[Overview] This herb is also known as Salt-Processed Fuzi, White Fu Pian, and Black Shun Pian. It is a processed product of the lateral root of Aconitum carmichaelii Debx., a plant of the Ranunculaceae family. It is mainly produced in Sichuan province, with cultivation introduced in many regions nationwide and small-scale production. The whole plant is harvested from late June to early August; the mother root, fibrous roots, and soil are removed, commonly called “mud Fuzi,” then processed and prepared into Salt-Processed Fuzi, Black Shun Pian, and White Fu Pian.
[Properties] Taste: acrid, sweet; nature: intensely hot; toxicity: toxic. Channels entered: Heart, Kidney, Spleen meridians.
[Actions] Rescues yang from collapse, tonifies fire and assists yang, warms the channels and disperses cold, dispels dampness and relieves pain.
[Applications]
1. Used for yang collapse syndrome, symptoms include cold extremities with reversed coldness, aversion to cold and fatigue, abdominal pain with diarrhea of clear food residue, lethargy and desire to sleep, weak breathing, and faint pulse nearly ceasing. Commonly combined with dried ginger and licorice, as in the formula Si Ni Tang (Four Reversal Decoction). If yang collapse is severe with profuse sweating, shortness of breath, and panting, it should be combined with ginseng, as in Shen Fu Tang (Ginseng and Prepared Aconite Decoction). If yin predominates and obstructs yang, with symptoms of diarrhea, faint pulse, flushed face, and irritability, it can be combined with dried ginger and scallion white, as in Bai Tong Tang.
2. Used for various yang deficiency syndromes, regardless of kidney yang, spleen yang, or heart yang deficiency. For kidney yang deficiency with symptoms such as impotence, spermatorrhea, soreness and weakness of the lower back and knees, cold uterus and infertility, it is often combined with cinnamon bark, prepared rehmannia, and cornelian cherry, as in Shen Qi Wan (Kidney Qi Pill). For spleen yang deficiency with cold limbs, abdominal pain, vomiting, and diarrhea, it is combined with ginseng, dried ginger, and white atractylodes, as in Fuzi Li Zhong Wan (Prepared Aconite Pill to Regulate the Middle). For spleen and kidney yang deficiency with impaired qi transformation, internal retention of dampness causing edema of the head, face, and limbs, difficult urination, cold body and reversed cold limbs, it can be combined with white atractylodes and poria, as in Zhen Wu Tang (True Warrior Decoction). For heart yang deficiency with palpitations, shortness of breath, chest obstruction, and heart pain, it is often combined with ginseng and cinnamon twig. For patients constitutionally yang deficient who catch wind-cold with symptoms of aversion to cold, fever, fatigue, desire to lie down, and a deep pulse, it is often combined with ephedra and asarum, as in Ma Huang Fu Zi Xi Xin Tang.
3. Used for cold-damp bi syndrome with whole body joint and bone pain, often combined with cinnamon twig, white atractylodes, and licorice, as in Gan Cao Fu Zi Tang.
4. Used for yin sores and ulcers with swelling but no ulceration, often combined with astragalus, ginseng, and angelica, as in Hui Yang San Jian Tang and Shen Gong Nei Tuo San.
Additionally, powdered and applied to the soles of the feet, it can treat chronic mouth ulcers; combined with potassium alum as a powder for tooth rubbing, it can treat wind-fire toothache.
[Dosage and Administration] Internal use: decoction, 3–15 g; or incorporated into pills or powders. External use: appropriate amount, powdered and applied topically.
[Cautions] Contraindicated in yin deficiency with yang excess, true heat with false cold, and pregnancy. This herb is highly toxic; raw Fuzi should not be taken internally. Internal use requires proper processing and decoction for 30–60 minutes to reduce toxicity; dosage should start small and increase gradually to avoid poisoning. Avoid combining with Banxia, Gualou, Baiwei, Baiji, and Beimu.
[Modern Research]
1. Chemical constituents: Fuzi contains aconitine, hypaconitine, mesaconitine, isoflavidine, new aconitine, benzoylmesaconine, new yunaconitine, racemic deoxyaconitine, guan-fang alkaloid chloride, deoxyhigenamine, and other alkaloids.
2. Pharmacological effects: Fuzi has cardiotonic effects, mainly due to racemic deoxyaconitine, guan-fang alkaloid chloride, and deoxyhigenamine. Fuzi injection significantly improves hypoxia tolerance in mice, antagonizes pituitary posterior lobe hormone-induced myocardial ischemia and arrhythmia in rats, and reduces acute myocardial ischemia in anesthetized open-chest dogs. In vitro, Fuzi promotes platelet aggregation and coagulation; intravenous injection also enhances platelet aggregation. Aconitine alkaloids have anti-inflammatory, analgesic, and local anesthetic effects; they enhance both cellular and humoral immune functions.
3. Clinical reports: (1) Shock: 30% Shen Fu injection 10–20 ml added to 20 ml of 5%–10% glucose solution, saline, or Ringer’s solution, injected intravenously; repeated every 0.5–1 hour if necessary; or 50–100 ml added to 250–500 ml of the above fluids for intravenous drip. In 138 cases, 91 were markedly effective, 32 effective, 15 ineffective; total effective rate 89.1%. [Journal of Traditional Chinese Medicine, 1988, 29(4):25] (2) Frostbite: 10 g Fuzi soaked in 50 g white wine for half an hour, then simmered gently and boiled for 3 minutes; while hot, applied with cotton balls to affected areas 5 times before sleep each night. Among 37 frostbite cases without ulceration, 20 cured, 10 improved. [Zhejiang Journal of Traditional Chinese Medicine, 1998, (10):441] (3) Alopecia areata: 30 g each of prepared Fuzi and Duzhong (Eucommia bark) coarsely powdered, soaked in 60 ml warm vinegar for 7 days. Applied irregularly with cotton dipped in the solution to affected areas at least 3 times daily. Among 26 cases, 8 showed sparse hair growth after 10 days, 12 showed dense fine yellow hair after 10 days, and 6 after 15 days. [Shandong Journal of Traditional Chinese Medicine, 1993, 12(6):41]
