[Overview] This herb is also known as Blue Leaf or Blue Vegetable Leaf. It is the leaf of Isatis indigotica Fort., a plant of the Cruciferae family. It is mainly produced in Jiangsu, Anhui, Hebei, Zhejiang, and other regions. Harvested in summer and autumn, impurities are removed and then it is sun-dried.

[Properties] Bitter in taste, cold in nature. Enters the Heart and Stomach meridians.

[Actions] Clears heat and detoxifies, cools the blood and dispels spots.

[Applications]
1. Used for externally contracted wind-heat with headache and sore throat, or warm diseases with high fever, delirium, rashes, vomiting blood, and nosebleed. For wind-heat, often combined with Lonicera japonica (Jinyinhua), Schizonepeta (Jingjie), and Arctium lappa seed (Niubangzi). For warm diseases with high fever and sweating, combined with Gypsum fibrosum (Shigao) and Anemarrhena asphodeloides (Zhimu). When heat enters the nutritive and blood levels causing delirium and rashes, combined with Rehmannia glutinosa (Sheng Dihuang), Moutan cortex (Danpi), Gardenia jasminoides (Zhizi), and Lithospermum erythrorhizon (Zicao). Currently, Folium Isatidis is commonly used to treat influenza, type B encephalitis, measles complicated by pneumonia, epidemic parotitis, and acute tonsillitis.
2. Used for damp-heat jaundice, diarrhea, and dysentery. For jaundice, it can be combined with Artemisia capillaris (Yinchen), Rheum palmatum (Dahuang), and Gardenia jasminoides (Zhizi). Folium Isatidis is now often used to treat viral hepatitis. For persistent damp-heat diarrhea and dysentery, it is often combined with Portulaca oleracea (Machixian) and Pulsatilla chinensis (Baitouweng). Additionally, it can be used for mouth and tongue ulcers, erysipelas, pemphigus, and simple ulcerative keratitis.

[Dosage and Administration] Internal use: decoction, 9–15 g (fresh herb can be used up to 30–60 g); or juice can be taken. External use: appropriate amount, fresh herb mashed and applied topically or decoction used for washing.

[Cautions] Contraindicated in patients with spleen and stomach deficiency.

[Modern Research]
1. Chemical constituents: Folium Isatidis contains indigo, isatan B, indirubin, and also inorganic elements such as iron, zinc, copper, selenium, chromium, arsenic, and nickel.
2. Pharmacological effects: Decoction of Folium Isatidis inhibits Staphylococcus aureus, Streptococcus pyogenes, Neisseria meningitidis, Streptococcus pneumoniae, Salmonella typhi, Escherichia coli, Haemophilus influenzae, Corynebacterium diphtheriae, etc. It also has anti-endotoxin effects against Escherichia coli O111B4.
3. Clinical reports: 1) Chronic granulocytic leukemia: tablets made from the active ingredient indirubin in Folium Isatidis or synthetic indirubin products were used to treat 40 cases, with a total effective rate of 90%. [Journal of Traditional Chinese Medicine, 1987, (10):24] 2) Eosinophilia: Folium Isatidis 30 g brewed with boiling water as a tea substitute, 300 ml per dose, often taken twice if thirsty. One dose daily, can be brewed 3–5 times. A total of 78 cases treated, with 54 cured, 19 effective, and 5 ineffective, total effective rate 93.6%. [New Chinese Medicine, 1998, 30(3):44] 3) Herpes simplex keratitis: Folium Isatidis injection 2 ml mixed with 4 ml physiological saline to prepare a 1:2 eye drop solution; severe cases also received 1 ml Folium Isatidis injection by subconjunctival injection near the fornix. Results: dendritic keratitis cure rate 95%, geographic corneal ulcer cure rate 92.1%, deep ulcer cure rate 89.32%, with no worsening of eye condition during treatment.
