[Overview] This herb is also known as Shui Xie, Mang Yu, Ze Zhi, and Yu Sun. It is the tuber of Alisma orientalis (Sam.) Juzep., a plant of the Alismataceae family. It is mainly produced in Fujian, Sichuan, Jiangxi, Guangdong, Hubei, and Hunan provinces. It is harvested in winter when the stems and leaves wither, then the stems, leaves, and fibrous roots are removed, the coarse skin is peeled off, washed clean, and sun-dried.
[Properties] Taste is sweet and bland; nature is cold. Enters the Kidney and Bladder meridians.
[Actions] Promotes urination and drains dampness; clears heat and resolves phlegm.
[Applications]
1. Used for edema with fullness, difficulty urinating, painful and scanty urination with heat strangury, excessive leukorrhea, and diarrhea, especially when caused by damp-heat accumulation. For edema and difficulty urinating, it is often combined with Poria (Fu Ling), Polyporus (Zhu Ling), and Atractylodes macrocephala (Bai Zhu). For damp-heat accumulation in the lower burner with painful, scanty urination, it is often combined with Plantago seed (Che Qian Zi), Talcum (Hua Shi), and Dianthus (Qu Mai). For damp-heat leukorrhea that is continuous and unremitting, it can be combined with Bai Zhu and Coix seed (Sheng Yi Ren). For loose stools and diarrhea, it is commonly combined with Bai Zhu and Che Qian Zi, based on the principle of “promoting urination while solidifying the stool.”
2. Used for phlegm-fluid retention and failure of clear yang to ascend causing dizziness, tinnitus, and vertigo; often combined with Bai Zhu, as in the formula Zexie Tang (泽泻汤). Currently often used to treat inner ear vertigo and middle ear effusion.
3. Used for kidney yin deficiency with hyperactive fire causing seminal emission and lumbar pain; often combined with Rehmannia glutinosa (Shu Di Huang), Moutan cortex (Dan Pi), and Cornus officinalis (Shan Zhu Yu), as in formulas Liu Wei Di Huang Tang and Zhi Bai Di Huang Tang. Currently, this herb is also commonly used to treat hyperlipidemia, hypertension, and diabetes.
[Dosage and Administration] Internal use: decoction, 6–9 g; or incorporated into pills or powders.
[Modern Research]
1. Chemical constituents include Alisol A, B, and C and their acetates, Alisol A acetate, epoxy alisol derivatives, as well as abundant potassium salts, volatile oils, alkaloids, asparagine, and resins.
2. Pharmacological effects: Decoctions and extracts of Rhizoma Alismatis have diuretic effects, increasing urine volume and excretion of sodium, chloride, potassium, and urea. Ethanol extracts significantly lower blood glucose and have anti-atherosclerosis effects, improve liver fat metabolism with anti-fatty liver effects, and also promote weight loss. The water-soluble fraction of the alcohol extract significantly dilates coronary arteries, promotes diuresis, and increases coronary blood flow. Decoctions have immunosuppressive and anti-inflammatory effects and inhibit Staphylococcus aureus, Streptococcus pneumoniae, and Mycobacterium tuberculosis.
3. Clinical reports: 1) Hyperlipidemia: Oral administration of Rhizoma Alismatis extract tablets (each tablet equivalent to 3 g raw herb), 3 tablets per dose, 3 times daily; or decoction of Rhizoma Alismatis 15 g and Bai Zhu 6 g, one dose daily divided into two administrations. In 30 cases treated, among 21 cases with high cholesterol, the effective rate was 85.7%; among 26 cases with high triglycerides, the effective rate was 80.8%. [Zhong Yi Yao Yan Jiu, 1988, (4):28] 2) Inner ear vertigo: Decoction of Rhizoma Alismatis and Bai Zhu, each 60 g, one dose daily. In 92 cases treated, 51 cured, 33 markedly effective, 8 ineffective; total effective rate 91.3%. [Shaanxi Zhong Yi, 1989, 10(12):534] 3) Diabetes: Tablets made from equal amounts of Rhizoma Alismatis, Pueraria lobata (Ge Gen), and Acanthopanax senticosus (Ci Wu Jia), each tablet weighing 0.307 g (about 1.11 g raw herb), taken 5–7 tablets per dose, 3 times daily, one hour before meals, for 30 days per course. In 24 cases observed, 9 markedly effective, 9 effective, 6 ineffective; total effective rate 75%. [Zhong Yi Za Zhi, 1983, (9):25] 4) Suppurative otitis media: Decoction of Rhizoma Alismatis and Bai Zhu, each 30 g, plus Bupleurum (Chai Hu) 15 g, one dose daily divided into three administrations; dosage reduced for children. In 35 pediatric cases treated, 29 cured, 6 improved; total effective rate 100%. [Chengdu Zhong Yi Xue Yuan Xue Bao, 1988, (1):19]
