biflavonoids such as amentoflavone and ginkgetin; catechins such as (+)-catechin and (-)-epicatechin. It also contains bitter terpenoids like ginkgolides A, B, C, J, M, and bilobalide A, as well as ginkgolic acids, hydrogenated ginkgolic acids, cascaric acid, ascorbic acid, linolenic acid, ginkgol alcohols, ginkgol phenols, various amino acids, and trace elements.
2. Pharmacological effects: Aqueous extracts of ginkgo leaf significantly protect ischemic myocardium and reduce arrhythmias; ethanol extracts and total glycosides antagonize adrenaline-induced vasoconstriction in isolated rabbit ear arteries; flavonoids dilate guinea pig coronary arteries and both in vivo and isolated rat hind limb vessels, increasing blood flow in rabbit carotid and ear arteries. Ginkgo leaf preparations increase cerebral blood flow, improve brain cell metabolism, protect brain cells, and prevent ischemia, hypoxia, and edema; also protect the blood-brain barrier in rats. Total flavonoids inhibit angiotensin-converting enzyme (ACE). Intraperitoneal injection of total flavonoids significantly reduces serum triglycerides in rats; protects the nervous system, improves learning and memory, and prevents aging and dementia. Ginkgo leaf inhibits platelet function; total flavonoids counteract thrombosis induced by intravenous collagen and adrenaline in mice; flavonoids relax tracheal smooth muscle and inhibit histamine-induced asthma in guinea pigs; flavonoids have antispasmodic effects on isolated intestines; aqueous decoctions inhibit Staphylococcus aureus, Shigella, and Pseudomonas aeruginosa.
3. Clinical reports: 1) Diabetic retinopathy: Ginkgo leaf tablets, 2 tablets three times daily, combined with aspirin 0.1 g once daily, treated 32 cases with a total effective rate of 88%. Post-treatment blood glucose, insulin, blood lipids, and hemorheology improved significantly (all P<0.01), superior to aspirin-only controls [Chinese Journal of New Drugs and Clinical Remedies, 2004, 23(3):163]. 2) Early cerebral infarction: adding ginkgo leaf extract to conventional treatment in 47 cases resulted in a cure plus marked improvement rate of 76.6%, total effective rate 91.5%, superior to controls [Journal of Cardiovascular Rehabilitation Medicine, 2003, 12(1):79]. 3) Type 2 diabetes: on top of sulfonylurea and biguanide therapy, ginkgo leaf tablets 80 mg three times daily, vitamin C 100 mg three times daily, vitamin E 100 mg daily for 3 months in 76 cases. Post-treatment fasting blood glucose decreased and fasting insulin increased significantly (P<0.05), but no significant changes in 2-hour postprandial glucose or insulin (P>0.05) [Zhejiang Journal of Integrated Traditional and Western Medicine, 2002, 12(5):275].
