[Overview] This herb is also known as Maoji, Qianzhencao, and Ciji Cai. It is the aerial part of the Asteraceae plant Cirsium setosum (Willd.) MB. It is found throughout China. It is harvested in summer and autumn during flowering, impurities are removed, and then it is sun-dried.

[Properties] Taste: sweet and bitter; nature: cold. Enters the Heart and Liver meridians.

[Actions] Cools the blood and stops bleeding; disperses blood stasis and reduces swelling.

[Applications]
1. Used for various bleeding syndromes caused by blood heat. Treats vomiting blood and coughing blood, often combined with Da Ji (Herba Cirsii Japonici), Ce Bai Ye (Folium Platycladi), Da Huang (Radix et Rhizoma Rhei), etc., as in the formula Shi Hui San; treats nosebleed, can be used as a single fresh herb juice or combined with Bai Mao Gen (Rhizoma Imperatae), Xue Yu Tan (charred human blood), Ce Bai Ye, etc.; treats bloody stools, often combined with Di Yu (Radix Sanguisorbae), Huai Hua (Flos Sophorae), Huang Lian (Rhizoma Coptidis), etc.; treats blood lin syndrome and hematuria, combined with Sheng Di Huang (Radix Rehmanniae), Ou Jie (Rhizoma Nelumbinis), Mu Tong (Caulis Akebiae), as in the formula Xiao Ji Yin Zi; treats continuous uterine bleeding in women due to excess heat, often combined with Qian Cao (Radix Rubiae), Pu Huang (Pollen Typhae), Huang Qin (Radix Scutellariae); for bleeding due to deficient fire, often combined with Sheng Di Huang, Gui Ban (Carapax Testudinis), Mo Han Lian (Eclipta prostrata); for traumatic bleeding, fresh juice can be applied externally.
2. Used for heat toxin-induced carbuncles, sores, and ulcers. Internally, it can be combined with Pu Gong Ying (Herba Taraxaci), Zi Hua Di Ding (Herba Violae), Zhao Xiu (Herba Patriniae) for decoction; externally, fresh herb is washed, crushed, and applied to the affected area. Decoction used as a wash can also treat vaginal itching in women. Currently, it is commonly used clinically for hypertension, damp-heat jaundice, hepatitis, nephritis, and other conditions.

[Dosage and Administration] Internal use: decoction, 4.5–9 g; or juice taken orally. External use: appropriate amount, fresh herb crushed and applied or dry herb powdered and applied.

[Cautions] Contraindicated in patients with spleen and stomach deficiency cold.

[Modern Research]
1. Chemical constituents: The whole flowering herb contains quercetin, protocatechuic acid, chlorogenic acid, caffeic acid, potassium chloride, mongolicin, acacetin, amino acids, taraxasterol, triacontanol, β-sitosterol, stigmasterol, and others.
2. Pharmacological effects: Herb Cirsii Segeti can constrict blood vessels and shorten bleeding time and thrombin time, thus having good hemostatic effects; decoctions or tinctures have adrenaline-like hypertensive effects, though some reports indicate hypotensive effects; it has a marked excitatory effect on isolated frog and rabbit hearts and causes vasoconstriction in isolated rabbit ears and rat hind limbs; its decoction inhibits hemolytic streptococcus, pneumococcus, diphtheria bacillus, and Mycobacterium tuberculosis; additionally, it has lipid-lowering, choleretic, and cardiotonic effects.
3. Clinical reports:
1) Early postoperative bleeding after gastrectomy: 30 g of Herb Cirsii Segeti was decocted in 800 ml water over low heat, filtered, concentrated to 200 ml, cooled to room temperature; 100 ml was injected into the stomach via gastric tube, tube closed for half an hour, then gastric juice was suctioned. If no fresh bleeding, low-pressure drainage was applied; if fresh blood was present, another 100 ml was injected and drained after half an hour. In 11 cases treated, 7 stopped bleeding after one treatment, 2 after two treatments, 2 were ineffective. [Zhejiang Journal of Traditional Chinese Medicine, 2001, 36(7):285]
2) Boils: skin disinfected with 2.5% iodine tincture and 75% ethanol, then appropriate amount of Xiaoji ointment (prepared by decocting 500 g Xiaoji in 1500 ml water for 8–10 minutes, filtering, then decocting residue in 1000 ml water for 5–8 minutes, mixing both decoctions and concentrating to ointment) applied to affected area, covered with gauze, changed once daily for 5–8 days. 3) Primary hypertension: 500 g dry Xiaoji, 500 g raw peanut with red skin, 250 ml white liquor, and 1000 ml rice vinegar prepared into “Xiaoji decoction”; peanut, liquor, and vinegar prepared into “wine vinegar peanut” and “peanut wine” for use. Patients took 10 wine vinegar peanuts each morning, and 10 ml Xiaoji decoction plus 10 ml peanut wine diluted with 100 ml boiled water each evening. Among 100 cases, 75 cured, 20 improved, 5 ineffective; total effective rate 95%. [Forum of National Medicine, 1989; 4(5):31]
4) Refractory insomnia: 6 g dry or 10 g fresh Xiaoji soaked in 30–50 ml boiled water for 10 minutes, taken before sleep for 2 months. Among 56 cases, efficacy was definite. [Shandong Journal of Traditional Chinese Medicine, 1995, (9):14]
