[Overview] This herb is also known as Fengshi, Fengxiangguo, Fengguo, Langmu. It is the mature fruit spike of Liquidambar formosana Hance, a plant of the Hamamelidaceae family. It is mainly produced in Jiangsu, Zhejiang, Anhui, Fujian, Hubei, Hunan, Shaanxi, and other regions. The fruit is harvested in winter when mature, impurities are removed, and then dried in the sun.
[Properties] Bitter in taste, neutral in nature. Enters the Liver and Kidney meridians.
[Actions] Dispels wind and eliminates dampness, soothes the Liver and unblocks the collaterals, promotes urination.
[Applications]
1. Used for wind-damp painful obstruction, numbness of limbs, hemiplegia due to stroke. For wind-damp painful obstruction, it can be combined with Haifengteng (Celastrus), Jixueteng (Spatholobus), Sangzhi (Mulberry twig), etc.; for qi and blood stasis with blocked collaterals and hemiplegia, it can be combined with Huangqi (Astragalus), Chuanxiong (Ligusticum), Dilong (Earthworm), etc.
2. Used for Liver qi stagnation, epigastric and abdominal pain, irregular menstruation, amenorrhea. For Liver qi invading the Stomach causing epigastric distention and pain, it can be combined with Foshou (Citron), Lumeihua (Green plum flower), Guangmuxiang (Radix Aucklandiae), etc.; for Liver qi stagnation causing lower abdominal pain, it is often combined with Yanhusuo (Corydalis), Jinlingzi (Melia seed), Chaihu (Bupleurum), etc.; for women with qi stagnation and blood stasis causing irregular menstruation and amenorrhea, it can be combined with Danggui (Angelica), Shaoyao (Paeonia), Xiangfu (Cyperus), etc.
3. Used for insufficient lactation or milk stasis, often combined with Chuanshanjia (Armadillo), Wangbuliuxing (Vaccaria seed), Qingpi (Green tangerine peel), etc.
4. Used for abdominal distention and edema with difficult urination, can be combined with Qianglang (Dung beetle), Tongcao (Tetrapanax), Sigualuo (Luffa), Xiangyuanpi (Citron peel), etc.
Additionally, Fructus Liquidambaris can also be used for urticaria, wind rash, tinea, allergic rhinitis, and traumatic injuries.
[Dosage and Administration] Internal use: decoction, 5–9 g; or powdered. External use: appropriate amount, powdered and sprinkled; or burned to smoke and inhaled.
[Cautions] Use with caution during pregnancy.
[Modern Research]
1. Chemical constituents: The fruit of Liquidambar contains 28-deoxodunorucic acid, storax, cyclostorax, isocyclostorax, oxidized eugenol, betulinic acid, 24-ethylcholest-5-en-3-ol, and others.
2. Pharmacological effects: The methanol extract of Fructus Liquidambaris and betulinic acid show significant protective effects against cytotoxicity induced by carbon tetrachloride and galactosamine in primary cultured rat hepatocytes; Fructus Liquidambaris can inhibit swelling in rat egg albumin-induced arthritis.
3. Clinical reports:
1) Cerebral infarction: 120 patients with cerebral infarction were randomly divided into treatment group (60 cases) receiving Fructus Liquidambaris injection 20 ml by intravenous drip once daily for 2 weeks, and control group (60 cases) receiving Danshen 16 ml intravenous drip once daily for 2 weeks. Results: The effective rate in the treatment group was 86.7%, significantly higher than the control group’s 68.3% (P<0.01). Neurological deficit scores in the treatment group were significantly reduced compared to before treatment (P<0.01) and were better than the control group (P<0.05). 2) Type 2 diabetes: 86 hospitalized patients with secondary failure of sulfonylurea drugs were randomly divided into experimental and control groups. The control group received intensified insulin therapy; the experimental group received intensified insulin therapy plus intravenous infusion of Fructus Liquidambaris. Results: All patients showed significant improvement in insulin sensitivity and islet function after treatment (P<0.01). There was no significant difference in insulin sensitivity, HOMA, and IRT indices between groups, but CRT index showed significantly better islet function in the experimental group, and treatment duration was shorter. 3) Hypertensive cerebral hemorrhage: Both groups received mannitol as basic treatment; patients with large hemorrhage and severe condition received compound glycerin intravenous infusion. The treatment group (140 cases) additionally received Fructus Liquidambaris injection 0.5 g plus 5% glucose saline or normal saline 250–500 ml intravenous drip once daily for 15–20 days; control group (74 cases) did not receive Fructus Liquidambaris. Results: Cure rates were 64 and 23 cases; marked effect 51 and 26 cases; effective 19 and 15 cases; ineffective 6 and 10 cases; total effective rates were 95.7% and 86.5%, respectively. 4) Hematuria: 116 patients were randomly divided into two groups. On the basis of conventional treatment, the treatment group (60 cases) received Fructus Liquidambaris injection 500 mg added to 5% glucose injection intravenous drip once daily for 15 days as one course; control group (56 cases) received tranexamic acid 0.5 g added to 5% glucose injection intravenous drip once daily for 15 days. Results: The treatment group had 34 markedly effective, 22 effective, 4 ineffective cases; control group had 20 markedly effective, 13 effective, 23 ineffective cases; total effective rates were 93.3% and 58.9%, respectively. The treatment group’s efficacy was significantly higher than control (P<0.01). [Clinical Digest, 2003, 18(8):429]
