[Overview] This item is also known as Substitute Haematite, Nail-headed Haematite, Red Haematite, and Purple Vermilion. It is an oxide mineral belonging to the corundum group, specifically Haematite. It is mainly produced in Shanxi, Hebei, Henan, Shandong, Hunan, Sichuan, and Guangdong provinces. It can be harvested year-round. After collection, select parts with ‘nail-head’ surface features and remove soil and miscellaneous stones.

[Properties] Bitter in taste, cold in nature. Enters the Liver, Stomach, and Heart meridians.

[Actions] Calms Liver Yang and suppresses yang rising; strongly sedates and reverses counterflow; cools blood and stops bleeding.

[Applications]
1. Used for dizziness, vertigo, eye distension, and tinnitus caused by Liver Yang hyperactivity. Commonly combined with Achyranthes root (Niuxi), raw Dragon bone (Longgu), and raw Oyster shell (Muli). Formula example: Zhen Gan Xi Feng Tang (镇肝熄风汤). For symptoms caused by Liver fire flaring upward, often combined with Chrysanthemum flower (Juhua), Prunella spike (Xiakucao), Scutellaria root (Huangqin), and Uncaria vine (Gouteng). If accompanied by Liver and Kidney Yin deficiency, often combined with Rehmannia root (Shengdi), White peony root (Baishao), Oyster shell (Muli), and Turtle shell (Guijia).
2. Used for epilepsy and mania. If caused by wind-fire with phlegm leading to epilepsy and mania, often combined with Coptis rhizome (Huanglian), Rhubarb (Dahuang), Polygala root (Yuanzhi), and Gallstone powder (Danxing). If caused by wind-phlegm rising counterflow leading to epilepsy, can be combined with Curcuma (Yujin), Acorus (Changpu), Alum (Mingfan), and Cinnabar (Zhusha). Also used for palpitations, insomnia, excessive dreaming, and infant night crying.
3. Used for vomiting, hiccup, and belching caused by rebellious Stomach Qi. If due to excessive Stomach heat, often combined with Liver-relieving Gypsum (Shixi Gengao) and Scrophularia root (Xuanshen). If accompanied by Gallbladder fire rising, can be combined with Gentiana (Longdancao) and Indigo naturalis (Qingdai). Formula example: Zhen Feng Ni Tang (镇风逆汤). If due to cold congealing in the middle burner, combined with Dried ginger (Ganjiang) and Clove (Dingxiang). If due to phlegm turbidity obstructing the middle, often combined with Inula flower (Xuanfuhua) and Pinellia (Banxia). If accompanied by Stomach Qi deficiency, add Ginseng (Renshen) and Jujube (Dazao). Formula example: Xuanfu Dai Zhe Tang (旋覆代赭汤).
4. Used for cough and asthma caused by rebellious Lung Qi. If due to phlegm-damp obstructing the Lung with cough and sputum, combined with Pinellia (Banxia), Tangerine peel (Chenpi), and Inula flower (Xuanfuhua). If due to Lung heat accumulation, often combined with Mulberry root bark (Sangbaipi) and Trichosanthes fruit (Gualou). If due to deficiency below and excess above causing asthma, combined with Ginseng (Renshen), Chinese yam (Shanyao), and Cornus (Shanzhuyu).
5. Used for bleeding due to blood heat such as vomiting blood, nosebleed, uterine bleeding, and bloody stools. For vomiting and nosebleed, use this item calcined with fire and vinegar, ground into powder and taken orally, or combined with Rehmannia root (Shengdi), Moutan bark (Danpi), and large thistle (Daji). For bloody stools, often combined with Sanguisorba (Diyu) and Sophora flower (Huaihua). For hematuria, combined with Small thistle (Xiaoji) and Imperata root (Baimaogen). For uterine bleeding, often combined with Rubia root (Qian Cao Gen), charred mugwort leaf (Aiye Tan), and donkey-hide gelatin (Ejiao). When combined with Dried ginger (Ganjiang) and Atractylodes (Baizhu), it can also treat bleeding due to deficiency cold. Formula example: Wen Jiang Tang (温降汤).

[Dosage and Administration] Internal use: decoction, 9–15 g, crushed and wrapped in cloth, decocted first; or used in pills or powders. External use: appropriate amount, ground into powder and sprinkled or applied as a paste. Generally used raw; calcined for hemostasis.

[Cautions] Use with caution in cases of deficiency cold and during pregnancy.

1. Chemical constituents mainly include iron(III) oxide (Fe2O3), with impurities such as silicon (Si), aluminum (Al), titanium (Ti), magnesium (Mg), manganese (Mn), calcium (Ca), lead (Pb), and arsenic (As).
2. Pharmacological effects: suspensions of raw and stir-fried Haematite administered by gastric gavage both increase white blood cell counts; the raw form shows a significantly higher effect than the stir-fried form (P<0.01). 3. Clinical reports: For intractable constipation, 30 g each of Substitute Haematite and Aloe were ground into fine powder and made into pills, 6 g per dose.
