[Overview] This herb is also known as Ku Zhang, Ban Zhang, Hua Ban Zhu, and Zi Jin Long. It is the rhizome and root of Polygonum cuspidatum Sieb. et Zucc., a plant of the Polygonaceae family. It is mainly produced in Jiangsu, Zhejiang, Anhui, Guangdong, Guangxi, Yunnan, Sichuan, Guizhou, and other regions. The roots are dug up in spring and autumn, with the fibrous roots removed, then cleaned and sun-dried. Fresh roots are used as harvested.
[Properties] Slightly bitter in taste, slightly cold in nature. Enters the Liver, Gallbladder, and Lung meridians.
[Actions] Dispel wind and eliminate dampness, disperse blood stasis and relieve pain, transform phlegm and stop cough.
[Applications]
1. Used for wind-damp painful obstruction, damp-heat jaundice, diarrhea and dysentery, turbid urinary dribbling and leukorrhea. For wind-damp painful obstruction with meridian blood stasis and joint immobility, it can be combined with Qiang Huo, Du Huo, Sang Ji Sheng, Ji Xue Teng, etc.; for damp-heat jaundice due to liver and gallbladder accumulation, it can be used alone or combined with Yin Chen, Huang Bai, Jin Qian Cao, etc. for decoction; for damp-heat dysentery and diarrhea, it can be combined with Ku Shen, Qin Pi, Huang Lian, etc.; for hot stranguria and stone stranguria, it is often combined with Hai Jin Sha, Qu Mai, Mu Tong, Hua Shi, etc.; for damp-heat leukorrhea, it can be combined with Huang Bai, Yi Yi Ren, Bi Xie, etc.
2. Used for blood stasis causing amenorrhea, abdominal masses, and traumatic injuries. For blood stasis amenorrhea and dysmenorrhea, it is often used with Dang Gui, Dan Shen, Yi Mu Cao, etc.; for postpartum retention of lochia, it is combined with Tao Ren, Hong Hua, Chuan Xiong, etc.; for abdominal masses, it is often combined with San Leng, E Zhu for decoction; for traumatic injuries with blood stasis and swelling, it can be combined with Niu Xi, Chuan Duan, Gu Sui Bu, Di Bie Chong, etc. for decoction or wine infusion.
3. Used for sores, carbuncles, toxic swelling, snake bites, and burns. For carbuncles, it is often combined internally with Jin Yin Hua, Pu Gong Ying, Zi Di Ding, Tu Da Huang, etc.; externally used fresh, pounded and applied or soaked in decoction. For snake bites, externally fresh pounded application or combined decoction with Ban Bian Lian, Mu Zhi Lian, etc.; for damp sores and ulcerated legs, burns and scalds, it can be used as a decoction wash or topical application, or burned to ash and mixed with sesame oil, strong tea, or egg white for application.
4. Used for lung-heat cough with excessive phlegm, can be combined with Huang Qin, Gua Lou, etc. Currently often used to treat acute and chronic bronchitis and pneumonia.
[Dosage and Administration] Internal use: decoction, 9–15 g; or soaked in wine; or incorporated into pills or powders. External use: appropriate amount, fresh pounded application; or powdered and applied; or decoction wash.
[Cautions] Contraindicated in pregnancy.
[Modern Research]
1. Chemical constituents: The root and rhizome contain free anthraquinones and anthraquinone glycosides, mainly including emodin, emodin methyl ether, emodin phenol, anthraglycoside A, anthraglycoside B, etc. Also contains resveratrol, polydatin (resveratrol glycoside), proto-catechuic acid, dextrorotatory catechin, cassia anthrone 8-glucoside, β-sitosterol glucoside, as well as glucose, rhamnose, gingerol, amino acids, tannins, and trace elements such as copper, iron, manganese, zinc, potassium and potassium salts.
2. Pharmacological effects: Rhizoma Polygoni Cuspidati decoction inhibits and kills Staphylococcus aureus, Staphylococcus albus, hemolytic streptococcus, corynebacterium, Escherichia coli, Proteus, Shigella flexneri, Pseudomonas aeruginosa, various virus strains, and leptospira; crude herb and polydatin have antitussive effects; decoction has antiasthmatic effects; emodin regulates gastrointestinal function, sometimes increasing intestinal muscle tone, sometimes relaxing it; the herb significantly inhibits hepatitis B surface antigen (HBsAg), increases bile secretion and relaxes the sphincter of Oddi; polydatin strongly inhibits lipid peroxidation, preventing accumulation of lipid peroxides in the liver, thus protecting the liver; anthraquinones have significant antihypertensive effects; polydatin and resveratrol reduce blood lipids; polydatin significantly inhibits arachidonic acid and ADP-induced platelet aggregation and thromboxane B (TXB) production in rabbits, and also inhibits Ca2+-induced platelet aggregation to some extent; emodin inhibits seven tumor strains including mouse sarcoma S180, liver cancer, breast cancer, Ehrlich ascites carcinoma, lymphoma, mouse melanoma, and rat Walker carcinoma.
3. Clinical reports:
(1) Acute icteric infectious hepatitis: 90 g of Rhizoma Polygoni Cuspidati decocted concentrated to 300 ml, taken orally three times daily; pediatric doses reduced accordingly. Generally taken continuously for 2–3 weeks or even months until symptoms disappear and liver function normalizes, followed by consolidation treatment for several weeks. Among 325 cases treated, 280 cases were basically cured (86%), 45 cases improved (14%). [Hubei Journal of Traditional Chinese Medicine, 1983,(4):13]
(2) HBsAg-positive chronic active hepatitis: rhizome extract tablets taken orally 3 times daily, 6 tablets each time; additionally, 30 g raw hawthorn used as tea substitute, with vitamin supplements as adjunct therapy; one course lasts 3 months. Among 32 cases treated, total effective rate was 90.63%. [Shandong Journal of Traditional Chinese Medicine, 1982,(2):84]
(3) Upper gastrointestinal bleeding: Rhizoma Polygoni Cuspidati powder taken orally 4 g each time, 3–4 times daily. Among 29 cases treated, effective rate was 100%. [Zhejiang Second Medical Journal of Traditional Chinese Medicine, 1980,15(5):210]
(4) Burns: 100 g of the herb decocted with 5 L water for 2 hours, filtered and concentrated to 500 ml, preservatives such as benzoic acid and nipagin added. After cleaning the wound with 0.1% benzalkonium bromide solution, the decoction is applied externally without dressing; blister drainage is generally avoided. Among 142 cases treated, except for 2 cases of extensive burn shock, 2 cases of sepsis from infection, and 1 case of respiratory burn who died despite rescue, the remaining 137 cases were cured. (5) Cervical erosion: Rhizoma Polygoni Cuspidati, Ku Fan, and fresh pig bile in equal amounts powdered and applied locally. Among 781 cases treated, 434 cured, 331 markedly effective, 16 ineffective. [New Chinese Medicine, 1983,(3):35]
(6) Radiation dermatitis: 200 g of the herb decocted twice with appropriate distilled water, combined decoction filtered and concentrated to 1000 ml, cooled, then 95% ethanol added to 65% alcohol content, pH adjusted to 7.0–8.0 with 5% sodium bicarbonate, distilled water added to 1000 ml, aliquoted and sealed. Applied externally with cotton swab 4–6 times daily for 10 days per course. Among 60 cases treated, 40 cured, 19 effective, 1 ineffective; total effective rate 98.33%.
