[Overview] This herb is also known as Thorny Tribulus, Tribulus Seed, White Tribulus, and Hard Tribulus. It is the mature fruit of Tribulus terrestris L., a plant of the Tribulaceae family. It is mainly produced in Henan, Hebei, Shandong, Anhui, Jiangsu, Sichuan, Shaanxi, and other regions. The whole plant is harvested when the fruit matures in autumn, then dried in the sun. The fruit is threshed, impurities removed, and dried again.

[Properties] The taste is pungent and bitter; the nature is slightly warm and it has slight toxicity. It enters the Liver meridian.

[Actions] Calms Liver and relieves constraint, activates blood and dispels wind, brightens the eyes, and relieves itching.

[Applications]
1. Used for headache and dizziness. This herb is a key medicine for treating headache and dizziness caused by Liver Yang rising, often combined with Gastrodia (Tianma), Abalone Shell (Shijueming), and Uncaria (Gouteng). For headache caused by wind-heat invasion, it is combined with Mulberry Leaf (Sangye) and Chrysanthemum (Juhua). For headache caused by wind-cold invasion, it is often combined with Angelica Dahurica (Baizhi), Ligusticum (Chuanxiong), and Saposhnikovia (Fangfeng).
2. Used for chest and hypochondriac pain or breast distension due to Liver Qi stagnation, or blood stasis causing amenorrhea and abdominal masses. For Qi stagnation causing chest, hypochondriac, or breast distension pain, it is commonly combined with Cyperus (Xiangfu), Curcuma (Yujin), and Green Tangerine Peel (Qingpi). If breast distension is severe or nodules are present, it can be combined with Vaccaria (Wangbuliuxing), Thorny Acanthopanax (Cishanjia), and Tangerine Leaf (Juye). For amenorrhea and dysmenorrhea caused by Qi stagnation and blood stasis, it is combined with Angelica (Danggui), Ligusticum (Chuanxiong), and Safflower (Honghua). For abdominal masses, it can be combined with Salvia (Danshen), Eupolyphaga (Tubiechong), and Turtle Shell (Biejia).
3. Used for red eyes with visual obstruction, itching, and excessive tearing. For red eyes with tearing, pain, or itching caused by Liver channel wind-heat, it is often combined with Chrysanthemum (Juhua), Cassia Seed (Caocao Ming), Vitex Seed (Manjingzi), and Abutilon Seed (Qingxiangzi), as in the formula Baijiganxili San (白蒺肝息藜散). For eye dryness and itching with blurred vision caused by Liver and Kidney deficiency, it can be combined with Dodder Seed (Tusizi), Goji Berry (Gouqizi), and Polygonatum (Gujingcao).
4. Used for urticaria, skin itching, and vitiligo. For urticaria and itching, it is often combined with Cicada Molting (Chanyi) and Saposhnikovia (Fangfeng). For blood deficiency causing wind, processed Polygonum multiflorum (Zhishouwu), Angelica (Danggui), and Rehmannia (Dihuang) should be added appropriately. For vitiligo, it can be used alone or combined with Lithospermum (Zicao), raw Polygonum multiflorum (Shouwu), and Smilax (Tufuling).
Additionally, this herb is used for sores and abscesses, often combined with Honeysuckle (Jinyinhua), Dandelion (Pugongying), and Red Peony (Chishao); for scrofula, it can be combined with Forsythia (Lianqiao), Scrophularia (Xuanshen), and Thorny Acanthopanax (Chuanshanjia).

[Dosage and Administration] Internal use: decoction, 6–9 g; or incorporated into pills or powders. External use: appropriate amount, decocted for washing; or ground into powder and applied topically.

1. Chemical constituents: The fruit contains tribuloside, furostanol diglycosides, kaempferol, kaempferol-3-glucoside, kaempferol-3-rutinoside, quercetin, vitamin C, and diosgenin saponins. The main fatty acids in the seed oil include palmitic acid, stearic acid, oleic acid, linoleic acid, and linolenic acid.
2. Pharmacological effects: Aqueous and alcohol extracts of Tribulus have antihypertensive effects, which are due to enhanced cholinergic nerve function. Tribulosides have significant anti-myocardial ischemia effects; the main furostanol diglycosides have aphrodisiac effects, enhancing male libido, improving and prolonging erection time, increasing sperm count and motility; for females, they improve ovarian function and are effective for lack of libido and infertility, and can prevent menopausal syndrome. Total saponins from Tribulus have shown anti-aging effects in animal experiments; the alkaloid fraction of the fruit inhibits acetylcholine-induced contractions in isolated rat small intestine and frog abdominal rectus muscle and has diuretic effects.
3. Clinical reports: 1) Vitiligo: Using Tribulus extract (10:1) mixed with sugar powder (1:4) to make granules, 30 g per packet, half a packet per dose, twice daily with warm water. In 27 cases treated, 4 cured, 7 markedly effective, 11 improved, 5 ineffective; total effective rate 81.5%. Use with caution in hypotension and pregnancy. [Hebei Medical Journal, 1981,(2):45] 2) Hand desquamation and itching: 100 g each of White Tribulus and raw Licorice soaked in 300 ml 75% ethanol, filtered. Wash affected area, then apply with cotton ball 2–3 times daily. In 40 cases treated, except 1 interrupted, 39 cured. [Hunan Medical Journal, 1982,(3):11] 3) Pediatric autumnal abdominal diarrhea: 30–60 g Tribulus decocted to about 500 ml, used warm to wash lower limbs and knees, massaging soles, dorsum of feet, and calves for 15–30 minutes, once in morning and evening. In 30 cases, average time to stop diarrhea was 3.11 days; fever and abdominal distension resolved in 2.31 and 3.51 days respectively. [Shandong Medical Journal, 1990,(9):25] 4) Coronary heart disease angina pectoris: Tribulus saponin capsules (Xinshu capsules) or tablets, oral 0.1–0.15 g (2–3 capsules or tablets), taken after meals, two weeks per course. In 406 cases, after 1–2 courses, angina symptoms relieved in 334 cases; total effective rate 82.3%. ECG significantly improved in 41 cases, improved in 173 cases; total effective rate 52.7%.
