[Overview] This herb is also known as Ren Shen San Qi, Shen San Qi, Dian San Qi, and Tian San Qi. It is the root of Panax notoginseng (Burk.) F.H.Chen, a plant of the Araliaceae family. It is mainly produced in Tibet, Sichuan, Yunnan, Guangxi, and Hunan, with extensive cultivation in southeastern Yunnan and southwestern Guangxi. It is harvested in autumn before flowering, then cleaned and separated into main root, lateral roots, and stem base, and sun-dried. The lateral roots are commonly called “tendons,” and the stem base is called “cut mouth.”
[Properties] Sweet, slightly bitter in taste; warm in nature. Enters the Liver and Stomach meridians.
[Actions] Disperses blood stasis and stops bleeding; reduces swelling and alleviates pain.
[Applications]
1. Used for various bleeding disorders. Treats internal injury bleeding such as coughing blood, vomiting blood, nosebleed, hematuria, hematochezia, metrorrhagia, etc. It can be taken alone as a powder or combined with Huairui Shi (Flos Chrysanthemi Indici) and Xue Yu Tan (charred blood residue), as in the formula Huaxue Dan. For bleeding due to blood heat, it is combined with Sheng Di Huang (Rehmanniae Radix), Bai Mao Gen (Imperatae Rhizoma), Ce Bai Ye (Platycladi Cacumen), etc. For yin-deficiency blood heat, combined with Han Lian Cao (Ecliptae Herba), E Jiao (Asini Corii Colla), Gui Ban (Testudinis Plastrum), etc. For deficiency-cold bleeding, combined with Shan Zhu Yu (Corni Fructus), Xian He Cao (Agrimoniae Herba), Pao Jiang (Zingiberis Rhizoma Preparatum), etc. For qi-deficiency bleeding, combined with Huang Qi (Astragali Radix), Dang Shen (Codonopsis Radix), Zao Xin Tu (Cinerariae Herba). For heat injuring lung collaterals with blood stasis, chest pain, dry cough with blood in sputum, combined with Bai Mao Gen, Da Huang (Rhei Radix et Rhizoma), Long Gu (Fossilia Ossis Mastodi), etc. For external bleeding, powdered San Qi can be applied directly to the bleeding site or combined with Long Gu, Ru Xiang (Olibanum), Mo Yao (Myrrha), Jiang Xiang (Dalbergiae Lignum).
2. Used for traumatic injuries such as bruises, sprains, fractures, chest pain due to blood stasis, amenorrhea from blood stasis, dysmenorrhea, and postpartum abdominal pain due to blood stasis. For traumatic injury with swelling and pain, used alone or combined with Dang Gui (Angelicae Sinensis Radix), Hong Hua (Carthami Flos), Tu Bie Chong (Eupolyphaga Steleophaga), Ru Xiang, Mo Yao, She Xiang (Moschus), Bing Pian (Borneol), etc. For blood stasis causing chest pain, combined with Quan Gua Lou (Trichosanthis Fructus), Xie Bai (Allii Macrostemonis Bulbus), Gui Zhi (Cinnamomi Ramulus); if qi deficiency is present, add Ren Shen, Huang Qi. For blood stasis amenorrhea and dysmenorrhea with yang deficiency and cold, combined with Gui Zhi, Ai Ye (Artemisiae Argyi Folium); if qi and blood deficiency, combined with Huang Qi, Dang Gui. For postpartum blood stasis abdominal pain and retained lochia, combined with Chuan Xiong (Chuanxiong Rhizoma), Dang Gui, Tao Ren (Persicae Semen).
3. Used for carbuncles and sores. For early-stage sores without pus, San Qi powder can be applied externally and taken internally. For ulcers that have broken and not healed, combined with Er Cha (Catechu), Xue Jie (Dragon’s Blood), Ru Xiang, Mo Yao powders, as in the formula Fu Jin Sheng Ji San.
Additionally, San Qi has tonic and strengthening effects and can be cooked with chicken to treat qi and blood deficiency with fatigue and dizziness.
[Dosage and Administration] Internal use: decoction 3–10 g; powder 1–3 g; or incorporated into pills or powders. External use: appropriate amount of powder mixed or applied topically; or juice applied externally.
[Cautions] Use with caution during pregnancy.
[Modern Research]
1. Chemical constituents: San Qi contains ginsenosides, notoginsenosides, jiaogulan saponins, various amino acids, quercetin, sugars, β-sitosterol, carotene, inorganic elements, etc. The water-soluble fraction of the root contains the hemostatic active component Tianqi amino acid. San Qi also contains anticancer polyacetylenes such as ginsentriol.
2. Pharmacological effects: San Qi has strong hemostatic effects, shortening coagulation time and prothrombin time, but total saponins inhibit platelet aggregation and have thrombolytic effects. Notoginsenosides and flavonoids counteract myocardial ischemia, lower blood pressure, protect the heart, and regulate immune function. It also has anti-inflammatory, hypoglycemic, and antitumor effects.
3. Clinical reports: 1) Upper gastrointestinal bleeding: San Qi powder 1.5 g orally three times daily with warm water. In 60 cases of various types of gastric bleeding, 58 cases achieved complete hemostasis, 2 cases ineffective. [Yunnan Journal of Traditional Chinese Medicine, 1985,(11):28] 2) Central retinal vein occlusion: San Qi injection 2–6 ml daily (each ml contains 50 mg total saponins) added to 40 ml 50% glucose injection, intravenous injection, 10 injections per course, 2–3 courses per patient. In 64 cases, 14 markedly effective, 38 effective, 12 ineffective. [New Medicine, 1983,14(3):138] 3) Coronary heart disease: San Qi extract oral preparation San Qi Guan Xin Ning, 0.6–1.2 g daily. In 828 cases, total effective rate for angina was 70.2%, marked effect 20.1%. [Chinese Herbal Medicine, 1980,(10):39] 4) Hepatitis: San Qi injection (2 ml per vial, containing 1 g crude drug) intramuscular or intravenous drip in 500 ml 10% glucose, treating 65 cases of refractory blood stasis type chronic hepatitis, total effective rate 80%. [Shanghai Journal of Traditional Chinese Medicine, 1983,(8):13] 5) Common warts: San Qi powder 0.5–1 g orally three times daily, 13 cases treated, total effective rate 92.3%. [Shanghai Journal of Traditional Chinese Medicine, 1990,(3):20]
