[Overview] This herb is also known as Qingteng, Xunfengteng, Zhuigufeng, and Zhuifengsan. It is the stem vine of Sinomenium acutum (Thunb.) Rehd. et Wils. or Sinomenium acutum (Thunb.) Rehd. et Wils. var. cinereum Rehd. et Wils., plants of the Menispermaceae family. It is mainly produced in southwest, south-central, and east China. The stem vines are harvested in spring and summer, with fine branches and leaves removed, then sun-dried, or soaked in water until soft, cut into sections, and sun-dried.

[Properties] Bitter and pungent in taste, neutral in nature. Enters the spleen and liver meridians.

[Actions] Dispels wind-dampness, unblocks the meridians.

[Applications] Used for wind-damp painful obstruction, chronic joint pain, hexi feng (a type of chronic arthritis), and swollen painful beriberi. It can be used alone as a decoction or soaked in alcohol, often combined with Hongteng (Cocculus), Guizhi (Cinnamon Twig), and Fangfeng (Saposhnikovia). For shoulder and arm pain, it may be combined with Jianghuang (Turmeric) and Qianghuo (Notopterygium); for lumbar and knee pain, combined with Duhuo (Angelica pubescens) and Niuxi (Achyranthes root). Additionally, it can be used to treat injuries from overexertion, stomach pain, edema, and itching.

[Dosage and Administration] Internal use: decoction, 6–12 g; or soaked in alcohol or made into a paste for oral administration. External use: appropriate amount, decoction for washing.

[Cautions] Internal use may cause adverse reactions such as itching, rash, dizziness, headache, skin redness, abdominal pain, chills and fever, allergic purpura, thrombocytopenia, and leukopenia. Use with caution.

[Modern Research]
1. Chemical constituents: The stems and roots contain sinomenine, qingfengteng alkaloid, bis-sinomenine, mu fengshulan alkaloid, jianfangji alkaloid, tetrahydroberberine, isosinomenine, tudulanning, racemic lignan phenol, N-demethyljianfangji alkaloid, baihuaran alkaloid, guangqianjinteng alkaloid. Also contains methyl hexadecanoate, β-sitosterol, and stigmasterol.
2. Pharmacological effects: Sinomenine has analgesic effects acting on the central nervous system, showing synergistic effects with propylmorphine and tolerance with continuous use; it significantly reduces experimental arthritis, possibly by affecting the hypothalamic-pituitary-adrenal axis, unrelated to histamine release; it has sedative and tranquilizing effects and antitussive effects comparable to codeine; it suppresses nonspecific cellular and humoral immunity similar to cyclophosphamide; qingfengteng alkaloid has acute hypotensive effects with rapid tolerance upon repeated dosing, possibly related to histamine release.
3. Clinical reports: 1) Rheumatoid arthritis: The peeled and chopped rhizome of Qingfengteng, 94 g per dose, or with 6 g of Ephedra (added later), decocted over low heat for about 2 hours twice, mixed evenly, taken after meals morning and evening, or concentrated into tablets, was observed in 330 cases with a total effective rate of 93.6%. [Shaanxi Journal of Traditional Chinese Medicine, 1980,(5):12] 2) Arrhythmia: Qingfengteng tablets starting at 20–40 mg per dose, gradually increased to 60–80 mg, three times daily for 2 weeks. Clinical observation of 60 cases showed certain efficacy for some atrial and ventricular premature beats caused by organic factors. [Journal of Xi’an Medical University, 1988, 9(2):133]
