[Overview] This herb is also known as Ganzi, Lingze, and the root of Zhongshouhua. It is the tuberous root of Euphorbia kansui T.N.Liou ex T.P.Wang, a plant of the Euphorbiaceae family. It is produced in Gansu, Shaanxi, Henan, and Shanxi provinces. The root is harvested either before flowering in spring or after the stems and leaves wither in autumn, then the outer skin is removed by pounding and the root is sun-dried.
[Properties] Bitter in taste, cold in nature, and toxic. It enters the Lung, Kidney, and Large Intestine meridians.
[Actions] Drains water and expels retained fluids; resolves phlegm and calms convulsions; detoxifies and reduces swelling.
[Applications]
1. Used for various syndromes of water retention and accumulation. For edema of the body and face, abdominal ascites, fullness and pain in the chest and hypochondria, constipation with retention of urine, and when the upright qi is not yet depleted, it can be used alone or combined with Euphorbia pekinensis (Daji), Daphne genkwa (Yuanhua), and Semen Pharbitidis (Qianniuzi). Formulas include Shizao Tang and Zhouchuan Wan. If water retention is combined with heat pathogen causing chest binding syndrome, it can be combined with Rheum palmatum (Dahuang) and Natrii Sulfas (Mangxiao), as in Da Xian Xiong Tang. For postpartum women with blood and water retention causing lower abdominal distension like a drum and difficult urination without thirst, it can be combined with Dahuang and Ejiao, as in Da Huang Gansui Tang. For phlegm-fluid accumulation in the chest and diaphragm causing neck, chest, and back pain and pulling, it can be combined with Daji and Sinapis Alba (Baijiezi), as in Kongxian Dan. Modern clinical use includes treatment of intestinal obstruction and intestinal fluid distension pain, often combined with Dahuang, Taoren, and Houpo.
2. Used for wind-phlegm epilepsy, it can be combined with Dahuang, Daizheshi, and Banxia; also can be powdered and cooked with pig heart, then made into pills with Cinnabaris, as in Suixin Dan. It is also used for stubborn phlegm causing asthma, cough, and choking sensation.
3. Used for sores, carbuncles, swelling and toxicity, and phlegm nodules or breast lumps. For sores and carbuncles, it can be ground into powder and mixed with water for external application; for phlegm nodules and breast lumps, it can be combined with Panax notoginseng, green walnut branches, and raw licorice to make a resolving stasis ointment for external application.
[Dosage and Administration] Internal use: 0.5–1 g, processed and incorporated into pills or powders. External use: appropriate amount, ground into powder and applied topically. Internal use is preferably vinegar-processed to reduce toxicity.
[Cautions] Contraindicated in qi deficiency, yin deficiency, spleen and stomach weakness, and pregnancy; discontinue immediately if adverse reactions occur; incompatible with licorice.
[Modern Research]
1. Chemical constituents include euphol, kansuiphol, euphorbia diene alcohol, alkaloids, citric acid, resin, and tannins.
2. Pharmacological actions: Kansui stimulates the intestines, promotes peristalsis, and produces a laxative effect; decoctions or injections can damage the placenta and impair fetal circulation, thus having abortifacient effects; crude preparations significantly suppress immune function in mice; it has anti-leukemia effects and some analgesic properties.
3. Clinical reports:
1) Tuberculous exudative pleurisy: used kansui, dahuang, and mangxiao 9 g each, decocted in water. Six cases treated; in four with small pleural effusion, effusion disappeared after 6–9 doses. [Shanghai Journal of Traditional Chinese Medicine, 1983,(1):26]
2) Postoperative urinary retention: finely powdered roasted kansui 3–6 g mixed with wine into a cake, applied to Shenque (CV8) point, covered with soft film and bandaged for 4–6 hours; simultaneously licorice 10 g decoction taken orally. Twenty-seven cases treated, all effective. [Hunan Journal of Traditional Chinese Medicine, 1989,(5):37]
3) Oliguria phase of epidemic hemorrhagic fever: kansui 5 g powdered, decocted in 500 ml water for 10 minutes, retained enemas 3–4 times daily; two cases treated, both cured. [Henan Traditional Chinese Medicine, 2003, 23(8):78]
4) Adhesive intestinal obstruction: kansui suspension (1.5 g kansui in 100 ml purified water) administered via gastric tube, tube clamped for 1.5 hours, then warm saline 500 ml enema given after 0.5 hours; treatment can be repeated with good effect. [Chinese Journal of Integrated Traditional and Western Medicine, 2001, 21(4):312]
5) Induced abortion: 50% kansui anhydrous ethanol extract 0.6 ml injected into amniotic cavity by syringe; 143 cases induced abortion with 100% success rate on first injection. [Guizhou Medicine, 1989, 13(2):92]
6) Pertussis: kansui, euphorbia, and daphne 31 g each, stir-fried with vinegar until charred, ground into fine powder, mixed with 60 g wheat flour stir-fried yellow, made into pills; 1–3 pills taken each morning. 283 cases treated, generally cured within 3–5 days. [Hubei Journal of Traditional Chinese Medicine, 1982,(3):6]
