[Overview] This herb is also known as Wild Zhangren and Baitougong. It is the root of Pulsatilla chinensis (Bge.) Regel, a plant of the Ranunculaceae family. It is mainly produced in Jilin, Heilongjiang, Liaoning, Hebei, Shandong, Shaanxi, Shanxi, Henan, Anhui, and Jiangsu provinces. The roots are dug up in spring and autumn, with stems, leaves, and fibrous roots removed, washed clean of soil, and sun-dried.
[Properties] Bitter in taste, cold in nature. Enters the Stomach and Large Intestine meridians.
[Actions] Clears heat and detoxifies, cools the blood and stops dysentery.
[Applications]
1. Used for red and white dysentery. Applicable for dysentery caused by damp-heat toxin, residual cold dysentery, yin-deficiency heat dysentery, especially suitable for heat-toxin blood dysentery syndrome. For treating gastrointestinal damp-heat and heat toxin in the blood causing dysentery with abdominal pain, urgency, tenesmus, and dysentery with pus and blood, it is often combined with Coptidis Rhizoma (Huanglian), Scutellariae Radix (Huangqin), and Cortex Fraxini (Qinpi), as in Baitouweng Tang (白头翁汤). For chronic bloody dysentery with cold abdominal pain, it is combined with Ejiao (Donkey-hide gelatin), Dried Ginger, and Red Halloysite. For residual dysentery, it is combined with Dried Ginger or Mugwort leaves. Currently, Baitouweng decoction retention enemas are commonly used clinically to treat severe bacterial dysentery and chronic amoebic dysentery.
2. Used for foul-smelling yellow vaginal discharge due to damp-heat descending; can be combined with Sophora flavescens (Kushen), Coix lacryma-jobi (Yiyiren), and Cortex Acanthopanacis (Chubai Pi). For trichomonal vaginitis, it can be decocted with Kushen and Fructus Cnidii (Shechuangzi) for external washing.
3. Used for epistaxis, hemorrhoidal bleeding, and metrorrhagia caused by blood heat. For epistaxis due to blood heat, it can be combined with Scutellariae Radix (Huangqin) and Imperatae Rhizoma (Baimaogen). For hemorrhoidal bleeding, it is combined with Sanguisorbae Radix (Diyu) and Sophora japonica charcoal (Huaihua Tan). For metrorrhagia, it can be combined with Lotus seedpod charcoal and Palm charcoal.
4. Used for sores, carbuncles, scrofula, mumps, and red swollen painful eyes. For sores and mumps, it can be used alone or combined with Lonicerae Flos (Jinyinhua), Taraxaci Herba (Pugongying), and Forsythiae Fructus (Lianqiao) decoctions. For scrofula, it can be combined with Angelicae Sinensis Radix (Danggui), Moutan Cortex (Danpi), and Prunellae Spica (Xiakucao). For red painful eyes with tearing, it can be taken as a decoction or used as a fumigation wash.
[Dosage and Administration] Internal use: decoction, 9–15 g; or incorporated into pills or powders. External use: appropriate amount, wash with the second decoction or apply as a crushed poultice.
[Cautions] Use with caution in cases of deficiency-cold diarrhea.
[Modern Research]
1. Chemical constituents: The root of Baitouweng contains Baitouweng saponins, which hydrolyze to produce triterpenoid saponins, D-glucose, rhamnose, and also contain betulinic acid, 3-oxo-betulinic acid, carotenoid glycosides, pulsatilla saponins, and proto-pulsatilla saponins. Related species such as Pulsatilla koreana root contain wilfoside bisdesmoside saponins I and II, saponin II and III; Pulsatilla cernua root contains pulsatilloside, paeoniflorin, verbascoside, and wilfoside saponins.
2. Pharmacological effects: Fresh juice, decoctions, and ethanol extracts of Baitouweng show significant antibacterial effects against Staphylococcus aureus, Pseudomonas aeruginosa, Shigella dysenteriae, Bacillus subtilis, Salmonella typhi, and Salmonella spp. The active components are proto-pulsatilla saponins and pulsatilla saponins. Baitouweng decoction and saponins inhibit the proliferation and growth of amoebae. Ethanol extracts or aqueous solutions can kill vaginal trichomonads. Baitouweng and its formulas such as Baitouweng Tang inhibit skin fungi, yeasts, trypanosomes, and Candida albicans. Ethanol extracts also have sedative and analgesic effects.
3. Clinical reports:
1) Peptic ulcer: A syrup named “Weitongling” made from Baitouweng, raw Astragalus, and honey in a 6:3:8 ratio, 20 ml per dose, three times daily before meals with hot water, treated 147 cases with a total effective rate of 91.8%. [Jiangsu Journal of Traditional Chinese Medicine, 1982,(3):18]
2) Chronic ulcerative colitis: 100 g Baitouweng decocted with 1000 ml water to 150 ml, retained as an enema once nightly for 15 days. For spleen qi deficiency, 50 g each of Astragalus and Atractylodes added. Among 37 cases, 26 cured, 9 improved, 2 ineffective; total effective rate 94.6%. [Jiangsu Traditional Chinese Medicine, 1998, 19(6):25]
3) Intestinal amoebiasis: 30 g Baitouweng decoction 100 ml combined with 0.8 g crushed metronidazole tablets and 10–20 ml 654-2 injection for retention enema once nightly before sleep, retained for over 2 hours, one course is 1 week. Among 44 ordinary cases, 37 cured, 6 improved, 1 ineffective; 4 acute cases, 2 improved, 2 ineffective; 2 chronic cases, 2 improved; total effective rate 94%. 4) Pneumonia: Decoction of Baitouweng, Prunella vulgaris, Scrophularia ningpoensis, and raw Rheum palmatum used for ultrasonic nebulization. Among 7 cases of Pseudomonas aeruginosa pneumonia, 6 were effective and sputum cultures turned negative. [Journal of Traditional Chinese Medicine, 1996,(12):103]
5) Toothache: 25 g Baitouweng decocted and strained, frequently held in the mouth; toothache stopped after one day. [Jilin Journal of Traditional Chinese Medicine, 1992,(2):32]
