[Overview] This herb, also known as Shudihuang, is the processed product of the tuberous root of Rehmannia glutinosa Libosch, a plant of the Scrophulariaceae family. It is prepared by steaming or sun-drying with yellow wine. It is produced throughout most regions of China, with major production and high quality from Wenxian, Boai, Wuzhi, and Mengxian counties in Henan Province. The roots are harvested in autumn, cleaned of stems, fibrous roots, and soil, then dried. They are mixed with yellow wine and steamed until the outer skin turns black and glossy, then dried again; alternatively, the roots are steamed for 8 hours, sealed overnight, steamed again for 4–8 hours the next day, sealed overnight again, then dried to about 80% dryness before being sliced thickly and dried further.
[Properties] Taste: sweet; nature: slightly warm. Enters the Liver and Kidney meridians.
[Actions] Tonifies blood and nourishes yin; enriches essence and replenishes marrow.
[Applications]
1. Used for various blood deficiency syndromes such as pale and sallow complexion, dizziness, palpitations, insomnia, irregular menstruation, uterine bleeding, and vaginal discharge. Commonly combined with Angelica sinensis (Danggui), Paeonia lactiflora (Baishao), and Ligusticum chuanxiong (Chuanxiong), as in the formula Siwu Tang (Four-Substance Decoction). For combined qi and blood deficiency with symptoms of limb weakness, fatigue, and pale complexion, it is combined with Panax ginseng, Poria cocos, Atractylodes macrocephala, Angelica sinensis, and Paeonia lactiflora, as in Bazhen Tang (Eight-Treasure Decoction). For blood deficiency with scanty or irregular menstruation, it is combined with Angelica sinensis, Paeonia lactiflora, Schizonepeta carbonisata, and Cornus officinalis, as in modified Siwu Tang. For blood deficiency and qi weakness during pregnancy with threatened miscarriage, it is combined with Panax ginseng, Dipsacus asper, Angelica sinensis, and Scutellaria baicalensis.
2. Used for Liver and Kidney yin deficiency syndromes such as bone steaming fever, night sweats, pain in knees and feet. For severe deficiency fire, it is combined with Anemarrhena asphodeloides and Phellodendron amurense. For Liver and Kidney yin deficiency with dizziness, tinnitus, soreness and weakness of the waist and knees, or mild tidal fever and night sweats, it is combined with Cornus officinalis, Dioscorea opposita, and Moutan cortex, as in Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill). For Liver and Kidney deficiency with Liver yang rising and dizziness, it is combined with Chrysanthemum, Abalone shell, and Lycium barbarum.
3. Used for Kidney deficiency syndromes such as kidney essence depletion, delayed development, premature graying of hair. Often combined with Polygonum multiflorum, Lycium barbarum, and turtle shell gelatin. For Kidney deficiency with asthma, it is combined with honey-fried Glycyrrhiza uralensis and Angelica sinensis, as in Zhenyuan Yin. For Kidney qi deficiency with urinary incontinence, it is combined with Schisandra chinensis and Psoralea corylifolia. For elderly patients with Spleen and Kidney deficiency and chronic diarrhea, it is combined with Atractylodes macrocephala and Dioscorea opposita.
4. Used for Heart, Stomach, and Lung yin deficiency syndromes. For Heart yin deficiency with heart fire rising, irritability, insomnia, epilepsy, it is combined with Coptis chinensis, Ophiopogon japonicus, and Ziziphus spinosa. For Stomach yin deficiency with virtual fire rising and symptoms such as headache and toothache, it is combined with Gypsum fibrosum and Achyranthes bidentata, as in Yunu Jian (Jade Woman Decoction). For Lung yin deficiency with dry cough, scanty sputum, blood-streaked sputum, and dry painful throat, it is combined with Scrophularia ningpoensis, Lilium brownii, and Fritillaria cirrhosa. Additionally, prepared Rehmannia is used for Kidney yang deficiency with diminished Mingmen fire, aversion to cold, cold limbs, impotence, spermatorrhea, urinary dribbling, or digestive weakness with loose stools, often combined with Aconitum carmichaelii and Cinnamomum cassia, reflecting the principle “to tonify yang well, one must seek yang within yin.”
[Dosage and Administration] Oral administration: decoction, 9–15 g; or incorporated into pills or powders; or prepared as a paste or infused in wine.
[Cautions] Contraindicated in cases of Spleen and Stomach deficiency with qi stagnation, excessive phlegm, abdominal fullness, and loose stools.
[Modern Research]
1. Chemical Constituents: Prepared Rehmannia contains small amounts of iridoid glycosides, including catalpol, rehmanniosides A, B, C, D, melittoside, rehmannioside A and D, and chlororehmanniol glycosides. It also contains monoterpenes such as catalpol A, B, C, catalpolide, catalpol furan, and rehmannioside aglycones. Other constituents include trihydroxy β-violet ketone, dihydroxy β-violet ketone, wild rice acid, 5-hydroxy wild rice acid, succinic acid, 15-oxoproline, 5-hydroxymethylfurfural, uracil, and uridine.
2. Pharmacological Effects: Decoctions of prepared Rehmannia promote recovery of red blood cells and hemoglobin in anemic animals, accelerating proliferation and differentiation of hematopoietic stem cells and erythroid progenitor cells in bone marrow. It significantly inhibits hepatic hemorrhagic necrosis and simple necrosis. It suppresses thrombosis formation in hyperlipidemia induced by high-fat diets, fatty liver, and endotoxin-induced hepatic venous hemorrhage in rats. Ether-soluble components reduce peripheral T lymphocytes in mice. Ethanol extracts inhibit mouse hemolytic plaque-forming cells. Alcohol and steam-processed Rehmannia exhibit significant antihypertensive effects, lowering both systolic and diastolic blood pressure. It enhances glutathione peroxidase activity, reducing lipid peroxidation.
3. Clinical Reports: For pruritus, a formula of 300 g prepared Rehmannia, 300 g Salvia miltiorrhiza, and 450 g Cicada slough was ground into fine powder; 3 g was taken orally three times daily for 15 days per course, with two courses administered. Among 35 cases treated, 26 were cured, 8 improved, and 1 was ineffective, with a total effective rate of 97%. [Guangxi Journal of Traditional Chinese Medicine, 1989, (6):45]
