[Overview] This herb is also known as False Coptis or Hu Lian. It is the rhizome of Picrorhiza scrophulariiflora Pennell, a plant of the Scrophulariaceae family. It is mainly produced in Tibet. Harvested in autumn when the aboveground parts wither, the rhizome is dug up, with fibrous roots and soil removed, then cleaned and sun-dried.
[Properties] Bitter in taste, cold in nature. Enters the Liver, Stomach, and Large Intestine meridians.
[Actions] Clears deficiency heat, eliminates nutritional heat, clears damp-heat, detoxifies.
[Applications] Heat disorders:
1. Used for yin deficiency internal heat or pediatric nutritional heat. For treating yin deficiency bone steaming and tidal fever with night sweats, it is often combined with Yin Chai Hu (Stellaria root) and Di Gu Pi (Lycium bark), as in Qing Gu San (Clear Bone Powder). For pediatric nutritional accumulation with fever, indigestion, abdominal distension, emaciation, and irregular stools, it can be combined with Dang Shen (Codonopsis), Bai Zhu (Atractylodes), Shi Jun Zi (Quisqualis seed), and Shan Zha (Hawthorn), as in Fei Er Wan (Fat Child Pill).
2. Used for damp-heat dysentery or jaundice. For dysentery, often decocted with Bing Lang (Areca seed) and Bai Shao (White Peony root); for jaundice, combined with Yin Chen (Artemisia), Shan Zhi (Gardenia), and Yu Jin (Curcuma).
3. Used for hemorrhoids, carbuncles, and painful swellings; the powdered herb can be applied topically to affected areas. For fistula-in-ano, it is often combined with Chuan Shan Jia (Pangolin scale) and Huai Hua (Sophora flower) and decocted together.
Additionally, this herb can clear heat and drain fire, treating liver fire causing red, swollen eyes and mouth or tongue ulcers; it also cools the blood and stops bleeding, used with Sheng Di Huang (Rehmannia root) and Zhu Dan Zhi (Pig bile) to treat bleeding due to blood heat such as hematemesis and epistaxis.
[Dosage and Administration] Internal use: decoct 6–10 g; or used in pills or powders. External use: appropriate amount powdered and applied topically; or juice extracted for eye drops.
[Cautions] Use with caution in patients with spleen and stomach deficiency.

1. Chemical constituents: The root contains picroside, D-mannitol, vanillic acid, picroside alcohol, picroside sterol, and vanillin ketone. Bitter components isolated include picroside I and picroside II.
2. Pharmacological actions: Picroside II has hepatoprotective effects in mice with carbon tetrachloride-induced liver toxicity and choleretic effects in rats; aqueous extracts of the root or rhizome inhibit immune function; a 1:4 aqueous extract inhibits various skin fungi in vitro.
