[Overview] This herb is also known as Douling, Shui Ma Xiang Guo, and Chou Lingdang. It is the mature fruit of plants from the Aristolochiaceae family: Aristolochis contorta Bunge (Northern Madouling) or Aristolochia debilis Seib. et Zucc. (Madouling). The former is found in Northeast China, Inner Mongolia, Hebei, Shandong, Shanxi, Henan, Shaanxi, and Gansu provinces; the latter is distributed south of the Yangtze River basin. The fruit is harvested in autumn when it changes from green to yellow and then dried in the sun.

[Properties] Bitter in taste, slightly cold in nature. Enters the Lung and Large Intestine meridians.

[Actions] Clears Lung heat and descends Qi, stops cough and relieves asthma, cools the blood and reduces swelling.

[Applications]
1. Used for Lung-heat cough and asthma with profuse phlegm; often combined with Huang Qin (Scutellaria baicalensis), Sang Bai Pi (Morus alba root bark), Bei Mu (Fritillaria), Xing Ren (Prunus armeniaca seed), etc. For cough and asthma with urgent breathing, commonly combined with Gan Cao (Glycyrrhiza uralensis) and Ma Huang (Ephedra), as in Madouling Decoction (马兜铃汤). For cough and asthma with swelling and difficult urination, combined with Ting Li Zi (Descurainia sophia seed). Also used for chronic Lung-heat cough with Yin deficiency and blood in phlegm, often combined with Mai Men Dong (Ophiopogon japonicus), Sheng Di (Rehmannia glutinosa root), E Jiao (donkey-hide gelatin), Ou Jie (lotus rhizome node), Bai Mao Gen (Imperata cylindrica root), etc. For whooping cough, Madouling can be steamed with Bai Bu (Stemona root) and garlic juice for oral administration.
2. Used for intestinal heat causing hemorrhoidal bleeding and painful, swollen hemorrhoids; combined with Di Yu (Sanguisorba officinalis), Huai Hua (Sophora japonica flower), Zhi Qiao (Aurantii Fructus), Huang Qin, etc., decocted for oral use or applied topically as fumigation or wash.
Additionally, this herb has liver-calming and blood pressure-lowering effects, suitable for hypertension with liver Yang rising; often combined with Huang Qin, Xia Ku Cao (Prunella vulgaris), Gou Teng (Uncaria rhynchophylla), etc.

[Dosage and Administration] Oral use: decoct 3–9 g; or used in pills and powders.

[Cautions] Contraindicated in early-stage wind-cold cough, deficiency-cold cough and asthma, and spleen deficiency with loose stools. Avoid overdose and long-term use. Raw herb strongly clears heat and purges the Lung, suitable for Lung-heat cough and hemorrhoids; honey-fried preparation moderates its effect, tending to clear heat and moisten the Lung, suitable for Lung deficiency cough.

[Modern Research]
1. Chemical constituents include aristolochic acid, aristolactam, aristolochic alkaloids, magnoflorine, β-sitosterol, and others.
2. Pharmacology: Madouling decoction has mild expectorant effects; its extract relaxes smooth muscle of bronchial tubes, showing antiasthmatic effects and significant antitussive action; it inhibits growth of Staphylococcus aureus and some fungi.
3. Clinical reports: (1) Leukopenia and chronic infections: treatment with madouling alkaloids in 254 patients of various types showed a white blood cell increase effective rate of 89.3% and infection control rate of 93.7%. [Zhong Cao Yao, 1981, 12(5):227] (2) Hypertension: dried madouling powder encapsulated (0.3 g per capsule), 2 capsules orally three times daily; after blood pressure normalizes, dosage is gradually reduced. In 31 cases of stage II and 19 cases of stage III hypertension, cure rates were 19.3% and 5.2%, marked effect rates 64.0% and 36.8%, effective rates 16.0% and 47.3%, and ineffective rates 0% and 10.5%, respectively.
