[Overview] This herb is also known as Chuanwu, Caowu, and Wuhui. It is the mother root of Aconitum carmichaeli Debx., a plant of the Ranunculaceae family. It is mainly produced in Sichuan, Yunnan, Shaanxi, and Hunan provinces. Harvested from late June to early August, the roots are dug up, with lateral roots, fibrous roots, and soil removed, then sun-dried.
[Properties] Pungent and bitter in taste, hot in nature, highly toxic. Enters the Heart, Liver, Spleen, and Kidney meridians.
[Actions] Dispels wind and scatters cold, eliminates dampness and unblocks the channels, warms the meridians and relieves pain.
[Applications]
1. Used for wind-cold-damp bi syndrome with joint pain and restricted flexion and extension. Often combined with licorice, ephedra, astragalus, white peony, etc., as in Wutou Tang (Aconite Decoction). For chronic bi syndrome complicated by phlegm and blood stasis, numbness and spasms of the limbs, or hemiplegia, it can be combined with earthworm, stiff silkworm, scorpion, frankincense, myrrh, etc., as in Xiao Huoluo Dan (Minor Invigorate Collaterals Pill).
2. Used for windstroke with hemiplegia, headache, and head wind. In cases of sudden unconsciousness, facial distortion, hemiplegia, phlegm and qi rising, and phlegm rattling in the throat, it should be combined with raw Nanxing (Arisaema), raw Fuzi (Aconiti Lateralis Radix Praeparata), and costus root. For cold-damp stagnation causing head wind and headache due to suppressed yang qi, it can be taken alone after frying yellow and soaking in wine, or combined with Tian Nanxing and fresh ginger decoction.
3. Used for cold pain in the heart and abdomen, cold hernia pain, and toothache. For yin cold excess with obstruction of heart yang causing severe heart or heart and abdomen cold pain, it is often combined with Fuzi, dried ginger, red halloysite, and Sichuan pepper, as in Wutou Chishizhi Wan (Aconite and Red Halloysite Pill). For cold hernia abdominal pain, it can be decocted alone or combined with cinnamon twig, Lindera root, and green tangerine peel. For wind-cold induced persistent toothache, it can be powdered with Fuzi and made into pills to apply topically to the painful tooth area for rapid pain relief.
4. Used for yin carbuncles, scrofula, and phlegm nodules, especially suitable for diffuse swelling without ulceration. Raw herb can be combined with raw Pinellia, raw Nanxing, powdered and applied topically. For swelling and toxins that are about to ulcerate but have not yet surfaced, it can be combined with water-ground Semen Momordicae for external application.
5. Used for traumatic injuries, anesthesia, and pain relief. For traumatic injuries, it can be combined with natural copper, earthworm, and Lindera root, as in Huisheng Xuming Dan (Life-Extending and Fate-Continuing Pill). Traditional formulas often use raw Chuanwu combined with raw Caowu and Rhododendron for internal use as anesthetic and analgesic agents.
[Dosage and Administration] Internal use: decoct 1.5–3 g, preferably added early and decocted for a long time; or used in pills or powders. External use: appropriate amount, powdered and applied topically. Internal use requires processed herb; external use uses raw herb.
[Cautions] Contraindicated in yin deficiency with fire blazing and in pregnancy. Incompatible with Pinellia, Trichosanthes, Trichosanthes root, Fritillaria, Bletilla, and Smilax; afraid of rhinoceros horn.
[Modern Research]
1. Chemical constituents include aconitine, hypaconitine, mesaconitine, talatamine, racemic demethylhengchunine, bitaladine, new unine, demethylpigweed alkaloid, aconitine alpha and beta, and various other alkaloids. Also contains uracil and aconite polysaccharides A, B, C, and D.
2. Pharmacological effects: Chuanwu has anti-inflammatory, analgesic, sedative, and local anesthetic effects; both raw and processed decoctions have cardiotonic effects; aconite polysaccharide A has hypoglycemic effects; aconite injection inhibits gastric cancer cells.
3. Clinical reports:
1) Periarthritis of the shoulder: Using 90 g each of Chuanwu, Caowu, and camphor, finely powdered and mixed with aged vinegar to apply to the affected area about 0.5 cm thick. In 35 cases, 22 cured, 8 markedly effective, 4 improved, 1 ineffective. Usually effective after 3 applications, average 7 applications. [Shanghai Journal of Traditional Chinese Medicine, 1987,(1):29]
2) Heel spur: 30 g raw Chuanwu (dose for one heel) powdered and mixed with white wine to a paste, applied nightly after washing foot with warm water, covered with plastic wrap and bandaged. Each application lasts 24 hours. Usually 2–3 applications relieve pain. Stop when pain ceases; avoid prolonged use. [Shandong Journal of Traditional Chinese Medicine, 1987,(6):42]
3) Cervical spondylosis (nerve root type): 10% Chuanwu solution used as iontophoresis liquid. In 58 cases, 23 cured (40.9%), 28 improved (47.4%), 7 ineffective (11.6%). [Shizhen National Medicine, 2003,14(11):678]
4) Hyperplastic knee arthritis: 250 g each of raw Chuanwu and Caowu crushed or wet-ground to powder, soaked in 2500 ml 95% alcohol for 7–10 days, then distilled water added to 5000 ml, soaked another 7 days, filtered and bottled to make 10% tincture. The liquid is introduced into Ashi points or selected acupoints. In 70 cases, 36 cured, 33 improved, 1 ineffective; total effective rate 98.5%. 5) Cancer: Aconite injection 2 ml (containing 8 mg total aconite alkaloids) intramuscularly once daily for 30 days per course, up to 3 courses. In 10 cases (8 gastric cancer, 1 cardia cancer, 1 pancreatic cancer), 6 showed recent effectiveness, 2 stable, 2 changed. [Beijing Traditional Chinese Medicine, 1986,(3):27]
6) Chronic bronchitis: Compound Sansheng injection (raw Chuanwu, raw Fuzi, raw Nanxing) 10 ml each time intravenously. In 18 cases, 10 markedly effective, 6 effective, 2 ineffective. [Journal of Nanjing University of Traditional Chinese Medicine, 1986,(1):40]
[Additional Herb] Zhichuanwu (Processed Chuanwu): This is the processed product of Chuanwu. It is pungent, bitter, hot, and toxic. Enters the Heart, Liver, Spleen, and Kidney meridians. Its actions and applications are the same as Chuanwu. Internal use: decoct 1.5–3 g, preferably added early and decocted for a long time. Usage cautions are the same as for Chuanwu.
