[Overview] This herb is also known as Xiangfu Mi, Xiangfuzi, Cyperus Root, and Sanlengcao Root. It is the rhizome of Cyperus rotundus L., a plant of the Cyperaceae family. It is mainly produced in Shandong, Zhejiang, Fujian, Hunan, Henan, and other regions. The rhizomes are harvested in spring and autumn, the fine hairs are removed by slow fire roasting, then briefly boiled or steamed thoroughly in boiling water before drying in the sun.
[Properties] Taste: pungent, sweet, slightly bitter; nature: neutral. Enters the Liver, Spleen, and Sanjiao meridians.
[Actions] Regulates Qi and relieves constraint; regulates menstruation and alleviates pain; calms the fetus.
[Applications]
1. Used for hypochondriac distending pain caused by Liver Qi stagnation, breast distension and pain, hernia pain. Commonly combined with Curcuma, Corydalis, Bupleurum, and White Peony Root. If Liver meridian Qi stagnation is complicated by damp-heat, symptoms include hypochondriac pain, fever, and jaundice; often combined with Scutellaria, Artemisia capillaris, and Rhubarb. For women with Liver Qi stagnation causing premenstrual breast distension, often combined with Angelica sinensis and White Peony Root, as in the formula Kaiyu Zhongyu Tang (开郁种玉汤). For hernia pain predominantly due to Qi stagnation, combined with Citrus seed, Toosendan fruit, and Fennel, as in Heju Wan (核橘丸); if due to cold congealing, combined with Evodia and Fennel, as in Juhe Wan (橘核丸).
2. Used for epigastric distension and pain caused by Liver Qi invading the Stomach, loss of appetite, acid regurgitation, vomiting, and nausea. Often combined with Amomum and Licorice, as in Kuaiqi Tang (快气汤). If Qi stagnation with cold congealing causes stomach pain relieved by warmth and pressure, combined with Alpinia officinarum in pill form, as in Liangfu Wan (良附丸). If Liver fire invades the Stomach, often combined with Gardenia, as in Xiangfu San (香附散). For Spleen deficiency and weak Stomach with Liver Qi stagnation, symptoms include epigastric and abdominal fullness, poor appetite, and nausea; often combined with Ginseng, Atractylodes, and White Cardamom, as in Xiangsha Yangwei Tang (香砂养胃汤).
3. Used for irregular menstruation, dysmenorrhea, and amenorrhea. Xiangfu is a key herb for regulating menstruation and treating gynecological disorders, often combined with Angelica sinensis and White Peony Root. For blood heat and stasis causing irregular menstruation or dysmenorrhea, combined with Red Peony Root, Moutan Cortex, Leonurus, and Angelica sinensis. For uterine cold with lower abdominal cold pain and irregular menstruation, combined with Mugwort leaf, Cinnamon twig, and Evodia.
4. Used for restless fetus and pregnancy-related vomiting, often combined with Agastache, as in Erxiang San (二香散). For threatened miscarriage with fetal restlessness, combined with Rehmannia glutinosa, Donkey-hide gelatin, Angelica sinensis, Atractylodes, and Eucommia, as in Qianjin Baotai Wan (千金保胎丸).
Additionally, this herb can be used for scrofula, lumps, abscesses, and traumatic injuries, utilizing its Qi-regulating, blood-activating, and swelling-reducing properties.
[Dosage and Administration] Internal use: decoction, 6–9 g; or incorporated into pills or powders. External use: appropriate amount, ground into powder and applied topically. For regulating Qi and relieving pain, processed Xiangfu is preferred; for hemostasis, charred Xiangfu is used.
[Cautions] Not suitable for use alone in cases of blood deficiency and Qi weakness. Use with caution in Yin deficiency with blood heat.
[Modern Research]
1. Chemical constituents: The rhizome contains volatile oils including β-pinene, camphene, cineole, limonene, cyperene, α- and β-cyperone, patchoulene ketone, cyperotundone, cyperol, cyperol ketone, and caryophyllene. It also contains glucose, fructose, starch, and other components.
2. Pharmacological effects: The volatile oil of Xiangfu exhibits estrogen-like effects; subcutaneous or vaginal administration induces complete keratinization of vaginal epithelial cells. Xiangfu tincture inhibits isolated uterine contractions in various animals, whether pregnant or not. The aqueous ethanol precipitate has cardiotonic and heart rate slowing effects and can lower blood pressure. Alcohol extracts significantly inhibit rat paw edema induced by carrageenan and formaldehyde. The volatile oil inhibits Staphylococcus aureus; rhizome extracts have antimalarial activity. Decoctions promote bile secretion in rats and protect liver cells. Xiangfu also has antipyretic, sedative, analgesic, antiemetic, and antibronchospasm effects.
3. Clinical reports: 1) Chronic gastritis and peptic ulcer: Using a formula mainly composed of Xiangfu and Perilla (Biantong Xiangsu San), 185 cases were treated, including 122 chronic gastritis cases with a total effective rate of 95.9%, and 63 peptic ulcer cases with a total effective rate of 93.7%. [Hebei Journal of Traditional Chinese Medicine, 1989, 11(3):37] 2) Dysmenorrhea: Using a formula mainly composed of Xiangfu and Angelica sinensis (Zhitong San), 56 cases of primary dysmenorrhea were treated, with 35 cured, 19 improved, 2 ineffective, total effective rate 97.4%. [Journal of Guiyang College of Traditional Chinese Medicine, 1989,(4):15] 3) Acute cystitis: Fresh Xiangfu 30 g was decocted with 300 ml water to 200 ml, decocted twice per dose, two doses combined and taken once, twice daily. Ninety-eight cases treated, 92 cured within 3 days. [Zhejiang Journal of Traditional Chinese Medicine, 1992, 27(2):82] 4) Flat warts: Xiangfu 500 g and horsetail 250 g were ground into coarse powder, soaked in 70% ethanol for 10 days to make a tincture, applied to lesions twice daily. Forty-five cases treated; after 2 weeks, 28 cured, 5 markedly effective, 8 effective, 4 ineffective; total effective rate 91.1%.
