[Overview] This herb is also known as Fuchang, Ziqin, Suqin, and Tiaoqin. It is the root of Scutellaria baicalensis Georgi, a plant of the Lamiaceae family. It is mainly produced in Northeast China, North China, Shanxi, Henan, Shaanxi, Inner Mongolia, and other regions. It is harvested in spring and autumn by digging up, removing fine roots and dirt, then sun-dried after peeling off the coarse bark by pounding. The medicinal material is divided into Ziqin and Kuqin; Ziqin refers to the young secondary roots, while Kuqin refers to the older perennial roots.

[Properties] Bitter in taste, cold in nature. Enters the Lung, Heart, Liver, Gallbladder, and Large Intestine meridians.

[Actions] Clears heat and dries dampness; drains fire and detoxifies; stops bleeding; calms the fetus.

[Applications] 1. Used for damp-warmth and summer-heat dampness, damp-heat fullness and oppression, jaundice, diarrhea, and dysentery. For damp-warmth and summer-heat dampness with heat obstructing the chest and epigastrium causing fullness and nausea, with yellow greasy tongue coating, often combined with talcum, white cardamom, and rice paper plant, as in Huangqin Huashi Tang (Scutellaria and Talcum Decoction). For damp-heat middle burner obstruction with fullness and vomiting, often combined with Coptis, dry ginger, Pinellia, as in Banxia Xiexin Tang (Pinellia Decoction to Drain the Epigastrium). For damp-heat jaundice, often combined with Artemisia capillaris and Gardenia. For damp-heat in the large intestine causing diarrhea and dysentery, often combined with Coptis and Pueraria, as in Gegen Qinlian Tang (Pueraria, Scutellaria and Coptis Decoction).
2. Used for lung heat cough, febrile disease with vexation and thirst, and Shaoyang syndrome. For cough with yellow phlegm due to lung fire, can be used alone as Qingjin Wan (Clear the Metal Pill), or combined with Trichosanthes and Citrus to form Qingqi Huatan Wan (Clear Qi and Transform Phlegm Pill). For Yangming qi-level heat or high fever in warm diseases with irritability, thirst, delirium, hematemesis, and petechiae, often combined with Coptis, Gypsum, and Gardenia. For Shaoyang syndrome with alternating chills and fever, combined with Bupleurum as in Xiao Chaihu Tang (Minor Bupleurum Decoction). For malaria-like chills and fever with predominant heat, combined with Artemisia annua and bamboo shavings, as in Haoqin Qingdan Tang (Artemisia and Scutellaria Decoction to Clear the Gallbladder).
3. Used for bleeding due to reckless movement of blood heat such as hematemesis, epistaxis, bloody stools, and metrorrhagia, often combined with Rehmannia glutinosa, Imperata cylindrica root, Panax notoginseng, and Scrophularia.
4. Used for toxic sores and ulcers, sore throat, scrofula, and scabies. For toxic sores and throat pain, combined with Lonicera japonica, Forsythia suspensa, and Arctium lappa. For scrofula with phlegm nodules, combined with Ostrea, Scrophularia, and Semen Momordicae to make a paste, as in Lvyun Gao (Green Cloud Paste). For damp eczema and scabies with red, itchy, and weeping skin, often ground into powder and mixed with Coptis, Sophora flavescens, Cnidium monnieri, and potassium alum for topical application.
5. Used for fetal heat causing restlessness, irritability, nausea, or vomiting of bitter water with poor appetite, often combined with Atractylodes and Angelica sinensis, as in Danggui San (Angelica Powder). For deficiency heat disturbing the fetus causing unstable pregnancy, fetal movement, and bleeding, combined with Panax ginseng, Rehmannia glutinosa, and Dipsacus as in Taishan Panshi San (Taishan Rock-Solid Powder).

[Dosage and Administration] Internal use: decoction, 3–9 g; or incorporated into pills or powders. External use: appropriate amount, powdered and applied topically or made into a paste for application. For clearing heat, usually used raw; for calming the fetus, often stir-fried; for stopping bleeding, charred. Kuqin is lighter and ascending, mostly used for upper burner fire; Ziqin is heavier and descending, mostly used for lower burner heat.

[Cautions] Contraindicated in cases of spleen and stomach deficiency cold and absence of real fire.

[Modern Research] 1. Chemical constituents: The root contains baicalin, baicalein, wogonin, oroxylin A, chrysin, and other flavonoids. It also contains phenylethanol glucosides, 14 amino acids, volatile oils, and sterols.
2. Pharmacological effects: Baical skullcap decoction inhibits growth and kills various cocci, bacilli, fungi, and pathogens in vitro; methanol extracts, baicalein, baicalin, and wogonin exhibit anti-inflammatory effects; aqueous extracts significantly inhibit passive skin allergic reactions in rats; baicalin and baicalein significantly inhibit passive systemic anaphylaxis in guinea pigs, showing anti-allergic effects; tinctures or alcohol extracts have significant hypotensive and diuretic effects; flavonoids reduce serum cholesterol in experimental hyperlipidemic rats; in vitro, baicalein, wogonin, oroxylin A, baicalin, and chrysin inhibit collagen-induced platelet aggregation in rats; baicalin has mild sedative effects; methanol extracts reduce serum alanine aminotransferase, total bilirubin, and direct bilirubin, indicating hepatoprotective effects; ether extracts show cytotoxicity against leukemia L1210 cells in vitro, indicating antitumor activity.
3. Clinical reports: 1) Pediatric pneumonia: a powder containing baical skullcap and Lonicera japonica (11 g raw herbs per dose), half a dose per day for infants, increasing with age, discontinuing other drugs. Treated 100 cases with 84 cured and 16 improved. [Heilongjiang Journal of Traditional Chinese Medicine, 1986,(5):38] 2) Pediatric bacillary dysentery: equal parts of Coptis, baical skullcap, and Phellodendron, powdered; 1 g per dose for under 1 year old, 2 g for 2–3 years, 3 g for over 4 years, suspended in 30–40 ml saline and retained as an enema once daily, twice daily for severe cases, with dietary control during treatment. Treated 146 cases, 112 cured in 2–6 days, 28 ineffective. [Zhejiang Journal of Traditional Chinese Medicine, 1989,24(3):114] 3) Hordeolum (stye): decoction of Lonicera japonica 20 g and baical skullcap 20 g, one dose daily, decocted twice and taken in divided doses. Treated 150 cases, most cured after 1–2 doses, some cured in 3 days, superior to antibiotics. [Shandong Medicine, 1989,29(11):22] 4) Viral keratitis: decoction of baical skullcap and Houttuynia cordata 50 g each, decocted twice, mixed; two-thirds taken orally in three doses, one-third used to wash affected eyes four times daily. One dose per day for 5 days per course. Treated 30 cases: 19 cured of punctate keratitis; 3 cured and 6 improved of dendritic keratitis; 1 improved and 1 ineffective of geographic keratitis.
