[Overview] This herb is also known as Iron-foot Weilingxian, Black-foot Weilingxian, Lingxian, and Black Bone. It is the root and rhizome of Clematis chinensis Osbeck, Clematis hexapetala Pall., and Clematis manshurica Rupr., all belonging to the Ranunculaceae family. Radix Clematidis is mainly produced in Jiangsu, Zhejiang, Jiangxi, Hunan, Hubei, and Sichuan provinces; Clematis hexapetala is mainly produced in Liaoning, Heilongjiang, and Shandong provinces; Clematis manshurica is mainly produced throughout Northeast China. The roots are dug up in autumn, cleaned of stems and leaves, washed free of soil, and dried in the sun or cut into sections and dried.

[Properties] The taste is pungent and salty, and the nature is warm. It enters the Bladder meridian.

[Applications]
1. Used for wind-damp painful obstruction (bi syndrome), joint stiffness, and numbness of the limbs. Radix Clematidis is a key herb for treating wind-damp bi syndrome and can be selected for various bi patterns. For predominant wind causing wandering pain, it is often combined with Fangfeng; for heavy dampness, combined with Cangzhu; for damp-heat bi pain, combined with Huangbai; for severe cold evil, combined with Guizhi; for severe pain, combined with Wutou or Fuzi. For chronic bi syndrome with liver and kidney deficiency and weakness of the waist and knees, it is often combined with Sangjisheng, Gouji, and Duzhong.
2. Used for abdominal masses and lumps with qi stagnation and food accumulation, and for food stagnation causing choking sensation; often combined with Taoren, Danggui, Yanhusuo, Ruxiang, and Moyao.
3. Used for bone obstruction stuck in the throat; can be decocted alone or with vinegar and sugar to gargle until dissolved.
Additionally, Radix Clematidis has effects of promoting qi movement and resolving stagnation; it can treat elderly patients with qi and fluid deficiency and dry constipation, combined with Huangqi, Zhike, and Maren; for mastitis, it can be powdered and mixed with rice vinegar into a paste for topical application.

[Dosage and Administration] Oral administration: decoction 6–9 g; for bone obstruction in the throat, dosage can be increased up to 30 g; also used in pills and powders.

[Cautions] Use with caution in cases of qi and blood deficiency and during pregnancy.

[Modern Research]
1. Chemical constituents mainly include protoanemonin and saponins with hederagenin, oleanolic acid, and hederagenin aglycones.
2. Pharmacological effects: Radix Clematidis decoction can raise pain threshold and has certain analgesic effects; decoction and alcohol extracts promote bile secretion in rats and antagonize histamine-induced excitation in isolated rabbit intestinal smooth muscle; it inhibits Staphylococcus aureus and Shigella dysenteriae; tinctures and decoctions have significant diuretic effects in mice, rats, and guinea pigs.
3. Clinical reports:
1) Cholelithiasis: Radix Clematidis 60 g, decocted twice daily. In 120 cases treated, clinical symptoms disappeared with stone expulsion in 60 cases with no recurrence over one year; symptoms disappeared without stone expulsion in 44 cases; no improvement or surgery required in 16 cases; total effective rate 87%. 2) Heel pain: Radix Clematidis 5–10 g, mashed and mixed with aged vinegar into a paste. The affected foot is soaked in hot water for 5–10 minutes, dried, then the paste is applied to the heel and wrapped with wet gauze and bandaged; dressing changed once daily.
