[Overview] This herb is also known as Qingju Pi or Qinggan Pi. It is the pericarp of the young or immature fruit of Citrus reticulata Blanco and its cultivated varieties, belonging to the Rutaceae family. It is mainly produced in Fujian, Sichuan, Guangdong, and other regions. The young fallen fruit is collected in May to June and sun-dried, commonly called “Ge Qingpi.” The immature fruit is harvested in July to August, the peel is longitudinally cut into four sections to the base, the pulp is removed, and then sun-dried, commonly called “Sihua Qingpi” or “Sihua Qingpi” (Four-Flower Green Peel).
[Properties] Bitter and pungent in taste, warm in nature. Enters the Liver, Gallbladder, and Stomach meridians.
[Actions] Soothes the Liver and breaks Qi stagnation; reduces accumulation and resolves stagnation.
[Applications]
1. Used for Liver Qi stagnation causing pain and distension in the chest, hypochondrium, and breasts, as well as hernia pain. For Liver Qi stagnation with hypochondriac distension and pain, irritability, and easy anger, it is often combined with Xiangfu (Cyperus), Chaihu (Bupleurum), Zhike (Fructus Aurantii), etc. If accompanied by breast distension or lumps, it may be combined with Yujin (Curcuma), Juye (Citrus leaf), and Silkworm Gourd Fibers. For disharmony between Liver and Stomach with frequent belching, poor appetite, and hypochondriac pain, it can be combined with Muxiang (Aucklandia), Sharen (Amomum), Houpo (Magnolia bark), and Xiangfu. For hernia pain, it is combined with Wuyao (Lindera), Xiaohuixiang (Fennel), and Chuanlianzi (Melia fruit), as in the formula Tiantai Wuyao San.
2. Used for food accumulation and Qi stagnation causing epigastric and abdominal distension and pain, infantile malnutrition, and abdominal distension in the elderly due to food retention. Often combined with Shan Zha (Hawthorn), Mai Ya (Barley malt), Shen Qu (Massa Fermentata), as in Qingpi Wan. For damp-heat accumulation with abdominal distension, fullness, and constipation, it can be combined with Binglang (Areca), Dahuang (Rhubarb), as in Pingqi San. For infantile malnutrition, often combined with Shan Zha, Baizhu (Atractylodes), Banxia (Pinellia), etc.
3. Used for abdominal masses, chronic malaria nodules, often combined with Yuxiang (Agallochum), Chuanxiong (Ligusticum), Sanleng (Sparganium), Ezhu (Curcuma), etc. For chronic malaria with persistent nodules, it is suitable to combine with Biejia (Turtle shell) and Renshen (Ginseng).
[Dosage and Administration] Internal use: decoction, 3–9 g; or incorporated into pills or powders.
[Cautions] Use with caution in cases of Qi deficiency.
[Modern Research]
1. Chemical constituents: Qingpi contains the pressor active ingredient L-synephrine acetate, as well as various amino acids including aspartic acid and glutamic acid. The peel contains volatile oils, mainly D-limonene and limonene. It also contains tangeretin, hesperidin, neohesperidin, vitamin B1, and inositol. The synephrine content in Qingpi (0.26%) is higher than in Chenpi (aged peel).
2. Pharmacological actions: The volatile oil of Qingpi mildly stimulates the gastrointestinal tract, promoting digestive juice secretion and expelling internal gas, and relaxes gastrointestinal smooth muscle with stronger effects than Chenpi, thus mainly regulating gastrointestinal function. It has expectorant effects, possibly due to citric acid and pinene. Qingpi injection antagonizes histamine-induced isolated bronchial spasms and reduces histamine-induced decreases in guinea pig bronchopulmonary perfusion. It has a pressor effect due to synephrine, which acts by stimulating α-adrenergic receptors. Intravenous injection of Qingpi injection significantly increases bile secretion in rats, thus having a choleretic effect. The water decoction protects liver cells.
3. Clinical reports: 1) Paroxysmal supraventricular tachycardia: Qingpi injection 4 ml (equivalent to 4 g crude drug) added to 40 ml 25% glucose injection, slowly injected intravenously at about 5 ml/min. In 49 cases treated, 42 were effective and 7 ineffective. [Journal of Traditional Chinese Medicine, 1987, 28(3):21] 2) Shock: Qingpi injection 0.1–0.5 ml (1 ml contains 1 g crude drug) diluted in 20 ml 25% glucose solution for intravenous drip, then 5–10 ml added to 500 ml infusion fluid for continuous intravenous drip at 20–40 drops per minute. In 22 cases treated, 17 were markedly effective (77%) and 5 effective (23%). [Shanghai Journal of Traditional Chinese Medicine, 1987, (2):21]
