[Overview] This herb is also known as Zhuer Shen, Wild Sanqi, Earth Sanqi, and Niuziqi. It is the rhizome of Panax japonicus C.A. Mey. var. major (Burk.) C.Y. Wu et K.M. Feng or Panax japonicus C.A. Mey. var. bipinnatifidus (Seem.) C.Y. Wu et K.M. Feng, plants of the Araliaceae family. It is mainly produced in Yunnan, Shaanxi, Gansu, Sichuan, and other regions. The rhizomes are harvested in autumn, cleaned of soil, outer skin, and fibrous roots, then dried or steamed and dried.

[Properties] Bitter and sweet in taste, slightly cold in nature. Enters the Liver, Lung, and Stomach meridians.

[Actions] Tonifies the Lung and nourishes Yin, disperses blood stasis and stops bleeding, reduces swelling and relieves pain.

[Applications]
1. Used for Qi and Yin deficiency with symptoms of vexation, thirst, and chronic cough due to deficiency. For heat injuring fluids and Qi-Yin insufficiency with vexing heat and thirst, it can be combined with Ophiopogon japonicus (Maimendong), Trichosanthes root (Tianhuafen), Anemarrhena (Zhimu), etc.; for Lung Yin deficiency and chronic dry cough, it can be combined with Glehnia (Shashen), Anemarrhena (Zhimu), Fritillaria (Beimu), etc.
2. Used for various bleeding disorders such as hematemesis, epistaxis, hemoptysis, hematochezia, hematuria, metrorrhagia, and traumatic bleeding, especially suitable for bleeding caused by Yin deficiency and blood heat. Mild cases respond well to single use; severe cases may be combined with Biota leaf (Ce Bai Ye), large and small thistle (Da Xiao Ji), Imperata root (Bai Mao Gen), etc.
3. Used for traumatic injury swelling and pain, rheumatic joint pain, and stomach pain. For traumatic swelling and pain, it can be combined with Safflower (Honghua), Angelica (Danggui), Dragon’s blood (Xuejie), etc.; for chronic painful obstruction with blood stasis in the collaterals, it can be combined with Peach kernel (Taoren), Safflower (Honghua), Haifengteng, Gentiana macrophylla (Qinjiao), etc.; for chronic stomach pain with blood stasis, it can be combined with Nine fragrant insect (Jiuxiangchong), Angelica (Danggui), Purple Salvia (Zidanshen), Corydalis (Yuanhu), etc.
4. Used for wind-fire toothache, sore throat swelling and pain, and carbuncles and abscesses. For wind-fire toothache, often combined with Asarum (Xixin), Gypsum (Shigao), Angelica dahurica (Baizhi), etc.; for sore throat swelling and pain, can be combined with Platycodon (Jiegeng), Licorice (Gancao), Arctium lappa seed (Niubangzi), etc.; for carbuncles and abscesses, combined with Wild chrysanthemum (Yejuhua), Dandelion (Pugongying), Viola (Zihuadiding), etc.

[Dosage and Administration] Internal use: decoction, 3–9 g; or incorporated into pills or powders; or soaked in wine. External use: appropriate amount, ground into powder and applied dry or mixed and applied topically; or soaked in wine and rubbed on; or fresh herb mashed and applied as a poultice.

[Modern Research]
1. Chemical constituents: The rhizome contains various saponins such as Panax japonicus saponins Iva and V, ginsenosides Rd, Re, Rg2, Notoginseng saponin R2, Zhuzishen saponins R1 and R2, β-sitosterol 3-O-D-glucopyranoside. It also contains oleanolic acid, succinic acid, β-sitosterol, and glycoprotein ZPZ.
2. Pharmacological effects: Total saponins from Rhizoma Panacis Majoris enhance immune function, similar to ginsenosides; Panax japonicus F (a purified monomer from total saponins) has anti-lipid peroxidation effects; total saponins have sedative and analgesic effects; methanol extract (PME) saponin fraction and Panax japonicus saponin III have anti-ulcer effects; also exhibits anti-arrhythmic effects.
3. Clinical reports: Leukopenia—using Zhuzishen tablets (1.25 g per tablet), 5 g per dose, three times daily. In a total of 30 cases treated, 20 cases showed marked effect, 4 cases effective, 6 cases ineffective, with an overall effective rate of 80%, including 2 cases with significant platelet increase.
