[Overview] This herb is also known as Jing Sanleng, Guang Sanleng, and Jing Sanleng. It is the rhizome of Sparganium stoloniferum Buch.-Ham., a plant of the family Sparganiaceae. It is produced in Northwest China, North China, Northeast China, as well as Jiangxi, Jiangsu, and Tibet. It is harvested from winter to the following spring, washed clean, peeled, and sun-dried.

[Properties] Bitter and pungent in taste, neutral in nature. Enters the liver and spleen meridians.

[Actions] Breaks blood stasis and promotes qi movement, reduces accumulation and relieves pain.

[Applications]
1. Used for blood stasis causing amenorrhea, dysmenorrhea, and postpartum abdominal pain due to stasis, often combined with Honghua (Carthami Flos), Danggui (Angelicae Sinensis Radix), Niuxi (Achyranthis Bidentatae Radix), etc. For amenorrhea with lower abdominal pain or masses, combined with Ezhu (Curcumae Rhizoma), Puhuang (Typhae Pollen), Danggui, Chuanxiong (Chuanxiong Rhizoma), Niuxi, Dilong (Pheretima), etc., formulas such as Sanleng Wan; for severe dysmenorrhea, add Shixiao San and Yanhusuo (Corydalis Rhizoma); for cold congealing, add Wuzhuyu (Evodiae Fructus) and Wuyao (Linderae Radix); if accompanied by blood deficiency, combine with Danggui and Chuanxiong.
2. Used for abdominal masses and lumps, often combined with Ezhu and Qingpi (Citri Reticulatae Viride Pericarpium), formula such as Sanleng Decoction. Currently often combined with Ezhu, Moyao (Myrrha), Ruxiang (Olibanum), Shui Zhi (Hirudo), Danshen (Salviae Miltiorrhizae Radix et Rhizoma) to treat ectopic pregnancy; combined with Zhigui Jia (Trionycis Carapax), Yujin (Curcumae Radix), Danpi (Moutan Cortex), Danggui, etc., to treat hepatosplenomegaly, liver cirrhosis, and tumors.
3. Used for food accumulation and qi stagnation causing epigastric and abdominal distension and pain, sometimes combined with Laifuzi (Raphani Semen), or with Ezhu, Qingpi, Banxia (Pinelliae Rhizoma), Maiya (Hordei Fructus Germinatus), etc. For pediatric food retention with abdominal distension and poor appetite, often combined with Shenqu (Massa Medicata Fermentata), Muxiang (Aucklandiae Radix), formula such as Sanleng Wan. For food retention with diarrhea, can be combined with Ezhu, Yizhiren (Alpiniae Oxyphyllae Fructus), Fuling (Poria), formula such as Sanleng San. For long-term accumulation with spleen and stomach deficiency, add Dangshen (Codonopsis Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), and Gancao (Glycyrrhizae Radix et Rhizoma).

[Dosage and Administration] Internal use: decoction, 4.5–9 g; or incorporated into pills or powders. Raw Sanleng has a strong effect on resolving stagnation; vinegar-fried Sanleng has a stronger effect on breaking blood stasis and reducing accumulation, and can enhance analgesic effects.

[Cautions] Contraindicated in qi deficiency, blood deficiency amenorrhea, and pregnancy.

[Modern Research]
1. Chemical constituents: The rhizome of Sanleng contains volatile oils, with main components including phenylethanol, hydroquinone, hexadecanoic acid, dehydromatricarin lactone, β-caryophyllene, 2-furanmethanol, and also contains various organic acids, sparganin, β-sitosterol, β-stigmasterol, carotenoid glycosides, etc.
2. Pharmacological effects: Sanleng decoction can significantly inhibit platelet aggregation and reduce whole blood viscosity; it has protective effects against tissue ischemia and hypoxia; it also shows certain inhibitory effects on tumor models in experimental animals; the aqueous decoction has a stimulatory effect on isolated uterus.
3. Clinical reports: 1) Advanced malignant tumors: Using Poshihé Ji (a decoction containing 100 g of Chuanposhi, 50 g of Sanleng, and 50 g of Mabian grass per 100 ml), treating 31 patients with digestive tract cancers, esophageal cancer, and other tumors, the recent total effective rate was 77.4%, with better recent efficacy for digestive tract tumors and cervical cancer. [Journal of Zhejiang College of Traditional Chinese Medicine, 1983, (3):31] 2) Ulcerative colitis: Using Sanleng 10–20 g, Ezhu 10–20 g, and Fanxieye (Sennae Folium) 10–30 g to make Sanleng Tang, decocted and taken once daily. A total of 32 cases were treated, with 18 cases cured (56.3%), 6 cases effective (18.7%), 8 cases ineffective (25%), total effective rate 75%. [Hunan Journal of Traditional Chinese Medicine, 2002, 17(1):56] 3) Schizophrenia: On the basis of antipsychotic drugs, adding Dahuang Sanleng capsules to treat residual schizophrenia in 46 cases, with 5 cases cured recently (10.87%), 12 cases markedly improved (26.07%), 20 cases improved (43.48%), 9 cases ineffective (19.5%), total effective rate 80.43%.
