[Overview] This herb is also known as Changpu, Water Sword Grass, Sword-leaf Changpu, and Mountain Changpu. It is the rhizome of Acorus tatarinowii Schott, a plant of the Araceae family. It is mainly produced in Sichuan, Zhejiang, Jiangsu, and other regions. It is harvested in autumn and winter by digging up, removing fibrous roots and dirt, and then sun-dried.

[Properties] Taste: acrid, bitter; nature: warm. Channels entered: Heart, Liver, Spleen.

[Actions] Opens the orifices and awakens the spirit; transforms dampness and harmonizes the stomach.

[Applications]
1. Used for phlegm obstructing the clear orifices, mental cloudiness, and epilepsy. For damp-warm febrile disease with damp-heat producing phlegm blocking the clear orifices, symptoms include fever, mental cloudiness, and delirium; it is combined with Curcumae Radix (Yujin), Fritillariae Cirrhosae Bulbus (Chuan Bei), Forsythiae Fructus (Lian Qiao), etc. For stroke with phlegm blocking the heart orifices and coma, it is often combined with Pinelliae Rhizoma (Ban Xia), Arisaema (Tian Nan Xing), Citrus Reticulata Pericarpium (Ju Hong), as in the formula Ditan Tang. For phlegm-heat blocking the orifices with high fever and coma, it is combined with Curcumae Radix, Pinelliae Rhizoma, and bamboo sap, as in Changpu Yujin Tang. For phlegm-heat epilepsy with coma and convulsions, it can be combined with Aurantii Fructus Immaturus (Zhi Shi), Bambusae Caulis in Taeniam (Zhu Ru), Coptidis Rhizoma (Huang Lian), as in Qingxin Wendan Tang. For severe phlegm-heat mania, it can be combined with Polygalae Radix (Yuan Zhi), Cinnabaris (Zhu Sha), and raw iron filings, as in Sheng Tie Luo Yin.
2. Used for dizziness, insomnia, forgetfulness, and tinnitus. For phlegm turbidity blocking the orifices causing dizziness, drowsiness, excessive dreaming, and forgetfulness, it can be combined with Poria (Fu Ling), Polygalae Radix, and Dragon Bone (Long Gu). For forgetfulness due to heart and spleen qi deficiency, it can be combined with Ginseng (Ren Shen) and Poria, as in Bu Wang San. For insomnia with excessive mental work and heart spirit deficiency, palpitations and restlessness, it is often combined with Ginseng, Atractylodis Macrocephalae Rhizoma (Bai Zhu), Longan Arillus (Long Yan Rou), Ziziphi Spinosae Semen (Suan Zao Ren), and Poria (Fu Shen), as in Anshen Dingzhi Wan. For tinnitus and deafness due to heart and kidney deficiency, it can be combined with Cuscutae Semen (Tu Si Zi), Ligustri Lucidi Fructus (Nu Zhen Zi), and Ecliptae Herba (Han Lian Cao), as in Anshen Buxin Wan.
3. Used for damp obstruction of the middle burner, epigastric and abdominal distention and pain, and mouth-locked dysentery. For damp obstruction of the middle burner causing chest and epigastric fullness, loss of appetite, abdominal pain, and intestinal rumbling, it can be combined with Agastache (Huo Xiang), Pinelliae Rhizoma, Citri Reticulatae Pericarpium (Chen Pi), and Magnoliae Officinalis Cortex (Hou Po). If dampness transforms into heat with fever, diarrhea, chest and epigastric fullness, and yellow greasy tongue coating, it is combined with Coptidis Rhizoma, Poria, and Nelumbinis Semen (Shi Lian Zi), as in Lian Po Yin. For severe damp-heat toxin with inability to digest food and tenesmus with dysentery, it can be combined with Coptidis Rhizoma, Poria, and Nelumbinis Semen, as in Kai Jin San.

Additionally, the natural juice of Rhizoma Acori Tatarinowii boiled into a paste and applied to the eyes can treat red eyes and corneal opacity; combined with Chimonanthus Praecox Flos (La Mei Hua), Platycodonis Radix (Jie Geng), and Dendrobii Caulis (Shi Hu), it can treat hoarseness; decoction used for fumigation and bathing can treat rheumatic arthralgia and joint stiffness or deformity.

[Dosage and Administration] Internal use: decoction 3–9 g; or incorporated into pills or powders. External use: appropriate amount, decocted water for washing or powdered and applied topically.

[Cautions] Use with caution in cases of yin deficiency with yang hyperactivity, excessive sweating, or seminal emission.

1. Chemical constituents: This herb contains volatile oils, carbohydrates, organic acids, amino acids, etc. The main components of the volatile oil are β-asarone, also containing α-asarone, caryophyllene, α-gurjunene, acorone, eugenol, methyl isobutyl phenol, elemicin, dimeric asarone, myristic acid, and γ-asarone.
2. Pharmacological effects: Decoctions of Rhizoma Acori Tatarinowii and its oil-removed decoctions have sedative and analgesic effects; suspensions can antagonize pentylenetetrazol-induced convulsions; it contains multiple antispasmodic and bronchodilatory components that completely antagonize acetylcholine-induced contraction of tracheal and intestinal smooth muscle; oral decoction promotes digestive juice secretion and inhibits abnormal fermentation in the gastrointestinal tract, and relieves intestinal smooth muscle spasms; α-asarone antagonizes oxytocin-induced uterine contractions; high concentration extracts inhibit common pathogenic fungi; decoctions can kill ascitic cancer cells in vitro.
3. Clinical reports: 1) Hepatic encephalopathy: Rhizoma Acori Tatarinowii injection (0.5% total volatile oil solution), mild cases 20 ml/day, moderate 30 ml/day, severe 40 ml/day, usually 5–7 days per course. Total 279 cases treated, total effective rate 74.9%. [Zhongchengyao Yanjiu 1982,(10):22] 2) Cerebral infarction: Rhizoma Acori Tatarinowii, Chuanxiong Rhizoma, dried earthworm each 10 g, Spatholobi Caulis and Alismatis Rhizoma each 25 g, made into oral liquid (1 ml contains 1 g crude drug) for oral administration. Total 36 cases treated, total effective rate 88.89%. [Zhongguo Zhongxiyi Jiehe Zazhi, 1994,(2):17] 3) Pediatric chronic cough: Rhizoma Acori Tatarinowii 6–9 g, decocted twice, 100 ml each time, combined and taken in divided doses, one dose daily. Severe cough combined with Cicadae Periostracum; copious clear phlegm combined with Fritillariae Thunbergii Bulbus. Total 78 cases treated, 32 cured, 38 effective, 8 ineffective, total effective rate 89.7%. [Zhongyi Zazhi, 1996,37(10):583] 4) Neurogenic vomiting: daily 10–15 g Rhizoma Acori Tatarinowii, crushed, wrapped in gauze, decocted with 500 ml water over low heat for 15 minutes, juice taken frequently by sipping.
