[Overview] This herb is also known as Caowutou, Wutou, Wuhui, and Jidu. It is the tuberous root of Aconitum kusnezoffii Reichb., a plant of the Ranunculaceae family. It is mainly produced in Northeast and North China. Harvested in autumn when stems and leaves wither, the roots are dug up, cleaned of fine roots and soil, and sun-dried.
[Properties] Pungent and bitter in taste, hot in nature, and highly toxic. It enters the Heart, Liver, Kidney, and Spleen meridians.
[Actions] Dispel wind and scatter cold, warm the meridians, and relieve pain.
[Applications]
1. Used for wind-cold-damp bi syndrome with limb soreness and pain. For wind-cold bi syndrome with muscle and tendon spasms and pain, it is often used together with Radix Aconiti Carmichaeli (Chuanwu), and commonly combined with Fangfeng, Duhuo, and Qianghuo. For wind-dominant xing bi, it is often combined with Chuanwu, Qishe (Agkistrodon), and Qianjiandian. For severe cold pain, it is often combined with Xixin, Guizhi, and Fuzi. For cold-damp predominance, it can be combined with Chuanxiong, Cangzhu, and Wuzhuyu. For chronic bi syndrome with phlegm and blood stasis, joint deformity, and spasmodic pain, it can be combined with Dilong, Wugong, Dibeichong, Fengshi, Ruxiang, and Moyao.
2. Used for stroke paralysis and hemiplegia, often combined with Danxing, Dilong, Baijiangcan, and Ruxiang.
3. Used for headache due to wind, including migraine and wind-cold toothache. For wind headache, it can be combined with Chuanwu, Huoxiang, and Ruxiang powder, taken with Bohe decoction; for migraine, combined with Chuanxiong, Niuxi, and Yanhusuo; for wind-cold toothache, Xixin powder can be applied topically to the teeth.
4. Used for cold pain in the heart and abdomen, and cold hernia abdominal pain. For yin cold internal excess with chest impediment and heart pain, it can be combined with Chishizhi, Ganjiang, and Shujiao, as in the formula Wutou Chishizhi Wan. For spleen and stomach cold dampness with cold pain in the umbilical and abdominal region, diarrhea, poor appetite, it can be combined with Ganjiang, Cangzhu, and Chenpi. For cold hernia abdominal pain, it can be combined with Wuyao, Xiaohuixiang, and Qingpi.
5. Used for yin carbuncle, scrofula, and traumatic swelling and pain. For yin carbuncle swelling without head, it can be combined with Banxia and Nanxing powders for external application; for scrofula phlegm nodules, it can be combined with Mubiezi, Congtou, and Qiuyin powders mixed with rice vinegar for topical application; for traumatic injury with blood stasis swelling, it can be combined with Ruxiang, Moyao, Wulingzhi, and Shexiang.
Additionally, this herb has good anesthetic and analgesic effects and can be used for topical anesthesia of skin and mucous membranes or surgical anesthesia.
[Dosage and Administration] Internal use: decoction, 1.5–3 g; or in pills and powders. When used in decoctions, processed products should be used, decocted first and for a long time. External use: appropriate amount, ground into powder and applied topically; or ground with vinegar or wine and applied.
[Cautions] Internal use should employ processed Caowu (Zhicaowu). Contraindicated in cases of real heat or yin deficiency with fire excess, and in pregnancy. Incompatible with Banxia, Gualou, Tianhuafen, Beimu, Baiji, and Bailian; incompatible with rhinoceros horn.
[Modern Research]
1. Chemical constituents include aconitine, hypaconitine, mesaconitine, and other alkaloids. The constituents are basically the same as those of Radix Aconiti Carmichaeli (Chuanwu); see the entry for Chuanwu for reference.
2. Pharmacological effects: Caowutou injection has significant analgesic effects, with alkaloids as the main analgesic components; decoctions have anti-inflammatory effects and can promote the resolution of egg white-induced paw edema in rats; total alkaloids of Aconitum kusnezoffii can enhance the effect of adrenaline on myocardium, counteract T wave inversion caused by calcium chloride, and counteract early ST segment elevation or subsequent ST segment depression caused by posterior pituitary hormone.
3. Clinical reports: (1) Neurogenic tinnitus: 15 g raw Caowu soaked in 50 ml of 75% alcohol for 7 days, then used as ear drops 1–2 times daily, 2–3 drops each time. In 8 cases treated, 6 cured and 2 improved. [Zhongji Yikan, 1982,(7):6] (2) Advanced gastric cancer: 24 g processed Caowu decocted twice with water over gentle and vigorous fire to 480 ml, orally administered 20 ml twice daily. In 31 cases treated, short-term efficacy was CR (complete remission) in 2 cases (6.45%), PR (partial remission) in 16 cases (51.61%), NC (no change) in 7 cases (22.58%), PD (progression) in 6 cases (19.36%), total effective rate CR+PR 58.2%. Appendix: Processed Caowu (Zhicaowu) is the processed product of the tuberous root of Aconitum kusnezoffii Reichb. The raw Caowu is cleaned and separated by size, soaked in water until no dry core remains, then boiled until large pieces cut open show no white core and taste slightly numbing on the tongue. It is then dried to about 60% dryness, sliced thinly, and dried again to produce Zhicaowu. The properties, actions, and applications of Zhicaowu are the same as Caowu, but its toxicity is lower. Zhicaowu is mainly used internally, while Caowu is mainly used externally.
